Distal femoral malunion: Biomechanical drivers, fixation strategies and the role of fixator assisted correction.

Distal femoral malunion can have significant consequences because deformity at this level alters knee loading via its longest lever arm. Even relatively small magnitudes of malunion may lead to varus or valgus shift of the mechanical axis, creating abnormal compartmental contact pressures and external moments. This may lead to activity related pain and accelerated joint degeneration. Although distal femoral fractures account for only 0.5% of all skeletal injuries, they include high energy injuries in young patients, low energy mechanisms in older patients and a subgroup of periprosthetic injur

Circular frame fixation of complex tibial plateau fractures in the elderly: Functional and radiological outcomes for Schatzker V and VI type fractures.

Tibial plateau fracture fixation in the elderly poses a unique challenge due to poor bone stock, comminution and co-morbidities. There is limited evidence available in older patients sustaining multiple column Schatzker type V and VI injuries. In our institutions these injuries have traditionally been managed with circular frame fixation, often augmented with cross knee frame extension, internal screw fixation +/- bone graft substitutes. This study has reviewed the over 60-year-old cohort to better understand this complex injury. METHODS: Electronic fracture databases were queried for 'tibial

Far Posterior Approach for Rib Fracture Fixation: Surgical Technique and Tips.

BACKGROUND: The present video article describes the far posterior or paraspinal approach to posterior rib fractures. This approach is utilized to optimize visualization intraoperatively in cases of far-posterior rib fractures. This technique is also muscle-sparing, and muscle-sparing posterolateral, axillary, and anterior approaches have been shown to return up to 95% of periscapular strength by 6 months postoperatively1. DESCRIPTION: Like most fractures, the skin incision depends on the fracture position. The vertical incision is made either just medial to a line equidistant between the palp

Percutaneous Screw Fixation of Proximal Fifth Metatarsal Fractures.

UNLABELLED: Metatarsal fractures are one of the most common injuries of the foot, accounting for approximately 5% to 6% of all fractures confronted in the outpatient setting1. Approximately 45% to 70% of these fractures involve the fifth metatarsal, which have been described using a 3 zonal approach in 1993 by Lawrence and Botte2. Zone 2 fractures are difficult to manage given their retrograde vascular supply, leading to higher rates of nonunion1,3. Jones fractures (zone 2) are primarily treated surgically, with the 2 main methods being intramedullary screw fixation and plate fixation3. Surgic

Craniocerebral trauma inflicted by television falls.

Accidents and inflicted trauma account for 33% and 5-8% of childhood deaths, respectively. Injuries secondary to falling televisions have been reported in the clinical literature. However, descriptions of such injuries at autopsy are limited. The severity and patterns of injury may mimic those considered ''typical'' of inflicted trauma. Thus, integration of data from clinical, scene investigation, and autopsy is necessary for determination of the cause and manner of death. We present autopsy findings from two cases which illustrate injuries sustained from falling televisions. Findings common t

The use of ultrasound for axillary artery catheterization through pectoral muscles: a new anterior approach.

A palpable axillary artery pulse is a prerequisite for introducing an arterial line. The close proximity of four nerves to the artery increases the chance of nerve injury, especially in anesthetized patients. The highly colonized entry site results in frequent infection. Approaching the axillary artery through the pectoral muscles by using real-time imaging should improve success, decrease infection, and prevent nerve and vessel injuries because these structures and the needle can be visualized directly. I describe three patients who had successful axillary lines placed through the pectoral mu

Anatomical implant region - a critical determinant for the osteogenic potency of small extracellular vesicles and rhBMP-2.

PURPOSE: Mechanical stabilization is crucial for bone healing, yet complex fractures, particularly osteoporotic or comminuted, remain challenging due to impaired implant osseointegration, resulting in implant loosening or non-unions. This study investigated whether co-application of small extracellular vesicles (sEVs) derived from human umbilical cord mesenchymal stromal cells (hUC-MSC-sEVs) with a low dose of recombinant human bone morphogenetic protein 2 (rhBMP-2) could enhance screw implant osseointegration. METHODS: A novel small animal model was established to evaluate the effect of anat

Pellet gun trauma: An unusual and unexpected type of bone lesion.

Non-powder guns in general, and especially pellet guns are acknowledged to cause important injuries, also on bone tissue. However, pellet gun-related injuries have been poorly analyzed in forensic literature and scientists may have no idea of what signs a pellet gun can leave on bone. Moreover, since such lesions are not commonly encountered in everyday forensic practice, they could be misdiagnosed. For this reason, the present study was performed to investigate and illustrate the qualitative effects of pellet guns on human bone. An experiment was performed by shooting single pellets on four a
