Under Pressure: Increased Intracranial Pressure in Infants and Children. Presented at the 2024 AOC/AACO/AAO Sunday Symposium.

Intracranial hypertension (ICH) in children refers to increased pressure within the skull. The skull normally contains the brain, blood vessels, meninges, and cerebrospinal fluid (CSF). Abnormalities in any of these intracranial structures may cause ICH. The differential diagnosis for pediatric ICH includes brain tumors; intracranial hemorrhages; intracranial infections; craniosynostosis; disorders of CSF production, clearance, or abnormal CSF contents; and pseudotumor cerebri syndrome (PTCS), which may be primary (also known as idiopathic intracranial hypertension [IIH]) or secondary. Challenges and Opportunities of Vision Screening and Refractive Error Management for Underserved Children in the United States.

Uncorrected refractive error is a key cause of childhood visual impairment in the United States. As pediatric vision issues are often asymptomatic, vision screenings are essential to identify children's eye problems. Despite the importance of vision for children's health, well-being, and academic achievement, challenges remain in ensuring that children have equitable access to vision screenings and follow-up eye care. Children of racial and ethnic minorities experience a greater burden of myopia, while longstanding disparities in eye care access have been observed across both race and socioeco

Magnetic Resonance Imaging of Extra Ocular Muscles and Oculomotor Nerves in Patients with Congenital Monocular Elevation Deficit.

BACKGROUND: There is sparse literature on radiological findings in cases of monocular elevation deficit (MED). We conducted this study to report magnetic resonance imaging (MRI) characteristics of extra-ocular muscles and oculomotor nerves in patients with congenital MED and to investigate its patho-mechanisms. METHODS: We included patients with congenital MED without prior strabismus surgery. The cross-sectional area (CSA) of the extra-ocular muscles in quasi-sagittal and quasi-coronal scans was measured manually on 2-mm-thick T2-weighted MRI images using Osirix MD software. Oculomotor nerve

Putting the Puzzle Together: Case Report of Parinaud Syndrome in a Pediatric Patient.

Parinaud syndrome, also known as dorsal midbrain syndrome, is a condition affecting the dorsal midbrain region of the brainstem that presents with a triad of ophthalmic clinical findings, including upgaze paresis, convergence retraction nystagmus, and light-near dissociation. This case report will discuss the clinical presentation of Parinaud syndrome in a four-year-old patient who was seen in an out-patient clinic for intermittent exotropia 5 months after a suboccipital craniotomy resection of a pineal mass and ventriculoperitoneal (VP) shunt placement for associated hydrocephalus. Introducing the Novel Muscle Hook for Challenging Strabismus Surgery.

PURPOSE: Strabismus surgery on tight extraocular muscles poses significant challenges due to limited muscle mobility and increased surgical risks. This study introduces the Suh muscle hook, a novel instrument designed to enhance surgical maneuverability and outcomes in cases of tight extraocular muscles during strabismus surgery. DESIGN: The Suh muscle hook features five unique design enhancements, including an ultrathin profile, a curved handle, a beveled tip, a knobbed tip, and a grooved track for needle passage. These design features aim to improve usability and efficacy in surgeries invol

Risks and Complications Discussed in Consent for Strabismus Surgery.

This study is a qualitative analysis of the consent process for strabismus surgery, using recordings of physicians doing a mock consent for bilateral medial rectus recession. There is considerable variation in the risks of surgery and odds of complications that are cited by pediatric ophthalmologists during the consent process. We propose a reference table with complication rates for use during the consent process.

Post Stroke Visual Impairment: Interdisciplinary Collaborative Program - Canadian Perspective.

Post-stroke visual impairment (PSVI) affects more than 60% of the people who have a stroke but visual function is not routinely assessed in most stroke units. Recent high-quality research in this field has produced evidence-based assessment and management practices for PSVI. Orthoptists have integrated into the stroke care team in the United Kingdom (UK) to provide this valuable service. This paper outlines the development and implementation of the first known orthoptic PSVI service in North America. A pilot program was developed between the Ophthalmology, Neurology, and Physical Medicine and

How does the Surgical Repair of Ptosis Impact Refractive Error?

PURPOSE: To identify a correlation between the degree of congenital ptosis and levels of refractive error, visual stimulus deprivation and anisometropic amblyopia, and any improvement with surgical intervention. METHODS: A prospective investigation of 30 patients aged 3 months to 8 years, with a diagnosis of unilateral or bilateral congenital ptosis. A full orthoptic assessment and cycloplegic refraction were performed and patients requiring surgical intervention for ptosis were at surgeon discretion. RESULTS: The mean age at presentation was 27.1 months. The prevalence of amblyopia ranged f

Pediatric Cataract: Disparately Blinding Globally and in the United States.

Childhood cataract is a complex condition requiring longitudinal care, including early diagnosis, timely referral to a specialist, early surgical intervention, and dedicated postoperative care. Adherence to refractive correction and amblyopia therapy are critical for visual rehabilitation, even months to years after the cataract is removed. We review the impact of the social determinants of health on each step in the visual rehabilitation pathway for children with congenital and infantile cataracts. Children from socioeconomically marginalized backgrounds are more likely to experience delays i

Glaucoma Following Cataract Surgery: Pearls for Success Manuscript from the 2022 AOC/AACO/AAO Sunday Symposium.

Glaucoma Following Cataract Surgery (GFCS) remains a menace, so parents must be counseled prior to cataract removal in children. Age less than 7 months at the time of surgery increases this risk, and IOL placement has no effect. To lower IOP in GFCS, start with drops and before you escalate to surgery, consider phospholine iodide. Then, proceed cautiously with angle surgery and shunts, mixing in cycloablative procedures where appropriate in your hands. With patient-centered models for access, follow-up & adherence to treatment, GFCS can be controlled. Partnering with our certified orthoptist c

Short Loop Modification of the Semiadjustable Suture Strabismus Procedure for Optional Adjustment.

PURPOSE: To present a modification of the semiadjustable suture technique allowing for optional adjustment. METHODS: Short suture loops buried under closed conjunctiva were used instead of the exposed long muscle and sliding noose sutures involved in the standard semiadjustable suture procedure; an additional temporary tracing suture facilitated the retrieval of the buried muscle suture loop during adjustment. RESULTS: Fifty-three consecutive patients (57 rectus extraocular muscles) underwent recession with the modified semiadjustable short loop technique. The mean age was 44.5 years (range:

The Prevalence of Astigmatism and Spectacle Wear in Polish Schoolchildren.

INTRODUCTION: Unilateral or asymmetric astigmatism is considered a principal refractive error leading to amblyopia and regular eye examinations should be carried out during childhood to prevent visual impairment. The aim of this study was to evaluate the prevalence of astigmatism and spectacle wear among Polish schoolchildren. METHODS: A cross sectional study was carried out in children aged 6 to 14 years old from 50 schools in Poland. The presence of astigmatism was assessed by non-cycloplegic autorefraction, and defined as a cylinder equal or greater than 0.75 D. Children were classified as

Indications for Systemic and Genetic Testing in Patients with Congenital Cataracts.

Congenital cataracts account for a significant proportion of blindness in children worldwide. They affect approximately 12-136 per 100,000 births worldwide. A genetic etiology is present in a large proportion of patients and can lead to isolated cataracts or those in the context of genetic multisystem disorders. We present two examples of genetically determined childhood cataracts and briefly review the work-up of such patients. Mutations in numerous genes have been identified that cause congenital cataracts, such as those encoding for crystallins, connexins and aquaporins, as well as some dev
