A search was performed of the literature published in peer-reviewed journals from disciplines related to eye care and rehabilitation between 2000 and 2022. The purpose was to discover new thinking on the topic of the evaluation and management of convergence insufficiency in adults. Ninety-one papers were reviewed and summarized. The etiology, identification, evaluation, and management of convergence insufficiency in this population, as reported in the literature, is presented in this paper. Many publications addressing rehabilitation after traumatic head injury addressed the management of conv

Cohen Syndrome is a rare autosomal recessive condition characterized by facial abnormalities with or without microcephaly, non-progressive intellectual delay, hypotonia, ophthalmic abnormalities, and neutropenia. Due to its low incidence and variable presentation, much about the disorder, including ophthalmic manifestations, is not fully understood. Here, we present the first documented case of a 5-year-old Amish child with Cohen Syndrome who presented with bilateral subluxation of microspherophakic lenses - rare findings themselves, let alone coexisting in a patient with a rare genetic syndro

Horizontal gaze palsy and progressive scoliosis (HGPPS) is a rare autosomal recessive disorder caused by mutations in the ROBO3 gene. Clinical presentation consists of impairment of conjugate horizontal eye movements together with a progressive scoliosis beginning in childhood. We report dizygotic twins with HGPPS that had absence of conjugate horizontal eye movements combined with divergent strabismus and synergistic divergence. One of them also had a congenital palpebral ptosis and vertical strabismus of the right eye. Onset of scoliosis occurred in childhood with rapid progression in the se

Objective: To assess the value of measuring diplopia before and after pyridostigmine intake to differentiate myasthenia gravis from sagging eye syndrome. To establish a threshold for a positive response to pyridostigmine in the diagnosis of myasthenia gravis. Methods: 15 patients with myasthenia gravis and 15 with sagging eye syndrome diplopia were evaluated. Diplopia was measured in five positions (upgaze, downgaze, right gaze, left gaze, and primary position). After baseline measurements, the patient received a single dose (60 mg) of pyridostigmine. After 60 minutes the prism measure was perfo

New-onset diplopia is an important complication that may occur following several types of ocular surgery, including glaucoma surgery. Surgeons should be aware of the possibility of ocular motility disturbances after glaucoma surgery, whereas strabismus experts must understand the mechanisms and the most appropriate management of these forms of strabismus. The incidence, mechanisms, characteristics and treatment of diplopia and strabismus after glaucoma surgery will be reviewed.

There is nothing more disheartening for our patients to go through successful ocular surgery only to experience unexpected post-operative diplopia. Diplopia following any type of ocular or periocular surgery is a known risk. Despite advances in surgical techniques, the strabismus team continues to get referrals for this unwanted result. This symposium will address the incidence, mechanism and treatment of post-operative diplopia following various types of ocular surgery such as retinal, orbital, glaucoma, iatrogenic monovision and more. Both surgical and non-surgical treatments will be present

Purpose: To evaluate the effect of bupivacaine injection without electromyographic guide for correcting residual esotropia and exotropia after strabismus surgery. Methods: Thirty patients with residual esotropia or exotropia after strabismus surgery were included in the study. Three milliliters of bupivacaine 0.5% were injected into medial or lateral rectus muscle without electromyographic guide. Results: The mean pre-injection misalignments were 14.8 ± 3.4 (8-20) PD at distance and 14.7 ± 4.4 (6-25) PD at near. The 1 month post-injection alignment changes were 5.2 ± 2.6 (2-13) PD at distance

Purpose: To evaluate the outcome of augmented superior rectus transposition (SRT) with medial rectus (MR) recession in patients with Duane Retraction Syndrome (DRS) and sixth nerve palsy. Methods: Twenty four patients (16 DRS and 8 sixth nerve palsy) that underwent the procedure were included. The superior rectus muscle was secured, detached, and re-attached to the sclera along the spiral of Tilaux, adjacent to lateral rectus insertion. A non-absorbable augmentation suture was passed through the sclera, 8 mm posterior to the insertion of the lateral rectus. Results: At the last follow-up, the ef

Persistent ocular motility and sensorial abnormalities following retinal detachment surgery are not common. Scleral buckling increases the risk of deviation in secondary gaze positions most commonly horizontal deviations. Suspect superior oblique incarceration in patients with limited downgaze. For patients with persistent strabismus, surgery is an option. Removing the exoplant may be necessary to access the extraocular muscles but the effect on alignment is minimal except in cases were the scleral buckle migrates anteriorly causing restriction. Removing the scleral buckle may increase the ris

Binocular triplopia is a rare symptom and usually has a corneal or lenticular origin. Uniocular diplopia arising from ectopia lentis is quite common in Marfan syndrome. A visual phenomenon related to binocular triplopia due to strabismus was first reported in 1943 by Burian. Both monocular diplopia and binocular triplopia have been reported in concomitant strabismus. Strabismus is also common in Marfan syndrome but congenital superior oblique paresis has never been reported before. We report herein a rare case of binocular triplopia in a patient with Marfan syndrome arising from decompensation

PURPOSE: Selection of the ideal procedure to correct symptomatic excyclotropia depends on several factors including the degree of torsion and associated vertical, horizontal and pattern deviation. Selective tuck of the anterior temporal torsional fibers of the superior oblique (SO) tendon is an alternative procedure to the classical Harada-Ito. The purpose of this study is to report its stability and results.
METHODS: Retrospective review of all consecutive patients with symptomatic excyclotorsion of at least 5º (degrees) treated by selectively splitting and tucking the anterior temporal fibe

Concussion is a worldwide health concern among children and adolescents. Over the decades concussion has been gradually better recognized as an entity that accounts for a significant disability post head trauma in patients. Patients present with cognitive, somatic and oculo-vestibular symptoms that can be incapacitating. Most concussion symptoms are transient and resolve within 1-2 weeks but can persist for years. Concussion pathophysiology is complex and may not be fully understood but it involves numerous mechanisms including cellular metabolic derangements, cerebral blood inflow, and axonal

Much attention in the media has been given to concussion, a mild form of traumatic brain injury (TBI). Depending on the severity of the injury, the visual systems of children with TBI are at risk, and diplopia, subnormal acuity and visual processing difficulties often result. These children need careful evaluation and treatment by ophthalmologists and orthoptists. Many will benefit from a multidisciplinary team-based care. In this symposium, the assessment, diagnosis and long-term follow-up of children with TBI will be reviewed.

Traumatic Brain Injury (TBI) is a very broad diagnosis. Some TBI patients have minimal complaints for a short period of time. Other TBI patients suffer from drastic life-altering, permanent brain damage. One particularly troubling class of TBI patients arises from people who experience one or more concussions in their youth. Later, as adults, the TBI manifests as multiple learning difficulties or even personality changes. As ophthalmic professionals, we are often the first to see potential TBI patients as they complain of visual disturbances. It is of utmost importance to find ways to help the

BACKGROUND: Orbital myositis (OM) is a subgroup of idiopathic orbital inflammation. It can involve single or multiple extraocular muscles and result in restriction or paresis of extraocular muscles.
METHOD: We reported two unusual cases of extraocular muscle paresis in the fibrotic stage of chronic OM and reviewed the literature related to this finding.
RESULTS: The first case was of a 38-year-old woman with chronic OM with large-angle right eye exotropia and right medial rectus paresis. She received a botulinum A toxin injection into the right lateral rectus muscle as a first treatment, but

Although the pediatric eye examination can be made more challenging by inattention, poor cooperation, sleep, or just the wiggles, these should never be justification for an inadequate assessment of the child. In fact, a challenging patient should make the examiner ever more careful to get a good history and to look carefully for potential red flags that should be examined in greater detail. Missing a history of diplopia, or not noticing an abnormal pupil, or blurred disc margins, to name a few, may delay the diagnosis and treatment of potentially life-threatening entities. In this symposium, r

Monocular elevation deficiency is characterized by the inability to elevate one eye in abduction, adduction, and primary gaze. To date, various operations, including Knapp's procedure, have been used in the management of hypotropia associated with this condition. However, single muscle transposition has only recently been described as a feasible alternative, offering a number of advantages over other techniques. In particular, it reduces the risk of anterior segment ischemia and allows for an inferior rectus recession to occur simultaneously as is often required, thus avoiding the need for sta
