PURPOSE: To evaluate the prevalence of long-term urinary catheter (UC) indwelling and potentially inappropriate urinary catheterization among residents of long-term care facilities (LTCFs) in Taiwan.
METHOD: From January to March of 2007, residents with long-term urethral UC indwelling of LTCFs in northern Taipei were invited for study and were enrolled when the informed consent was obtained. OBJECTIVE: To evaluate whether dilated fundus examination is necessary on patients presenting to clinic with lid complaints and normal vision.
METHODS: Patients with lid complaints were recruited from general and emergency clinics. Patients with visual symptoms or previous ophthalmic history were excluded. Subjects were examined by a junior ophthalmologist with slit-lamp biomicroscopy and Goldmann tonometry. Dilated posterior segment examination was performed with a Volk lens and the peripheral retina was examined with a three-mirror contact lens.

OBJECTIVE: To evaluate prepuce development and retractibility in a group of boys. To point out the value of circumcision and prepucial forced dilation during childhood.
METHODS: Prepuce development and retractibility were evaluated in 400 boys ages between 0-16 year old.
RESULTS: In boys under 1 year prepuce retractibility (assessed only in children who did not undergo forced dilation previously) was type I (non retractile) in 71.5% whereas type V (completely retractile) was only 5.5%. We present a case of a probably unnecessary Caesarean section due to misinterpretation of the cardiotocography (CTG) trace during general anaesthesia. A 27-yr-old patient in her 30th week of an uneventful, normal first pregnancy presented with a deep venous thrombosis in the pelvic region. She was to undergo an emergency thrombectomy under general anaesthesia. Over a 26-year period, from 1970 to 1995, 2250 pre-operative vocal fold checks were carried out by the ENT team at Manchester Royal Infirmary in patients undergoing thyroid, parathyroid and cardiothoracic surgery. A register was kept of all pre-operative fold checks. This register was retrospectively analysed and 23 vocal fold palsies were documented. Disorders related to the falciform ligament are extremely rare. We present a case of a 56-year-old woman diagnosed with fatty-falciform ligament appendage torsion (F-FLAT). F-FLAT was detected on ultrasound and confirmed by computed tomography. The patient recovered well without further treatment beyond conservative measures. F- FLAT is usually self-limiting. Precise identification on ultrasound and computed tomography is crucial to avoid unnecessary surgical procedures.

Pityriasis rosea is a common, self-limited disease which in its typical form should not raise diagnostic doubts. Nevertheless, its atypical presentations can be a challenge for the clinician, and hence it is important to be aware of them. Actinomycosis is a relatively uncommon infectious process involving the chest. A case of thoracic actinomycosis mimicking an inflammatory breast carcinoma in an elderly woman is presented with a review of the literature. The authors suggest that considering this disease in the differential diagnosis of indolent pulmonary parenchymal and pleural lesions is essential if appropriate diagnostic tests are to be obtained and proper therapy initiated, thus avoiding unnecessary invasive procedures.

Up to one third of the general population suffers from symptoms caused by hemorrhoids. Conservative treatment comes first unless the patient presents with an acute hemorrhoidal prolapse or a thrombosis. BACKGROUND: We formulated a clinical pathway (CP) for elective laparoscopic cholecystectomy (LC), which included the following preoperative evaluation: history and physical (H&P), right upper quadrant ultrasound (US), and liver function tests (LFTs). We hypothesized that routine LFTs did not alter management beyond that dictated by H&P and US, and could be excluded from the CP.
