Gastric dilatation in patients with restrictive eating disorders.

OBJECTIVES: To better understand gastric dimensions in patients diagnosed with restrictive eating disorders (EDs). METHOD: In this retrospective study, 56 patients, with restrictive EDs, and 60 gender/age/race-matched patients from an outpatient clinic, were studied. Difference in stomach size, between cohorts, was ascertained, and regression analyses were used to examine associations with stomach size in the ED cohort. RESULTS: Patients with EDs were found to have significantly enlarged gastric dimensions when compared to the control group (M:14.8 cm [SD: 3.2] vs. 11.4 cm [SD: 2.9], p < .00

Emotional Deprivation in Children: Growth Faltering and Reversible Hypopituitarism.

Emotional deprivation can lead to growth faltering of infants and children. The mechanism(s) involved differ in that for infants, the major metabolic problem is inadequate energy intake for growth. In young children, it is likely that the emotional deprivation causes a syndrome not only of growth faltering, but with bizarre behaviors, especially with regard to food: hoarding, gorging and vomiting, hyperphagia, drinking from the toilet, and eating from garbage pails. Other disturbed behaviors include, poor sleep, night wanderings, and pain agnosia. The pathophysiology appears to be reversible h

On how to feed critically ill children in intensive care: A slowly shifting paradigm.

Critically ill children requiring treatment in a pediatric intensive care unit (PICU) suffer from anorexia and/or feeding intolerance. The resulting macronutrient deficit associates with poor outcome. Until recently, this association formed the basis for initiating enteral or parenteral feeding early to improve outcome. The multicenter "Early-versus-Late-Parenteral-Nutrition-in-the-Pediatric-Intensive-Care-Unit" randomized controlled trial (PEPaNIC-RCT) addressed whether this association is causal. It showed that early supplementation of insufficient/contraindicated enteral nutrition with pare

Audit of parenteral nutrition use in Guernsey.

OBJECTIVES: This audit was precipitated by a huge increase in the local prescribing of parenteral nutrition (PN) in Guernsey and concomitant burden on pharmacy/sterile services unit staff. It aimed to establish whether PN was being used appropriately and the extent to which feeding prescriptions followed U.K. National Institute for Health and Clinical Excellence and local Guernsey guidelines with regard to the initiation, monitoring and cessation of feeding. METHOD: All PN patients at the Princess Elizabeth Hospital in Guernsey during a 1 year period (November 2006-December 2007) were subject

The potential effect of Ramadan fasting on musculoskeletal diseases: new perspectives.

Ramadan, the ninth month of the Muslim lunar calendar, is a period of intermittent fasting alternated with moments of refeeding. The last decades have seen a growing number of reports that examine the potential effect of Ramadan intermittent fasting (RIF) on chronic musculoskeletal disorders. In this paper, we reviewed data that assessed the relationship of intermittent diurnal fasting during Ramadan with rheumatic diseases. Currently, recent evidence indicates that RIF may attenuate the inflammatory state by suppressing pro-inflammatory cytokine expression and reducing the body fat and the ci

[Contemporary criteria of the diagnosis and current recommendations for nutritional therapy in anorexia nervosa].

The basic criterion for the diagnosis of anorexia (AN - anorexia nervosa) by ICD-10 (International Classification of Diseases, version 10) is the body weight less than 15% of the expected normal body weight. According to DSM-IV (Diagnostic and Statistical Manual for Mental Disorders, version IV) the basic feature of AN is a refusal to maintain body weight equal or greater than the minimal normal weight. The prevalence of anorexia nervosa is 0.3-0.5% or even 1.3-3.7% if include pre-anorexic states (eg. the phenomenon of pro-ana). The main feature of anorexia is a reduction of caloric intake. Ac

[Impact of the creation of a nutritional support team on the quality, safety and effectiveness of total parenteral nutrition].

INTRODUCTION: Parenteral nutrition (PN) is an essential component in the treatment of many hospitalized patients; however its use is associated to severe complications. The monitoring of these patients has been demonstrated as an effective method for the success of nutrition therapy; therefore, nutritional support teams play an important role to increase the effectiveness and efficiency of the PN. In Spain, there is no data available about the situation of nutritional support teams in hospitals. OBJECTIVE: Develop a nutritional support team in our hospital and analyze the interventions and ou

Nutritional effects and management of diarrhoea in infancy.

The interactions between diarrhoeal disease and nutritional status are complex and synergistic. These are serious issues globally because they affect hundreds of millions of young children and annually cause > 3 million deaths in children aged under 5 y. Despite intensive field-based and laboratory studies over three decades, many questions remain unanswered about the causes, pathophysiology and best approaches to management and prevention of this "diarrhoea-malnutrition" syndrome. Oral rehydration therapy (ORT) has been a major advance and has saved many lives from acute diarrhoea. However, p

Wearable cardioverter defibrillator prevented sudden cardiac death in a pediatric case of takotsubo cardiomyopathy in Japan.

UNLABELLED: As an alternative to implantable cardioverter defibrillators, the wearable cardioverter defibrillator (WCD; LifeVest®, Asahi KASEI Zoll Medical Co., Tokyo, Japan) is the only noninvasive cardioverter defibrillator available that can be worn directly on the skin. The first consensus statement on WCD use in Japan was published in 2014, and, similar to guidelines in other countries, its recommendations focus on adult patients. Pediatric cases involving WCD remain limited, and appropriate indications for its use in children are yet to be established. Here, we report the first pediatric

2022 ISFM Consensus Guidelines on Management of the Inappetent Hospitalised Cat.

PRACTICAL RELEVANCE: Inappetence may have many origins and, as a presenting sign or observation in the hospitalised patient, is common in feline practice. Nutritional assessment of every patient is encouraged, to identify the need for, and appropriate type of, intervention indicated. The impact of malnutrition may be significant on the feline patient, perpetuating illness, delaying recovery, slowing wound healing and negatively impacting gut health and immunity. Delayed intervention may result in the cat's deterioration; hence prompt control of contributing factors such as the underlying illne

Nutritional support for severe malnutrition in newly diagnosed Crohn's disease: a case report.

Crohn's disease (CD) and ulcerative colitis are chronic inflammatory bowel diseases (IBDs) characterized by relapsing exacerbations related to a dysregulated immune response, resulting primarily in gastrointestinal symptoms and also frequently in signs of chronic systemic inflammation. Malnutrition is a possible complication of IBD with a prevalence ranging from 20 to 85%, varying with disease activity, location, and age, and is recognized as having a multifactorial pathogenesis. Malnutrition in patients with IBD should be diagnosed early and treated appropriately because it can heavily affect

Correlation Between Hypophosphatemia and Hyperventilation in Critically Ill Patients: Causes, Clinical Manifestations, and Management Strategies.

Hypophosphatemia, defined as serum phosphate levels below 2.5 mg/dL, is a common yet underrecognized electrolyte disturbance in critically ill patients, with prevalence estimates reaching up to 80%. This review explores the intricate bidirectional relationship between hypophosphatemia and hyperventilation, emphasizing its profound implications for respiratory function and critical care management. Hypophosphatemia impairs oxygen delivery by depleting 2,3-diphosphoglycerate (2,3-DPG), disrupts central respiratory drive, and weakens respiratory muscles, leading to hyperventilation, ventilatory f
