A rare knee extensor mechanism injury: Vastus intermedius tendon rupture.

INTRODUCTION: Quadriceps tendon injuries are rare. There is a limited number of studies in the literature, reporting partial quadriceps tendon ruptures. We did not find any study reporting an isolated vastus intermedius tendon injury in the literature. PRESENTATION OF CASE: A 22 years old professional rugby player with the complaints of pain in the right lower limb, decreased range of motion in right knee and a mass in the mid-anterior of the right thigh applied following an overloading on his hyperflexed knee during a rugby match. T2 sequence magnetic resonance images revealed discontinuity

Grip lock injury resulting in extensor tendon pseudorupture: case report.

Grip lock injuries are uncommon, potentially devastating occurrences in male gymnasts performing high bar routines, and typically cause severe wrist sprains or forearm fractures. We retrospectively reviewed medical records of a 24-year-old former collegiate gymnast surgically treated for complete loss of index and long finger extension (pseudorupture) after a grip lock injury. Intraoperative evaluation 3 weeks after injury revealed profound intratendinous attenuation of index and long finger extensors with adhesions in the fourth compartment. We performed tenolysis and imbrication of the affec

Results of primary extensor tendon repair of the hand with respect to the zone of injury.

BACKGROUND: Laceration of the hand extensor tendons is common in the upper extremities, causing soft tissue trauma. These tendons, because of their superficial location and lying adjacent to bones, have a greater tendency to be injured than flexor tendons. OBJECTIVES: The aim of this study was to determine the results of primary repairs of lacerated extensor tendons of the fingers, with respect to the zone of injury, and also whether the results are different according to the anatomical zone in which they occur. PATIENTS AND METHODS: During a period of two years and four months, 32 patients

MR imaging of subscapularis tendon injury in the setting of anterior shoulder dislocation.

OBJECTIVE: To evaluate the degree and location patterns of subscapularis tendon injury in patients with prior anterior shoulder dislocation (ASD). MATERIAL AND METHODS: Forty-five consecutive MR shoulder examinations in patients with a history of ASD and 20 consecutive MR examinations in patients without prior dislocation were reviewed. Two readers assessed for the presence and location of tendinosis and tearing in the subscapularis tendon, which was divided into three segments: superior, middle, and inferior. The readers also documented the presence of anterior labral tears, osseous Bankart

Immediate free tissue transfer for coverage of Achilles tendon injury or reconstruction.

BACKGROUND: The restoration of function and contour to the Achilles region is a complex problem. This is reflected in the variety of reconstructive options described in the literature. The aim however remains to normalise the range of movement at the ankle joint and restore the power of plantar flexion. Few techniques have demonstrated this. METHODS: Six patients underwent soft tissue reconstruction over the Achilles tendon with a free scapular flap. Two ruptured Achilles tendons were reconstructed with FHL transfers. RESULTS: All six flaps remained viable and achieved stable coverage over t

Shoulder tendon transfer options for adult patients with brachial plexus injury.

Enhancement of upper-extremity function, specifically shoulder function, after brachial plexus injury requires a good understanding of nerve repair and transfer, with their expected outcome, as well as shoulder anatomy and biomechanics enabling the treating surgeon to use available functioning muscles around the shoulder for transfer, to improve shoulder function. Surgical treatment should address painful shoulder subluxation in addition to improvement of function. The literature focuses on improving shoulder abduction, but improving shoulder external rotation should take priority because this

Gluteus medius tendon injury during reaming for gamma nail insertion.

The use of intramedullary hip screws such as the Gamma nail has become a popular option for treatment of proximal femur fractures. The manufacturer recommends that the Gamma nail be inserted at the junction of the anterior (1/3) and posterior (2/3) of the tip of the greater trochanter and on the tip in the same region that the gluteus medius inserts. The purpose of the current cadaveric study was to quantify the damage to the gluteus medius tendon secondary to reaming of the greater trochanter necessary for safe Gamma nail insertion. The gluteus medius was identified and its tendinous insertio

Unusual patellar tendon injury in an adolescent runner with generalised ligamentous laxity.

A case is reported of an acute traction injury of the patellar tendon in a boy of 14 with joint hypermobility. Such injuries are unusual but early diagnosis and surgical repair lead to a good long term outcome. In adolescents participating in sports, the awareness of the possibility of a rare knee extensor mechanism injury is essential for a successful outcome.

Injury of the tibialis posterior tendon: a cause of acquired valgus foot in childhood.

Accidental division of the tibialis posterior tendon near the medial malleolus is easily overlooked and can, if left untreated, cause a painful planovalgus deformity of the foot. Two such patients are described, who had small wounds near the medial malleolus and in whom the tendon's damage was not initially diagnosed. Both patients came later with a painful valgus flat foot. Diagnosis of the lesion may be difficult because the inversion and supination action of the tibialis posterior tendon can be mimicked by the long flexor tendons of the toes. A patient with an intact tibialis posterior tend

Delayed rupture of extensor pollicis longus tendon following closed injury.

Closed rupture of the extensor pollicis longus tendon is most often seen following lower radial fractures, but is uncommon in the absence of fracture. Redden has recently described rupture following rotational injury of the forearm. I wish to record three cases of closed delayed rupture following direct injury to the area of the tendon.

Tendon transfers to improve grasp in patients with cervical spinal cord injury.

Patients with cervical spinal cord injury can gain useful hand function from a good rehabilitation programme and non-operative hand care. Effective prehension can usually be achieved by proper positioning, exercises, and splinting but when grasp is poor, tendon transfers are very effective in furthering the goal of independence. These patients have been reviewed extensively and classified into groups according to remaining neurological function. Group I patients have weak elbow flexion and weak shoulder function or less. No tendon transfers were done. Group II patients have shoulder control, e

Capacity for sliding between tendon fascicles decreases with ageing in injury prone equine tendons: a possible mechanism for age-related tendinopathy?

Age-related tendinopathy is common in both humans and horses; the initiation and progression of which is similar between species. The majority of tendon injuries occur to high-strain energy storing tendons, such as the human Achilles tendon and equine superficial digital flexor (SDFT). By contrast, the low-strain positional human anterior tibialis tendon and equine common digital extensor (CDET) are rarely injured. It has previously been established that greater extension occurs at the fascicular interface in the SDFT than in the CDET; this may facilitate the large strains experienced during l

The effect of a local application of fibroblast growth factor-2 on tendon-to-bone remodeling in rats with acute injury and repair of the supraspinatus tendon.

METHODS: We investigated the effect of application of fibroblast growth factor (FGF)-2 on the tendon-to-bone remodeling of repaired supraspinatus tendon in rats subjected to bilateral detachment. FGF-2 (100 mg/kg) in a fibrin sealant or sealant alone was applied on the right and left shoulders, respectively. Twelve animals each at 2, 4, and 6 weeks after surgery were sacrificed for histological analysis (n = 5) and biomechanical Q1 testing (n = 7). RESULTS: Histologically, at 2 weeks, FGF-treated specimens had significantly higher tendon-to-bone insertion maturing scores then untreated specim

[Surgical therapy of frequent hand injuries. Part 2: Injuries of the tendons, neurovascular bundles and soft tissues].

Injuries to the extensor and flexor tendons are among the most common injuries to the hand (cf. Part 1 of this overview). Severance of individual flexor tendons as also isolated injuries to individual nerves may occasionally be overlooked. In the case of doubt, operative revision is to be recommended. With the aid of dynamic fixation as described by Kleinert, good functional results can be achieved even with injuries "in no-man's-land". In the event of infections following minor injuries, conservative treatment should be applied only in the early stage; thereafter, surgical revision is indicat

Neglected but Negotiated - A Case Report of Multiple Zone VII Extensor Tendon Injuries Reconstructed Using Fascia Lata Autograft.

INTRODUCTION: Extensor tendon injuries of the hand are frequently encountered owing to their superficial anatomical position and inadequate soft tissue protection [1]. When such injuries are neglected, particularly at the level of zone VII, they present substantial reconstructive challenges due to tendon retraction, fibrosis, adhesion formation, and increased tendon gaps. Multiple reconstructive strategies have been described in the literature, with tendon grafting being a dependable option in delayed presentations [2]. CASE REPORT: We describe a case of a 2-month-old neglected zone VII exten

Evaluation of the Short-term Functional Outcome of Repair of Acute Extensor Tendon Injuries of Foot and Ankle.

BACKGROUND: Extensor tendon injuries of the foot and ankle are common, and less literature is available regarding its outcome. Ours is an observational study that aims to evaluate the functional outcome of surgical repair of all extensor tendons of the foot and ankle. METHODS: The study was conducted over a period of 1½ years with extensor tendon injury that was repaired within 24 hours of injury. The patients were followed up for a minimum period of 6 months and the functional outcome was evaluated using the American Orthopaedic Foot & Ankle Society (AOFAS) ankle-hindfoot score. RESULTS: A
