Longitudinal Analysis of Refractive Errors in Premature Children during the First Three Years of Life.

PURPOSE: To examine the longitudinal behavior of refractive errors in both retinopathy of prematurity (ROP) and non-ROP screened premature children during the first three years of life. METHODS: This retrospective cohort included premature children (less than 37 weeks gestational age) born between 10/2011 and 8/2013 with ≥ two cycloplegic refractions. Cycloplegic refractions were converted into power vectors: M (spherical equivalent), J0 [positive for with-the-rule (WTR) and negative for against-the-rule (ATR) astigmatism], and J 45 (oblique astigmatism). Each power vector component was fitte

Compartmental Strabismus.

Recent reports confirm innervational compartments of select rectus extraocular muscles as well as the superior oblique.1 Histopathological and orbital imaging studies demonstrate well defined compartmental innervation of the horizontal rectus muscles with less differentiation in the vertical rectus muscles. Acquired vertical misalignment not associated with cyclovertical muscle dysfunction has been associated with horizontal rectus muscle compartment dysfunction. Pattern and other forms of strabismus have been associated with segmental or compartmental abnormal innervation of the extraocular m

Introduction to Traumatic Brain Injury in Children.

Over the last decade, the reported rates of traumatic brain injury (TBI) have increased dramatically in all age groups, but particularly in children. However, these reports likely underestimate the scope of the problem for several reasons. There is no uniformly accepted definition of mild TBI (concussion). There are often no outward and obvious physical signs of TBI. Interpersonal and psychological issues, which may be significant, may be misdiagnosed or misinterpreted. And there is no uniformly accepted neuroimaging protocol. This presentation will outline the major issues surrounding the cau

Neuroanatomy and Imaging Assessment in Traumatic Brain Injury.

Concussion is a common injury in childhood with an estimated 1.4 million children sustaining concussions annually in the United States. While many concussions occur in the sports and recreation setting, it is important to recognize that non-sports and recreation activities are also common causes of concussion in childhood. Since neurologic projections associated with the afferent and efferent visual system are widely distributed throughout the brain and thought to comprise over 50% of brain function, it is not surprising that the diffuse sear injury associated with concussion often produces vi

Traumatic Brain Injury in Children: Do the Eyes Have It? The Orthoptic Evaluation of Traumatic Brain Injury.

Because the neuropathological changes caused by mild traumatic brain injury (mTBI) more often manifest as functional impairments than structural abnormalities, the clinical diagnosis of mTBI may rely too heavily on a combination of history and self-reported symptoms. The mechanism of injury in mild traumatic brain injury (mTBI) predicts that supranuclear pathways controlling eye movement systems would be vulnerable to damage, and diagnostic tests of these systems would be high-yield. In fact, tests of oculomotor function have proven to be highly sensitive in detecting neurological soft signs,

Traumatic Brain Injury in Children: The Psychological Effects of Mild Traumatic Brain Injury.

Concussion, or mild traumatic brain injury (mTBI), results from a force to the head and can cause acute physical, cognitive, and psychological symptoms. The majority of concussion symptoms will resolve within a month, but upward of a third of patients will have persistent, chronic symptoms. When these symptoms become chronic and persist beyond 1-3 months, this is termed post-concussion syndrome (PCS). Psychological changes associated with PCS may in part be due to a traumatic event and the injury itself and therefore post-traumatic stress reactions may contribute. In addition, alterations to d

Impact of Amblyopia on the Central Nervous System.

Amblyopia is a common perceptual disorder resulting from abnormal visual input during development. The clinical presentation and visual deficits associated with amblyopia are well characterized. Less is known however, about amblyopia's impact on the central nervous system (CNS). While early insights into the neuropathophysiology of amblyopia have been based on findings from animal models and postmortem human studies, recent advances in noninvasive magnetic resonance imaging (MRI) techniques have enabled the study of amblyopia's effects in vivo. We review recent retinal and neuroimaging researc

Longstanding and Refractory Anti-Muscle Specific Tyrosine Kinase Antibody-Associated Myasthenia Gravis (Anti-MuSK-MG) in a Child Successfully Treated with Rituximab.

Anti-muscle specific tyrosine kinase antibody-associated myasthenia gravis (MuSK-MG) is a rare subtype of MG characterized by more frequent relapses and a clinical course that is refractory to standard treatments. Rituximab, a monoclonal antibody targeting CD20+ B cells, has been used effectively in the adult population to achieve stable remission. We describe a pediatric patient with MuSK-MG who demonstrated an excellent response to rituximab after failing standard therapy.

Don't Miss This! Red Flags in the Pediatric Eye Exam: Blurred Disc Margins.

Evaluating a patient with blurred optic disc margins, particularly a child, and establishing a diagnosis can be a demanding task. We aim to review the differential diagnosis of blurred disc margins, identify the clinical characteristics of a swollen optic disc, discuss imaging modalities used in the evaluation of the optic nerve head as tools for formulating a diagnosis, and identify red flags that may indicate a serious disorder.

Early Buckle Migration and Restrictive Strabismus after Successful Medical Management of Scleral Buckle Infection.

We report a case of restrictive strabismus caused by early scleral buckle (SB) migration within 1 month of surgery after successful medical management of SB infection. A 24 year-old man underwent scleral buckling surgery for left eye inferior retinal detachment (RD). A solid silicone buckle element was placed inferiorly along with an encircling silicone band. Two days after surgery, he presented with SB infection. Methicillin resistant Staphylococcus aureus was cultured from the exudate at conjunctival suture sites. Since the retina was well attached and it was only the second postoperative da

Diagnosis and Management Practice Patterns among Pediatric Ophthalmology Personnel in Israel.

PURPOSE: To investigate the diagnosis and management practice patterns of different aspects of pediatric ophthalmology among pediatric ophthalmologists and orthoptists in Israel. METHODS: A 21-question survey was delivered to all registered pediatric ophthalmologists and orthoptists in Israel. RESULTS: The response rate was 58.3%. Most pediatric ophthalmology personnel in Israel do not document intermittent exotropia (IXT) with a control scale, do not use modalities other than patching for amblyopia, and do not use distance stereoacuity tests. There was no consensus regarding patching and ov

Severe Acquired Restrictive Strabismus Fixus in Patients with Trisomy 21.

Acquired strabismus fixus is a rare form of severe esotropia described most frequently in patients with high axial myopia and infrequently associated with other conditions. Refractive errors, cataracts, and ocular motility disorders are common in patients with trisomy 21. We report two unusual cases of patients with trisomy 21 who developed severe acquired restrictive strabismus fixus. Both patients were successfully treated with medial rectus recession followed by lateral rectus and superior rectus myopexy combined with additional medial rectus weakening, including free tenotomy in one case.

Don't Miss This! Red Flags in the Pediatric Eye Examination: Subnormal Acuity.

Subnormal vision can be a harbinger of threating disease. Recognizing certain "red flags" on history or examination can facilitate early diagnosis in the pediatric patient. These objective tests or subjective complaints may help differentiate true organic causes of vision loss from possible functional overlay, amblyopia or poor results due to an uncooperative child.

Don't Miss This! Red Flags in the Pediatric Eye Examination: Ophthalmoplegia in Childhood.

Paralytic strabismus in children is rare, occurring in about 0.1% of children. This rate is far less common than the 3% rate usually noted for comitant strabismus. The relative rates of ocular motor pareses were fourth nerve palsies in 36%, sixth in 33%, third in 22%, with multiple ocular motor nerve palsies in 9%. In a single population series from Minnesota, few cases were associated with neoplasm. However, institutional case series reports a high rate of neoplasm for acquired third nerve and sixth nerve palsies after excluding trauma and congenital causes. Tumor is rare in children with fou

Ophthalmoplegia: Definition and Clinical Diagnostic Techniques.

Ophthalmoplegia can be caused by multiple etiologies. There are restrictive, paretic, neurologic, and myasthenic conditions that can lead to reduced mobility of the eyes that at first glance may be difficult to discern from one another. While the examiner may be tempted to order various lab tests and scans to reach a diagnosis, the assessment of ophthalmoplegia by clinical examination alone can often provide enough information to determine the etiology, or at least rule out causes that can spare the patient and facility from unnecessary time-consuming and costly tests. This article will discus

Acquired Ophthalmoplegia in Older Children and Adults.

Ophthalmoplegia or ophthalmoparesis are the terms given to paralysis or paresis of one or more of the extraocular muscles in one or both eyes. It may be representative of serious neurological or systemic disease. Other mechanisms may cause limitation of eye movement, including restrictive or myasthenic conditions, or combinations of several etiologies. Evaluating and differentiating the mechanisms causing ophthalmoplegia is important in determining the correct diagnosis and selecting the appropriate management in these complex cases.

Comparing the Single Maddox Rod Test to the Alternate Cover Test in Normal Individuals.

PURPOSE: The single Maddox rod test (SMRT) is a rapid subjective method for detecting ocular misalignment. In this study, we compared the prism single Maddox rod test (PSMRT) to the prism alternate cover test (PACT) in normal individuals to see if there was any disagreement. METHODS: Ocular alignment was measured at distance and near with the PSMRT and PACT in 52 normal participants. Secondary measurements included visual acuity, near point of convergence, and stereoacuity. RESULTS: The difference between each participants' measurements at near (ΔX') was 4.44 ± 3.87 Δ (p < .001) meaning each

Orbital Apex Syndrome Secondary to Middle Cranial Fossa Meningocele: A Rare Pediatric Case.

BACKGROUND: Orbital apex syndrome (OAS) is a rare, sight-threatening condition caused by injury or compression of neurovascular structures in the orbital apex, which, depending on the underlying etiology, may even pose a life-threatening risk. Prompt recognition and intervention are essential to prevent permanent visual loss and systemic complications. CASE PRESENTATION: We report a 10-year-old child presenting with acute left-sided proptosis, eyelid edema, ophthalmoplegia, and optic disc edema, following a history of orbital cellulitis and meningitis in early childhood. Imaging revealed a mi
