Preconception parental personality disorder and psychosocial outcomes during the perinatal period: a prospective population-based study.

PURPOSE: Birthing individuals with personality disorder in young adulthood before pregnancy may be at increased risk of potentially modifiable adverse outcomes in the perinatal period that impact parental and child health. We aimed to investigate the perinatal psychosocial outcomes of preconception (prior to pregnancy) personality disorder. METHODS: Prospective analysis of 398 birthing individuals with 609 infants from Victorian Intergenerational Health Cohort Study (VIHCS). Preconception personality disorder was measured using the Standardised Assessment of Personality (SAP) at age 24. A ran

International Symposium on Ruminant Physiology: The immunometabolism of transition dairy cows from dry-off to early lactation-Lights and shadows.

The mismatch between the nutrient intake from the diet and the output by the mammary gland causes a negative energy balance in transition dairy cows that, if excessive, can promote several metabolic disorders. Other relevant phenomena occur during transition, such as inflammation at calving and changes in immunocompetence, redox balance, and mineral metabolism. Despite previous efforts, some aspects of the adaptive mechanisms observed in the transition period still need to be clarified. For instance, alterations of physiological responses even before the dry-off or during the dry period can af

Sex Differences in Recovery and Device Replacement After Left Ventricular Assist Device Implantation as Destination Therapy.

Background The relevance of sex and preimplant factors for clinical outcomes among patients with left ventricular assist devices intended for destination therapy is unclear. Methods and Results INTERMACS (Interagency Registry for Mechanically Assisted Circulatory Support) data (2006-2017) from 6771 men and 1690 women with left ventricular assist devices as destination therapy were analyzed to evaluate the contribution of preimplant clinical, demographic, and clinically judged psychosocial characteristics to time until death, heart transplant, device explant due to recovery, or complication-rel

[Immunopsychiatry and SARS-CoV-2 pandemic: Links and possible consequences].

OBJECTIVE: The SARS-CoV-2 (or COVID-19) pandemic has been propagating since December 2019, inducing a drastic increase in the prevalence of anxious and depressive disorders in the general population. Psychological trauma can partly explain these disorders. However, since psychiatric disorders also have an immuno-inflammatory component, the direct effects of the virus on the host's immune system, with a marked inflammatory response, but also the secondary inflammation to these psychosocial stressors, may cause the apparition or the worsening of psychiatric disorders. We describe here the probab

Feeding-unrelated factors influencing the plasma leptin level in ruminants.

The triglyceride content of lipid depots associated with the current feeding level is the primary determinant of leptin gene expression and the circulating leptin level. In laboratory rodents and primates the plasma leptin is influenced also by the age, gender and physiological status (puberty, pregnancy, lactation, postpartum period), and by the health condition such as sepsis due to Gram-negative (GN) bacteria. Some pathologic conditions with intensive cytokine release evoke an increase in plasma leptin, which is thought to depress the subsequent feed intake. However, the effect of these sec

Suppression of B lymphopoiesis during normal pregnancy.

We describe a dramatic reduction in numbers and activity of committed B lymphocyte precursors in the bone marrow of normal pregnant mice. Changes in cells responsive to IL-7 were evident as early as 6.5 d of pregnancy and values were < 10% of normal at parturition. B lineage precursors, identified by display of CD45R and absence of surface IgM, were also substantially depressed, and subpopulations representing different stages in the B lineage were assessed by three-color flow cytometry. Early pro-B cells are medium to large in size and have been previously characterized by low expression of t

Multiple micronutrient supplementation for maternal anemia prevention (MMS-MAP): an individually randomized trial of higher-dose iron (60 mg, 45 mg) compared to low-dose iron (30 mg) in multiple micronutrient supplements in pregnancy.

BACKGROUND: Antenatal multiple micronutrient supplementation (MMS) has been shown to be more effective than iron-folic acid (IFA) alone in reducing adverse pregnancy and birth outcomes. However, there is a concern that MMS containing 30 mg of iron may be less effective in reducing maternal anemia compared to IFA supplements containing 60 mg of iron. This poses a clinical and programmatic dilemma for countries with a high burden of maternal anemia (> 40% prevalence) where the World Health Organization (WHO) recommends using IFA with 60 mg of iron. METHODS/DESIGN: We will conduct an individuall

Posttraumatic stress, anxiety and depression following miscarriage and ectopic pregnancy: a multicenter, prospective, cohort study.

BACKGROUND: Early pregnancy losses are common, but their psychologic sequelae are often overlooked. Previous studies have established links between miscarriage and early symptoms of anxiety and depression. However, the incidence of posttraumatic stress symptoms and the psychologic response specifically to ectopic pregnancies have not been investigated. OBJECTIVE: The purpose of this study was to investigate levels of posttraumatic stress, depression, and anxiety in women in the 9 months after early pregnancy loss, with a focus on miscarriage and ectopic pregnancy. Morbidity at 1 month was com

The current use of acupuncture during pregnancy and childbirth.

PURPOSE OF REVIEW: Acupuncture has a growing clientele during pregnancy, delivery and the puerperium for an ever increasing list of indications. Objective evidence for its benefit is necessary to establish its roles in current practice. RECENT FINDINGS: For many of the current uses, randomized studies when conducted using at least one control group have not established any clear advantages from treatment. Those areas which rely upon subjective assessment of symptoms are particularly difficult to investigate without rigorous blinding strategies, separating those who provide the acupuncture fro

Patrilocality in South Asia as a determinant of maternal mental health: A scoping review.

Patrilocality (post-marital residence in the husband's natal household) is common across South Asia and shapes women's autonomy, caregiving roles, and exposure to conflict, violence, and economic control, yet it is rarely defined or measured consistently in perinatal mental health research. We conducted a scoping review (Arksey and O'Malley; Preferred Reporting Items for Systematic Reviews and Meta-Analyses extension for Scoping Reviews [PRISMA-ScR]) of studies from South Asia and South-Asian diaspora populations published since 2000, searching eight databases and Google Scholar (last search N

Maternal health and autism risk: parsing direct and indirect genetic effects using 3-generation family linkage.

Despite autism's prominent genetic etiology and early-life origins, parsing genetic effects contributing to the condition into those that operate directly (via allelic transmission to offspring) vs. indirectly (via influencing prenatal environment) remains challenging. We examined this using a novel design leveraging 3-generation family linkage in Danish national registers. The cohort included all children born in Denmark from 1998-2015 and their relatives identified through 3-generation family linkage. The analytic sample comprised full maternal cousin pairs, including parallel (children of m

Bereavement and maternal mental disorders during the perinatal period: a nationwide register-based study in Sweden.

Background: Perinatal mental disorders are common, yet few robust risk factors are known. Stressful life events such as loss of a close family member due to death could serve as potential clinical predictors for which mothers develop perinatal mental disorders. This study investigated the association between bereavement and the risk of first-onset perinatal mental disorders.Method: This nationwide register-based study included 1,986,490 pregnancies in Sweden (1,097,126 mothers). The exposure was death of a family member (sibling, parent, partner or child) occurring any time before pregnancy an

Mental Health during the Interpregnancy Period and the Association with Pre-Pregnancy Body Mass Index and Body Composition: Data from the INTER-ACT Randomized Controlled Trial.

Mental health problems and obesity are two common complications during pregnancy and postpartum. The preconception period is considered an appropriate period for prevention. Therefore, insights into interpregnancy mental health and the impact on weight and body composition are of interest to developing effective weight management strategies. The primary aim of this study is to assess the difference in women's mental health during the interpregnancy period and the association with pre-pregnancy body mass index (BMI) and body composition. The secondary aim is to study whether this association is

'I've Changed My Mind', Mindfulness-Based Childbirth and Parenting (MBCP) for pregnant women with a high level of fear of childbirth and their partners: study protocol of the quasi-experimental controlled trial.

BACKGROUND: Approximately 25 % of pregnant women suffer from a high level of Fear of Childbirth (FoC), as assessed by the Wijma Delivery Expectancy Questionnaire (W-DEQ-A, score ≥66). FoC negatively affects pregnant women's mental health and adaptation to the perinatal period. Mindfulness-Based Childbirth and Parenting (MBCP) seems to be potentially effective in decreasing pregnancy-related anxiety and stress. We propose a theoretical model of Avoidance and Participation in Pregnancy, Birth and the Postpartum Period in order to explore FoC and to evaluate the underlying mechanisms of change of

Lack of action of prolactin suppression on the regulation of the human menstrual cycle.

Bromocriptin (CB 154) has been found to suppress established lactation at a time when human plasma prolactin (HPRL) concentrations have already returned to the nonpregnant range. This action is due to inhibition of prolactin from the pituitary. It was then thought that a similar degree of inhibition induced during the menstrual cycle may help to uncover other possible biological actions of prolactin. In an attempt to elucidate this question eight breast-feeding mothers and seven normally menstruating volunteers underwent treatment with CB 154, including blood sampling during a sleep period. Th

Brexanolone injection in post-partum depression: two multicentre, double-blind, randomised, placebo-controlled, phase 3 trials.

BACKGROUND: Post-partum depression is associated with substantial morbidity, and improved pharmacological treatment options are urgently needed. We assessed brexanolone injection (formerly SAGE-547 injection), a positive allosteric modulator of γ-aminobutyric-acid type A (GABAA) receptors, for the treatment of moderate to severe post-partum depression. METHODS: We did two double-blind, randomised, placebo-controlled, phase 3 trials, at 30 clinical research centres and specialised psychiatric units in the USA. Eligible women were aged 18-45 years, 6 months post partum or less at screening, wit

Maternal depression and perceived social support as predictors of cognitive function trajectories during the first 3 years of life for preterm infants in Wisconsin.

BACKGROUND: Among families of infants born preterm, the association between post-natal depression and children's cognitive function is not well understood, but thought to be compromised. The purpose of this study is to investigate maternal depressive symptoms and perceived social support as predictors of children's cognitive function trajectories. METHODS: This is a longitudinal study of a sample of infants born preterm (less than 37 weeks) in Wisconsin. This study includes 130 infants who were hospitalized in one of three Wisconsin neonatal intensive care units in 2002-2005 and followed unti
