Computational Fluid Dynamics Simulation of Hemodynamic Alterations in Sigmoid Sinus Diverticulum and Ipsilateral Upstream Sinus Stenosis After Stent Implantation in Patients with Pulsatile Tinnitus.

OBJECTIVE: To investigate the relationships between upstream venous sinus stenosis and pulsatile tinnitus (PT), and to assess the correlation with diverticulum growth and the effectiveness of stent implantation. METHODS: Patient-specific geometric models were constructed using computed tomography venography images from a patient with PT, with sigmoid sinus diverticulum, and with upstream transverse sinus stenosis, in whom stenting of the upstream sinus stenosis alone achieved complete remission of PT. Computational fluid dynamics simulation based on this patient-specific geometry was performe

Systematic radiographic evaluation of three potential implantation sites for a semi-implantable bone conduction device in 52 patients after previous mastoid surgery.

UNLABELLED: The aim of this study was the evaluation of three localizations for the implantation of a semi-implantable transcutaneous bone conduction device after previous mastoid surgery. This is a retrospective review of electronic datasets of cranial computed tomography studies. The study setting is one tertiary referral center and included 52 consecutive adult patients (60 temporal bones) with a history of mastoid surgery. The intervention was virtual placement of the device with a planning software within the remaining mastoid as well as dorsal of the sigmoid sinus and caudal of the trans

Stenting for acute cerebral venous sinus thrombosis in the superior sagittal sinus.

Endovascular treatment for superior sagittal sinus (SSS) thrombosis is not always successful because of difficult access and long thrombus lesions. We report the first two cases of patients with acute cerebral venous sinus thrombosis at the SSS that was not recanalized by anticoagulation, mechanical thrombectomy, or thrombolysis, but was successfully treated by stent placement. Case 1 was a 37-year-old woman with bilateral subdural hematomas. Digital subtraction angiography showed obstruction of the sinus from the SSS to the right transverse sinus. Recanalization was achieved by selective thro

Familial intracranial dural arteriovenous fistulas.

BACKGROUND AND IMPORTANCE: Intracranial dural arteriovenous fistulas (DAVFs) are acquired abnormal communications between dural arteries and veins. Risk factors for development include sinus thrombosis and hypercoagulability, such as occurs in heritable thrombophilias. While there have been reports of other types of vascular anomalies (such as cavernous and arteriovenous malformations) occurring in families, to our knowledge there have been no reports of familial intracranial DAVFs. We describe the first 2 cases of intracranial DAVFs occurring in first-degree relatives. CLINICAL PRESENTATION:

Association of transverse sinus stenosis and dominant venous sinus drainage pattern with chronic migraine: A matched case-control study.

Recent studies have indicated that transverse sinus stenosis (TSS) is an imaging feature in patients with chronic migraine (CM). However, the clinical significance of this imaging finding in CM remains unclear. Moreover, the role of the dominant venous sinus drainage pattern in CM remains to be elucidated. This matched case-control study aimed to investigate the associations of TSS and venous sinus dominance pattern with CM. This study included 94 patients with CM and 94 sex- and age-matched controls. All included patients underwent magnetic resonance venography. Dominant venous sinus drainage

[Multiple Tentorial Dural Arteriovenous Fistulas with Acquired Pial Arteriovenous Fistula Presented with Unilateral Eye Symptoms Successfully Treated by Transarterial Embolization and Direct Surgery:A Case Report].

A 58-year-old man presented with slowly progressive chemosis, exophthalmos, and blepharedema of the left eye. Digital subtraction angiography revealed multiple dural arteriovenous fistulas(dAVFs)involving the bilateral tentorium, superior sagittal sinus, and right frontal convexity. In addition, occlusion of the left cavernous sinus and left sigmoid sinus were observed, which could lead to venous hypertension and might be associated with the occurrence of multiple dAVFs. Along with the congestion of the left ophthalmic vein, increased intracranial pressure due to the left tentorial dAVF with c

Abscess of the clivus in an adolescent with complicated acute rhinosinusitis: a case report.

BACKGROUND: Complications of acute sinusitis affecting multiple sites are very uncommon, and generally develop for a delayed diagnosis of the primary infection, with possible severe and life-threatening evolution. Patients can have variable presentations according to the site and extent of the infection. Multiple forms generally include the coexistence of orbital manifestations and intracranial infections. We here present a case with unusual multiple sites locations (i.e.: intraorbital intraconic abscess, sigmoid sinus thrombosis, preclival abscess, multiple splanchnocranium osteomyelitic proc

Lateral Sinus Floor Elevation in the Severely Atrophied Maxilla: Concentrated Growth Factors Versus Bone Substitutes. A Controlled Clinical Trial.

The present study clinically and radiographically compares the outcome of implants inserted in maxillary sinuses augmented with concentrated growth factors (CGFs) or demineralized bovine bone matrix (DBBM) in a one-stage lateral approach. In 20 patients with a residual bone height of 1 to 4 mm, lateral sinus floor elevation was performed, using CGFs or DBBM as the sole grafting material, with simultaneous implant placement. Outcome variables were implant and prosthesis failures, complications, subjective satisfaction, and radiographic changes in marginal bone level (MBL) 12 months after surger

A school of red herring.

A 66-year-old man with chronic kidney disease presented with painless unilateral vision loss and bilateral optic disk swelling. Vision loss progressed in the affected eye and developed in the fellow eye. Evaluation for infectious, neoplastic, ischemic, and inflammatory disorders was unrevealing. High-dose corticosteroids appeared to stabilize his vision temporarily. Eventually he was diagnosed with papilledema in the setting of transverse venous sinus stenosis and dural arteriovenous fistula. His papilledema resolved, and his vision improved following stenting of the venous sinus stenosis and

Acquired Chiari malformation Type I associated with a supratentorial arteriovenous malformation. Case report and review of the literature.

Chiari malformation Type I (CM-I), a condition defined by caudal descent of the cerebellar tonsils through the foramen magnum, is generally considered a congenital lesion. Several authors, however, have described an acquired form that appears identical to the congenital lesion on neuroimages. The most commonly reported cause of an acquired CM-I is cerebrospinal fluid diversion through a lumboperitoneal shunt. In this paper, the authors report the case of a patient in whom an acquired CM-I developed in association with a supratentorial arteriovenous malformation (AVM) of the brain. Development

Carotid artery-sygmoid sinus fistula: a rare complication of gunshot wound on the base of the cranium.

Vascular lesions without clinical manifestation may occur in cranial-facial wounds produced by bullets that course the base of the cranium. This work describes a rare kind of vascular complication in cranial-facial gunshot wound. The authors present the case of a patient, the victim of a cranium-maxillary gunshot wound. Carotid angiography revealed a carotid-sygmoid sinus fistula that filled the sygmoid and transverse sinuses, concomitant to the arterial angiographic phase. A direct communication between the external carotid artery and the sygmoid sinus was disclosed. We are not aware of any o

Atypical neuroimaging in rabies encephalitis: A case report of concurrent cerebral venous sinus thrombosis in a nine-year-old patient.

Human rabies is a fatal viral encephalitis that presents a profound diagnostic challenge in the absence of a known history of animal exposure. We report the case of a 9-year-old boy from Morocco who presented with fever, seizures, psychomotor agitation, hypersalivation and hydrophobia, despite no reported history of an animal bite or scratch. Brain magnetic resonance imaging (MRI) served as a critical diagnostic tool, strongly suggesting rabies encephalitis by revealing classic bilateral symmetric grey matter hyperintensities in the thalami and temporal lobes. Concurrently, MRI revealed an aty

The growth and expansion of meningeal lymphatic networks are affected in craniosynostosis.

Skull malformations are associated with vascular anomalies that can impair fluid balance in the central nervous system. We previously reported that humans with craniosynostosis and mutations in TWIST1 have dural venous sinus malformations. It is still unknown whether meningeal lymphatic networks, which are patterned alongside the venous sinuses, are also affected. We now show that the growth and expansion of meningeal lymphatics are perturbed in Twist1 craniosynostosis models. Changes to the local meningeal environment, including hypoplastic dura and venous malformations, affect the ability of

Cerebral venous sinus thrombosis-A primer for emergency physician.

Cerebral venous sinus thrombosis (CVT) is notoriously known for its varied presentations and extremely high risk of mortality, if remains undetected and untreated. On the other hand, life can be saved with full functional recovery if CVT can be identified with high index of clinical suspicion with supportive imaging and treatment with appropriate anticoagulation. It is important for clinicians to be meticulous to screen for both the potential reversible and heritable causes of CVT so that appropriate measures can be taken to prevent such catastrophe. Here we report a case of CVT involving righ

Bilateral posterior fossa and temporo-occipital extra-dural hematomas: A simple novel technique.

Posterior fossa extra-dural hematoma is rare and early diagnosis and management is associated with good outcome. Twenty four year male presented with somnolence and soft tissue swelling of occipital region following road traffic accident. CT scan revealed bilateral posterior fossa with bilateral temporo-occipital Extradural Hematoma (EDH) with mass effect. He was operated through a trifoliate incision and craniotomies leaving a strip of bone over the transverse and occipital sinus. The blood loss with such technique was minimal and the control of sinus tear was easier as the dura on either sid

Facial pillars in dogs: an anatomical study.

This study describes the facial pillar system in dolichocephalic and brachycephalic breeds of dogs, and highlights its differences. Knowledge of facial pillar system anatomy is imperative to carry out successful fracture repair. The aim of our study was to describe the pillar system of the facial skeleton in dogs, and compare different shapes of dog skulls using native skulls, radiographs and computed tomography (CT). The heads of brachycephalic and dolichocephalic dogs were investigated following a maceration process using trans-illumination, radiography and CT. Ground sections were created f
