Treatment of a Diaphyseal Tibial Malunion with Use of the Clamshell Osteotomy.

UNLABELLED: Malunion, nonunion, congenital abnormalities, and osteometabolic diseases are the main causes of long bone deformities1. Although the exact incidence is unclear, it is estimated that about 10% of all fractures have some complication in terms of fracture-healing. In addition to the aesthetic impact, malunions generally substantially impair function and quality of life1. Every malunion is unique, and treatment is usually planned according to the degree of deformity and the postoperative expectations of the patient2. However, it is noteworthy that deformity correction usually requires

Saving the Limb-Tissue Collectors, Growth Factors, and Antibiotics in a War Injury.

ABSTRACT: As a life-saving procedure, limb amputation is frequently indicated in severe war injuries, especially in suboptimal medical supply. We report of a 56-year-old Ukrainian male who was wounded on the battlefield by an air attack during Russian early invasion in Zaporizhzhia. The blast injury caused a critical soft-tissue defect of the lower leg combined with comminuted fractures of the tibia and fibula. Although indicated, the patient refused the amputation of the lower leg. Initially closed reduction and stabilization by an external fixator, local debridement as well as soft-tissue co

Autopsy-Based Analysis of Landmine Blast Fatalities Associated With Left-Wing Extremism.

Background The utilization of landmine detonations, frequently facilitated via improvised explosive devices (IEDs), continues to be a favored strategy employed by Maoist insurgents in regions of India afflicted by left-wing extremism (LWE). Personnel tasked with anti-insurgency operations face an elevated probability of incurring fatal blast-related injuries. Conducting a forensic analysis of such fatalities is crucial to establishing consistent patterns of injury, enhancing protective measures, and guiding medico-legal protocols. Methodology A retrospective analysis was conducted at the Depar

Is augmented femoral lateral plating with helically shaped medial plates biomechanically advantageous over straight medial plates?

Dual plating of comminuted distal femoral fractures allows for early patient mobilization. An additional helically shaped medial plate avoids the medial vital structures of the thigh. The aim of this study is to investigate the biomechanical competence of an augmented lateral locking compression plate distal femur (LCP-DF) using an additional straight versus a helically shaped medial LCP of the same length. Ten pairs of human cadaveric femora were instrumented with a lateral anatomical 15-hole LCP-DF. Following, they were pairwise instrumented with either an additional medial straight 14-hole

Effect of Radiocapitellar Joint Over/Under Stuffing on Elbow Joint Contact Pressure.

PURPOSE: Comminuted radial head fractures are commonly treated by surgical resection or replacement with a prosthesis. A potential problem with radial head replacement is overlengthening of the radial neck ("overstuffing" of the radial head), which has been shown to affect both ulnohumeral kinematics and radiocapitellar pressures. We hypothesized that an overstuffed radial head prosthesis increases capitellar pressure and reduces coronoid pressure. METHODS: Seven human cadaveric elbows were prepared on a custom-designed apparatus simulating stabilizing muscle loads, and passively flexed from

Combined volar and dorsal plating vs. volar plating of distal radius fractures. A single-center study of 105 cases.

Despite recent advances in the surgical management of distal radius fractures (DRFs), the optimal treatment remains controversial as different fixation methods often have similar clinical functional and radiographic outcomes. The objective of this study was to assess the differences in outcomes 1 year postoperatively between volar plating and combined plating for DRFs. In a retrospective cohort study, we evaluated 105 consecutive patients operated with either a volar locking plate or combined dorsal and volar plating. The primary outcome was wrist range of motion (ROM). Secondary outcome measu

Clinical results of revision total elbow arthroplasty: comparison of infected and non-infected total elbow arthroplasty.

BACKGROUND: Total elbow arthroplasty (TEA) is considered a successful treatment for several conditions, including rheumatoid arthritis and comminuted fractures. However, failure rates as high as 62% have been reported, with many patients requiring surgical revision. Causes of failure requiring revision can be classified as infected or non-infected. This study evaluated the clinical and radiologic outcomes of TEA revision surgery according to causes of failure. METHODS: Twenty patients undergoing revision TEAs in 2010-2015 were retrospectively evaluated. Mean follow-up was 52.7 months. Patient

Anatomy and morphometrics of the radial head: an anthropometric study on dried cadaveric radiae.

BACKGROUND: The radial head (RH) is in close relationship with the elbow ligaments and is crucial for the elbow biomechanics. Comminuted RH fractures and radiocapitellar arthritis are indications for RH excision and replacement with a prosthesis. An RH prosthesis should closely resemble the natural anatomy both in size and shape. This anthropometric study collects raw data on the shape and size of the RH and investigates correlations between RH measurements and other radius landmarks to help design more anatomically accurate RH prostheses. METHODS: The present study was conducted on 68 dried

Replantation of multiple fingertip amputations using super microsurgery: A case report and literature review.

BACKGROUND: The fingertip amputation is an amputation type of the finger beyond the proximal nail fold. There is no vein available for anastomoses on the dorsal side of the finger, and the palmar vein of the finger is small and tightly attached to the skin. Therefore, it is relatively difficult to implement surgical anastomoses, which poses challenges to the clinical treatment of fingertip amputations. CASE REPORT: A 29-year-old male was admitted to the hospital due to "the amputation of the fingertips of the right index, middle, and ring fingers caused by a heavy object compression 3 h ago".

Severe thoracolumbar osteoporotic burst fractures: treatment combining open kyphoplasty and short-segment fixation.

INTRODUCTION: The majority of osteoporotic, spinal cord compressive, vertebral fractures occurs at the thoracolumbar junction level. When responsible for neurological impairment, these rare lesions require a decompression procedure. We present the results of a new option to treat these lesions: an open balloon kyphoplasty associated with a short-segment posterior internal fixation. MATERIALS AND METHODS: Twelve patients, aged a mean 72.3 years, were included in this prospective series; all of them presented osteoporotic burst fractures located between T11 and L2 associated with neurological i

Pediatric laryngeal trauma: a case series at a tertiary children's hospital.

BACKGROUND: Pediatric blunt or sharp laryngotracheal injuries are infrequent because of the softer cartilages and the protection of the prominent mandible. These injuries usually occur secondary to striking furniture or via the "clothesline" injury. METHODS: We present five cases of pediatric laryngotracheal injury (thyroid cartilage, true vocal cords, cricoid cartilage, cricotracheal junction, and posterior tracheal wall). RESULTS: We examined the need for intubation, need for tracheostomy, length of intubation, length of hospital stay, interval until direct laryngoscopy, use of steroids, p

Orthodontic-surgical treatment after posttraumatic bilateral condylectomy of the mandible in an adult patient.

A posttraumatic open bite associated with a bird face is reported. Condylectomy was indicated in relation to the plurifragmentary fracture of the condyles with limitation of mandibular movement. Condylectomies were mandatory as a result of delayed maxillofacial surgical treatment, which was related to the poor general condition of the patient after trauma. Functional disorder recovery and aesthetic deformity correction were planned by a team approach between orthodontists and maxillofacial surgeons with the support of a logopedist for the postural-related muscle problems. The main practical an

Activation of blood coagulation in pigs following lower limb gunshot trauma.

A standardized, quantifiable gunshot trauma to one hind leg of fourteen anaesthetized and sedated pigs was used to investigate the extent to which an isolated gunshot trauma causes activation of blood coagulation. The traumatized pigs were mechanically ventilated in intensive care for 48 h before they were euthanized. Blood samples were drawn at baseline (t = 0), 24, 27 and 48 h after trauma to examine the late effects on haemostasis. The median energy absorption in the pigs was 27.57 (22.6-31.7) J/kg. This gunshot injury caused increased creatine kinase and body temperature and led to a combi

Functional outcomes following surgical treatment of bilateral mandibular condylar fractures.

Debate continues regarding unilateral or bilateral treatment for mandibular condylar fractures. This retrospective study evaluates the functional outcomes of bilateral condylar process fractures after surgical intervention. From May 1994 to December 2004, 51 adult patients with bilateral mandibular condylar process fractures were studied. There were 33 cases of bilateral condylar fractures (type I); 12 cases of condylar-subcondylar fractures (type II); and six cases of bilateral subcondylar fractures (type III). All patients underwent open reduction and internal fixation. Four patients had chi

Manners of killing and rituals in Apulian mafia murders.

The Apulian (South of Italy) territory saw the birth of a criminal organization called Sacra Corona Unita (SCU, United Holy Crown) which transformed the rules of traditional mafia organizations. This work examined 83 victims of the SCU between 1980 and 2000. The bodies were mainly of SCU members and in some cases, of police and law enforcement officers and other citizens caught in the crossfire. Some of these were discovered; thanks to the collaboration of "repented" SCU members who became police informers. The condition of the bodies varied in relation to the date and manner of killing. In so

Unusual presentation of blunt laryngeal injury with cricotracheal disruption by attempted hanging: a case report.

The management of a patient with traumatic disruption of the cricotracheal junction in an attempted suicide by hanging is described. Such injury is uncommon, and many patients die at the scene; detailed radiologic imaging is rare because of the urgency of airway management. The delayed complete disruption of the major airway in this patient allowed adequate imaging and corrective management. Associated soft-tissue injuries of the great vessels of the neck are also described.
