Mapping of Orthopaedic Fractures for Optimal Surgical Guidance.

Orthopaedic fractures may be difficult to treat surgically if accurate information on the fracture propagation and its exit points are not known. Even with two-dimensional (2D) radiographic images, it is difficult to be completely certain of the exact location of the fracture site, the fracture propagation pattern and the exit points of the fracture. Three-dimensional (3D) computerised tomographic models are better in providing surgeons with the extent of bone fractures, but they may still not be sufficient to allow surgeons to plan open reduction and internal fixation (ORIF) surgery.Fracture

A new concept of the anatomy of the thoracic oesophagus: the meso-oesophagus. Observational study during thoracoscopic esophagectomy.

BACKGROUND: During thoracoscopic oesophageal surgery, we observed not previously described fascia-like structures. Description of similar structures in rectal cancer surgery was of paramount importance in improving the quality of resection. Therefore, we aimed to describe a new comprehensive concept of the surgical anatomy of the thoracic oesophagus with definition of the meso-oesophagus. METHODS: We retrospectively evaluated 35 consecutive unedited videos of thoracoscopic oesophageal resections for cancer, to determine the surgical anatomy of the oesophageal fascia's vessels and lymphatic dr

Efficacy of a virtual reality-based basic and clinical fused curriculum for clinical education on the lumbar intervertebral disc.

OBJECTIVE: Today, minimally invasive procedures have become mainstream surgical procedures. Percutaneous endoscopic transforaminal discectomy for lumbar disc herniation (LDH) requires profound knowledge of the laparoscopic lumbar anatomy. Immersive virtual reality (VR) provides three-dimensional patient-specific models to help in the process of preclinical surgical preparation. In this study, the authors investigated the efficacy of VR application in LDH for training orthopedic residents and postgraduates. METHODS: VR images of the lumbar anatomy were created with immersive VR and mAnatomy so

Surgical Consideration in Total Knee Arthroplasty for a Patient With Incidental Femoral Intramedullary Fibrous Dysplasia.

Fibrous dysplasia (FD) is an uncommon, benign bone disorder caused by a somatic mutation in the GNAS gene on chromosome 20, leading to impaired osteoblastic differentiation and the replacement of normal bone with structurally weak, fibro-osseous tissue. It is typically diagnosed during childhood, though some cases remain undetected until incidentally discovered in adulthood. We present a patient with symptomatic knee osteoarthritis and an incidental finding of monostotic femur FD. The presence of FD in this anatomical location complicates an otherwise straightforward total knee arthroplasty (T

Standardized documentation and synoptic reporting of complex intestinal anatomy in enteric fistulation and intestinal failure.

AIM: In intestinal failure, delineation of both structure and function are key to controlling symptoms and planning further intervention. We have developed a template for developing an 'anatomy at a glance' patient-specific map to aid decision making and counselling. METHOD: A core dataset was developed and used to create an editable template to demonstrate the gastrointestinal tract, its relationship to the genitourinary tract, and specific anterior abdominal wall features. This was then used to create an anatomical template, specific to each patient, and stored in the electronic patient rec

Comparing Simulation Approaches Used in Finite Element Modelling of a Medial Opening Wedge High Tibial Osteotomy.

PURPOSE: Medial opening wedge high tibial osteotomy is a surgical procedure intended to relieve the symptoms of osteoarthritis in the medial compartment of the knee by addressing malalignment within the structures of the knee. In previous studies, the osteotomy cut and wedge opening in a Medial opening wedge high tibial osteotomy finite element model has been represented using either one of two techniques, which we define herein as wedge opening and wedge removal approaches. The purpose of this study is to determine whether the wedge removal for a medial opening wedge high tibial osteotomy fin

Malone antegrade continence enema for faecal incontinence and constipation in adults.

BACKGROUND: In the Malone antegrade continence enema (ACE) technique the appendix or a 'neoappendix' created from the caecum is brought to the abdominal wall creating a small stoma. Thereby, antegrade enemas can be administered to empty the colorectum and prevent faecal incontinence or constipation in children with congenital malformations. The aim of this study was to describe the results of the ACE in adults suffering from faecal incontinence or constipation. METHODS: In 16 adult patients (ten women and six men) suffering from severe faecal incontinence (ten patients) or constipation (six p

Creating an Optimal 3D Printed Model for Temporal Bone Dissection Training.

OBJECTIVE: Making a 3-dimensional (3D) temporal bone model is simple using a plaster powder bed and an inkjet printer. However, it is difficult to reproduce air-containing spaces and precise middle ear structures. The objective of this study was to overcome these problems and create a temporal bone model that would be useful both as a training tool and for preoperative simulation. METHODS: Drainage holes were made to remove excess materials from air-containing spaces, ossicle ligaments were manually changed to bony structures, and small and/or soft tissue structures were colored differently w

Navigation-guided gap arthroplasty in the treatment of temporomandibular joint ankylosis.

Gap arthroplasty, used in the treatment of temporomandibular joint (TMJ) ankylosis, is challenging, requiring resecting of massive abnormal bone formation at the skull base with complex and distorted anatomy. This study evaluated the application of image-guided navigation to gap arthroplasty. Four gap arthroplasties were performed on patients with unilateral TMJ ankylosis under computer-assisted navigation guidance. After preoperative planning and 3-dimensional simulation, the normal anatomic structures of the TMJ were created by superimposing and comparing the unaffected and affected sides. T

Surgical Decision Making in Incisional Blepharoplasty in Asians.

BACKGROUND: Owing to the prevalence of congenital single eyelids among Asians, methods are needed to create a double-eyelid fold. Incisional blepharoplasty is considered an ideal method for this purpose. The classical approach tends to create a stable but less dynamic fold. Subsequent refinements aim to achieve a balance between stability and dynamism. However, there are many procedural variations due to anatomical differences, which may account for the varying surgical outcomes. This systematic review explored the current techniques used in Asian blepharoplasty and focuses on the efforts to b

External fixators as an adjunct to wound healing.

Complex foot and ankle wounds present multiple challenges for the reconstructive surgeon. Soft tissue deficits must be closed to protect underlying structures from infection and to provide a stable environment for healing. Surgical options commonly used include healing by secondary intention, local flap closure, skin grafts, pedicled flaps, and free tissue transfer. Despite a surgeon's best operative efforts, these strategies may fail because of postoperative shear forces created by premature joint motion or pressure (either weight bearing or decubitus). In the properly selected patient popula

Facial Gender-Affirming Surgery: Facial Analysis and Procedures for Facial Masculinization.

Feminine facial features may be a source of significant gender dysphoria in certain patients. A masculine appearance includes an M-shaped hairline, wider and taller forehead, supraorbital bossing, larger and wider nose with a large dorsal hump, broader chin, wider mandible with a sharper angle, thinner lips, presence of facial hair, and prominent thyroid cartilage. Given these critical differences between feminine and masculine facial features, a comprehensive and systematic way of evaluating these distinct structures is necessary before facial gender-affirmation surgery. While no single appro
