Does the method of heart transplantation affect left ventricular filling?

BACKGROUND: For over 40 years now orthotopic heart transplantation (OHT) has been the treatment of choice in patients with advanced heart failure. For many years patients undergoing OHT have been treated with the classical approach involving anastomosis of the donor atria with the recipient atria resulting in a heart in which the atria are enlarged. An alternative method for OHT is the bicaval anastomosis technique, which involves connecting both of the donor's venae cavae with the recipient's venae cavae. AIM: To assess left ventricular (LV) filling in patients undergoing OHT using the class

High frequency of diastolic dysfunction in a population-based cohort of elderly women--but poor association with the symptom dyspnea.

BACKGROUND: The European Society of Cardiology recently proposed a new algorithm "How to diagnose heart failure with normal ejection fraction". Central element of the diagnostic strategy is the demonstration of diastolic dysfunction, either by tissue Doppler-derived indices in first line, or in second line by a combination of elevated blood levels of natriuretic peptide with abnormal tissue Doppler findings. We thought to use this diagnostic flowchart in a population-based cohort of elderly women, in whom the prevalence of diastolic dysfunction and heart failure is believed to be high. The pur

Usefulness of echocardiography and doppler echocardiography in endomyocardial fibrosis.

OBJECTIVE: Our goal was to demonstrate the usefulness of echocardiography and cardiac Doppler echocardiography (echo-Doppler) in the diagnosis of endomyocardial fibrosis, an unusual restrictive cardiomyopathy in Argentina. METHODS: Between 1980 and 1998, we studied 10 women (aged 27 to 58 years) with endomyocardial fibrosis confirmed by surgery and/or endomyocardial biopsy. Of the 10 cases of endomyocardial fibrothrombosis, 8 were biventricular and 2 were left ventricular. Six patients had only an echocardiographic study, and the last 4 patients (after 1987) had an echo-Doppler study also; 3

Exercise training improves left ventricular diastolic filling in patients with dilated cardiomyopathy. Clinical and prognostic implications.

BACKGROUND: Patients with dilated cardiomyopathy (DCM) often have left ventricular (LV) diastolic dysfunction that can precede the development of systolic dysfunction. Recent reports showed that exercise training (ET) improves the exercise capacity of these patients. Although this improvement is primarily due to peripheral adaptations, the contribution of LV diastolic filling has not been well defined. The purpose of this study was to determine whether ET can induce changes in LV diastolic filling that can account for an increase in exercise capacity and whether these changes can influence pro

The New York City Marathon: A systematic review of performance, participation, pacing, and health-related outcomes.

The 'New York City Marathon' is one of the world's largest and most influential mass-participation marathons. Although numerous studies have examined performance trends, participation patterns, pacing behavior, environmental influences, and physiological aspects of runners in this event, no review has synthesized the evidence specific to this race. This study aimed to systematically summarize the scientific literature on the 'New York City Marathon'. A systematic search of Scopus, PubMed, Web of Science, Embase, and Cochrane was conducted using terms related to the 'New York City Marathon' to

Multiethnic growth standards for fetal body composition and organ volumes derived from 3D ultrasonography.

BACKGROUND: A major goal of contemporary obstetrical practice is to optimize fetal growth and development throughout pregnancy. To date, fetal growth during prenatal care is assessed by performing ultrasonographic measurement of 2-dimensional fetal biometry to calculate an estimated fetal weight. Our group previously established 2-dimensional fetal growth standards using sonographic data from a large cohort with multiple sonograms. A separate objective of that investigation involved the collection of fetal volumes from the same cohort. OBJECTIVE: The Fetal 3D Study was designed to establish s

Comparison of Biomechanical Outcome Measures From Characteristically Different Blast Simulators and the Influence of Exposure Location.

INTRODUCTION: Simulation of blast exposure in the laboratory has been inconsistent across laboratories. This is primarily because of adoption of the shock wave-generation techniques that are used in aerodynamic tests as opposed to application of blast exposures that are relevant to combat and training environments of a Warfighter. Because of the differences in blast signatures, characteristically different pathological consequences are observed among the preclinical studies. This is also further confounded by the varied exposure positioning of the animal subject (e.g., inside the blast simulat

GLP-1 receptor agonists in the treatment of type 2 diabetes - state-of-the-art.

BACKGROUND: GLP-1 receptor agonists (GLP-1 RAs) with exenatide b.i.d. first approved to treat type 2 diabetes in 2005 have been further developed to yield effective compounds/preparations that have overcome the original problem of rapid elimination (short half-life), initially necessitating short intervals between injections (twice daily for exenatide b.i.d.). SCOPE OF REVIEW: To summarize current knowledge about GLP-1 receptor agonist. MAJOR CONCLUSIONS: At present, GLP-1 RAs are injected twice daily (exenatide b.i.d.), once daily (lixisenatide and liraglutide), or once weekly (exenatide on

Treatment of chronic canalithiasis can be beneficial for patients with vertigo/dizziness and chronic musculoskeletal pain, including whiplash related pain.

Background and aim Chronic musculoskeletal pain, e.g. whiplash associated disorders (WAD), fibromyalgia and myalgia, causes significant burden on both the individual and on society as a whole. In a previous study, the authors concluded that there is a likely connection between chronic benign paroxysmal positional vertigo (BPPV)/canalithiasis and headache, neck pain, generalized pain, fatigue, cognitive dysfunctions as well as tinnitus. The balance dysfunction in BPPV/canalithiasis is dynamic and not static. This leads to a perpetual postural mismatch. The vicious cycle of a disturbed equilibri

Pain and other symptoms in patients with chronic benign paroxysmal positional vertigo (BPPV).

UNLABELLED: Background and aim A diagnosis of chronic benign paroxysmal positional vertigo (BPPV) is based on brief attacks of rotatory vertigo and concomitant nystagmus elicited by rapid changes in head position relative to gravity. However, the clinical course of BPPV may vary considerably from a self-limiting to a persisting and/or recurrent disabling problem. The authors' experience is that the most common complaints of patients with chronic BPPV are nautical vertigo or dizziness with other symptoms including neck pain, headache, widespread musculoskeletal pain, fatigue, and visual disturb

[Application of spiral computerized tomography in the study of traumatic lesions of the thoracic aorta].

PURPOSE: Acute thoracic aortic injuries account for up to 10-20% of fatalities in high-speed deceleration road accidents and have an estimated immediate fatality rate of 80-90%. Untreated survivors to acute trauma (10-20%) have a dismal prognosis: 30% of them die within 6 hours, 40-50% die within 24 hours, and 90% within 4 months. We investigated the diagnostic accuracy of Helical Computed Tomography (Helical CT) in acute traumatic injuries of the thoracic aorta, and the role of this technique in the diagnostic management of trauma patients with a strong suspicion of aortic rupture. Dietary index for gut microbiota, plasma metabolome, and risks of metabolic dysfunction-associated steatotic liver disease and other chronic liver diseases.

BACKGROUND: The dietary index for gut microbiota (DI-GM) is a newly proposed metric for assessing diet quality linked to gut microbiota. However, prospective evidence is scarce on the associations between DI-GM and adverse liver outcomes. METHODS: The DI-GM was calculated by averaging the intakes of 12 foods and nutrients. Elastic net regression was performed to identify metabolites associated with DI-GM and metabolic signature reflecting higher adherence to DI-GM was constructed. Cox proportional hazards regression and mediation analyses were employed to explore the potential associations an

Traffic-Related Air Pollution and Birth Weight: The Roles of Noise, Placental Function, Green Space, Physical Activity, and Socioeconomic Status (FRONTIER).

INTRODUCTION: FRONTIER aimed to provide a robust and comprehensive evaluation of the impact of maternal exposure to traffic-related air pollution (TRAP) on fetal growth. Toward this aim it (1) disentangled the effects of noise as a co-exposure; (2) identified the relevant window(s) of vulnerability for this impact; (3) evaluated its modification by household and neighborhood level socioeconomic status (SES), stress, physical activity, and the timing of conception and delivery in relation to the COVID-19 pandemic lockdown; (4) elucidated the role of placental function as an underlying mechanism

User Character Strengths and Engagement Prediction on a Digital Mental Health Platform for Young People: Longitudinal Observational Study.

BACKGROUND: Mental ill health is a leading cause of disability worldwide, but access to evidence-based support remains limited. Digital mental health interventions offer a timely and low-cost solution. However, improvements in clinical outcomes are reliant on user engagement, which can be low for digital interventions. User characteristics, including demographics and personality traits, could be used to personalize platforms to promote longer-term engagement and improved outcomes. OBJECTIVE: This study aims to investigate how character strengths, a set of positive personality traits, influenc

NUMERICAL study of MAGNETO convective Buongiorno nanofluid flow in a rectangular enclosure under oblique magnetic field with heat generation/absorption and complex wall conditions.

A mathematical model is presented to analyze the magnetohydrodynamic (MHD) convective flow in a rectangular enclosure. Earlier studies on the Tiwai-Das volume fraction nanofluid model did not consider the Buongiorno nanofluid model. This is the focus of the present analysis which examines the laminar mixed convection magnetohydrodynamic flow of a nanofluid in a differentially heated rectangular enclosure with complex boundary conditions under an inclined magnetic field. Buongiorno's two-component nanofluid model is employed, which incorporates the effects of Brownian motion and thermophoretic

Association between fetal abdominal growth trajectories, maternal metabolite signatures early in pregnancy, and childhood growth and adiposity: prospective observational multinational INTERBIO-21st fetal study.

BACKGROUND: Obesity predominantly affects populations in high-income countries and those countries facing epidemiological transition. The risk of childhood obesity is increased among infants who had overweight or obesity at birth, but in low-resource settings one in five infants are born small for gestational age. We aimed to study the relationships between: (1) maternal metabolite signatures; (2) fetal abdominal growth; and (3) postnatal growth, adiposity, and neurodevelopment. METHODS: In the prospective, multinational, observational INTERBIO-21st fetal study, conducted in maternity units i

Sex allocation in a simultaneously hermaphroditic marine shrimp.

Two fundamental questions dealing with simultaneous hermaphrodites are how resources are optimally allocated to the male and female function and what conditions determine shifts in optimal sex allocation with age or size. In this study, I explored multiple factors that theoretically affect fitness gain curves (that depict the relationship between sex-specific investment and fitness gains) to predict and test the overall and size-dependent sex allocation in a simultaneously hermaphroditic brooding shrimp with an early male phase. In Lysmata wurdemanni, sperm competition is absent as hermaphrodi
