Bilateral Internuclear Ophthalmoplegia Caused by Dengue Fever.

A 22-year-old male was referred from the medicine department with the chief complaint of double vision for 2 months. The patient had suffered from dengue viral hemorrhagic encephalitis for 2 months for which intensive medical care was given. On ocular examination, both eyes (OU) showed limited adduction with contralateral abducting nystagmus on attempted horizontal gaze. Magnetic resonance imaging of brain showed pontine and midbrain hemorrhages which involved the region of medial longitudinal fasciculus and caused bilateral internuclear ophthalmoplegia (INO). Bilateral INO is very rare and mo

Recurrent Painful Ophthalmoplegic Neuropathy with Unilateral Oculomotor and Trochlear Nerve Palsy in an 8-year-old Girl.

BACKGROUND: Recurrent painful ophthalmoplegic neuropathy (RPON) is a rare disorder with a unilateral headache accompanied by ipsilateral episodes of painful ocular cranial nerve neuropathy, which typically occurs in childhood. CASE REPORT: We report an 8-year-old female with four episodes of RPON involving unilateral third and fourth cranial nerves. Right eye exotropia and complete ptosis were detected on examination. Brain MRI images revealed right third nerve enhancement where it exits from the brainstem. She completely recovered after 5 weeks with the administration of prednisolone and ind

Comparison of the Accuracy of Anterior Segment Optical Coherence Tomography and Ultrasound Biomicroscopy in Localizing Rectus Muscle Insertions.

PURPOSE: To compare the accuracy of anterior segment optical coherence tomography (AS-OCT) and ultrasound biomicroscopy (UBM) in localizing rectus muscle insertions. METHODS: The study was performed on 27 patients (39 rectus muscles) who required primary or secondary surgery. Using caliper function in the AS-OCT and UBM software, the distance from the insertion site to the anterior chamber angle was measured. The actual muscle insertion distance from limbus was considered as the measured distance plus 1 mm. The measurements by UBM and AS-OCT were compared with intraoperative measurements and

Reading Difficulty Due to Oculomotor Dysfunction following Mild Traumatic Brain Injury: A Case Report.

Reading involves adequate coordination of the oculomotor system. As interlink consists of neuronal control, an insult to the brain might affect the signal processing and lead to oculomotor dysfunction that can affect reading performance. Appropriate training to enhance the oculomotor coordination is effective in such scenarios. The purpose of this case report is to highlight the role of neuro-optometric vision therapy as a management option in oculomotor-based reading difficulty.

Skew Deviation.

Ocular skew deviation is an acute vertical misalignment resulting from the disruption of vestibular input to the interstitial nucleus of Cajal, involving the utriculo-ocular pathway, the cerebellum or the thalamus. A large percentage of skew cases associated with globe torsion and head tilt have complex or sinister etiologies, necessitating its differentiation from more common causes of acute onset vertical deviation in a sensitive and cost-efficient manner. This paper briefly reviews the anatomy and vestibular function of the inner ear, outlines the utriculo-ocular pathway, and discusses the t

Evaluation and Management of Symptomatic Vertical Strabismus and Diplopia.

The evaluation and management of vertical strabismus is more challenging and nuanced than that of horizontal strabismus. Vertical strabismus often results from a variety of restrictive or paretic causes, which can be further characterized as either acquired or congenital. In some cases, identifying the correct etiology of the strabismus can mean uncovering a potentially life-threatening condition, such as a brain tumor or stroke. The keys to identifying the correct diagnosis are, first and foremost, a careful history, and secondly, a detailed examination. The characteristics, etiologies, and e

Thyroid-Associated Orbitopathy: Management and Treatment.

Thyroid-associated orbitopathy (TAO) is a leading cause of orbital and strabismus symptoms in adults. Over the last decade, new treatments have greatly changed available options to alleviate symptoms and improve outcomes. This article discusses the pathophysiology and natural disease course of TAO, including when to pursue urgent treatment and when to consider other diagnoses. This article highlights the interventions that may alter the disease course and offers a comprehensive review on evidence-based interventions for both supportive therapy and systemic agents. The surgical strategies and p

A 2020 Update on 20/20 X 2: Diplopia after Ocular Surgery: Diplopia after Orbital Surgery.

In a review of 261 strabismus surgeries performed after previous orbital surgery, it was found that diplopia associated with orbital surgery most commonly occurs after orbital decompression for thyroid eye disease or after repair of orbital fractures. Other types of orbital surgery account for only a small number of cases. While the orbital surgery itself may contribute to the development or worsening of diplopia, in most cases, the post-operative strabismus is largely attributable to the underlying disease or trauma. However, in a small number of cases, a specific surgical misadventure can re
