Amelioration of cerebellar dysfunction in rats following postnatal ethanol exposure using low-intensity pulsed ultrasound.

BACKGROUND: The neonatal development stage of the cerebellum in rats is equivalent to a human foetus in the third trimester of pregnancy. In this stage, cell proliferation, migration, differentiation, and synaptogenesis occur. Clinical and experimental findings have shown that ethanol exposure during brain development causes a variety of disruptions to the brain, including neurogenesis depression, delayed neuronal migration, changes in neurotransmitter synthesis, and neuronal depletion.During postnatal cerebellar development, neurons are more vulnerable to the destructive effects of ethanol. Clinical and laboratory aspects of thyroid autoantibodies.

This review describes the aetiology of the major thyroid antigens. Iodination of thyroglobulin produces multiple antigen configurations which are functionally active but immunologically distinct. The thyroid stimulating hormone (TSH) receptor is a two-subunit glycoprotein; the extracellular A subunit is recognized by thyroid stimulating antibodies, while those antibodies recognizing the B subunit, located much nearer the cell surface, appear to function as blocking antibodies. Thyroid peroxidase (TPO), originally described as thyroid microsomal antigen, is present on the apical surface of thyr

Septic shock in pregnancy.

OBJECTIVE: To evaluate the etiology, management, and maternal and perinatal outcome in patients with septic shock during pregnancy. METHODS: In 18 patients with septic shock during pregnancy, the criteria for the diagnosis were sepsis-induced hypotension unresponsive to adequate fluid resuscitation and requirement for vasopressors. RESULTS: Causes of shock were pyelonephritis (n = 6), chorioamnionitis (n = 3), postpartum endometritis (n = 2), toxic shock (n = 2), and one each of septic abortion, ruptured appendix, ruptured ovarian abscess, necrotizing fasciitis, and bacterial endocarditis. Consultation-liaison psychiatry in an obstetric service.

OBJECTIVE: The aim of this paper is to provide an overview of the work of a consultation-liaison (C-L) psychiatry service to an obstetric inpatient unit in a university affiliated teaching hospital, with the aid of a comprehensive clinical database. METHOD: The MICRO-CARES clinical database system was used for prospective recording of data on all obstetric inpatients referred to the C-L psychiatry service and the hospital clinical database was used to compare referred inpatients with all obstetric inpatients. RESULTS: Ninety patients were referred in 3 years, a referral rate of 1.2% of obste

Unveiling the hidden triad of antidepressants, benzodiazepines, and Z-hypnotics during pregnancy: A 15-year population-based study in Taiwan.

BACKGROUND AND OBJECTIVE: Understanding the trends and determinants of antidepressant use during pregnancy, especially when combined with sedative-hypnotics, is critical for balancing maternal mental health needs with potential fetal risks. However, the patterns of antidepressant use, both alone and with sedative-hypnotics, across different stages of pregnancy remain poorly investigated. This study aims to investigate trends in antidepressant use during pregnancy and explore the prevalence and determinants of concomitant use with BZDs or Z-hypnotics. METHODS: We conducted a retrospective coho

Different Doses of Ropivacaine either with Sufentanil or with Dexmedetomidine for Labor Epidural Anesthesia regarding Painless Childbirth: A Retrospective, Multicenter Study.

INTRODUCTION: Sufentanil is used with ropivacaine in labor pain management but it can have respiratory depression. Dexmedetomidine is effective to maintain hemodynamic stability in parturient women and allow awake intubation. However, dexmedetomidine is suggested only in patients with major cardiovascular disease that mandates stable hemodynamics (Reference ID: 3987541-USFDA). The objective of the study was to compare different doses of ropivacaine either with sufentanil or with dexmedetomidine for epidural anesthesia regarding labor pain management. METHODS: Parturient women have received 0.

Cash Transfers and Their Effect on Maternal and Young Children's Health: A Randomized Clinical Trial.

IMPORTANCE: Mothers and children in low-income households are more likely to experience worse mental and physical health than those from higher-income households. OBJECTIVE: To determine the effect of 4 years of monthly unconditional cash transfers on the mental health of mothers with low-income and the physical health of mothers and children. DESIGN, SETTING, AND PARTICIPANTS: This was a parallel-group, randomized clinical trial conducted from May 2018 to July 2023. Mother-infant dyads were recruited (May 2018-June 2019) from postpartum wards in 12 hospitals in 4 cities: Omaha, Nebraska; Mi

Postmarket Safety Surveillance of FDA-Cleared Prescription Digital Therapeutics Using the Manufacturer and User Facility Device Experience (MAUDE) Database: A Pharmacovigilance Study.

Background and aim Prescription digital therapeutics (PDTs) are software-based medical treatments authorized by the U.S. Food and Drug Administration (FDA) for a range of chronic conditions. As clinical adoption increases, postmarket surveillance becomes essential to ensure safety beyond controlled trial environments. This study aimed to characterize postmarket adverse events and product problems associated with FDA-cleared PDTs using data from the FDA's Manufacturer and User Facility Device Experience (MAUDE) database. This approach supports the generation of hypothesis-forming safety signals

Intimate partner violence identified through routine antenatal screening and maternal and perinatal health outcomes.

BACKGROUND: This study investigated the association between intimate partner violence (IPV) identified on routine prenatal screening and perinatal outcomes for mother and infant. METHODS: Routinely collected perinatal data for a cohort of all women and their infants born in public health facilities in Sydney (Australia) over the period 2014-2016 (N = 52,509) were analysed to investigate the risk of adverse maternal and perinatal outcomes associated with a history of IPV. The association between an affirmative response on prenatal IPV screening and low birth weight (LBW) < 2.5 kg, preterm birt

Risks and predictors of readmission for a mental disorder during the postpartum period.

CONTEXT: It has been suggested that the risk of inpatient psychiatric readmissions is elevated during the postpartum period. To our knowledge, no prior study has compared mothers and nonmothers to determine whether the risk of readmission differs between these 2 groups of women. OBJECTIVES: To compare mothers and nonmothers to assess whether childbirth increases the risk for psychiatric readmission and to identify predictors of psychiatric readmission during the postpartum period. DESIGN: A population-based cohort study merging data from the Danish Civil Registration System and the Danish Ps

Pacific Islands families study: intimate partner violence and postnatal depression.

AIM: The present study examined the association between maternal intimate partner violence (IPV) and postnatal depression (PND) 6 weeks after giving birth. STUDY DESIGN: Data were gathered from the Pacific Islands Families Study. Mothers of a cohort of Pacific infants born in Auckland, New Zealand during 2000 were interviewed 6 weeks after giving birth. There were 1,085 mothers cohabiting in married or de-facto partnerships who completed measures of IPV and PND at the 6-week assessment point. RESULTS: Women who were victims of physical violence were more likely to report postnatal depressive

Acute psychiatric disturbance in pregnancy and the puerperium.

Psychiatric disturbance occurring during pregnancy or the puerperium can occur as a first presentation or as a recurrence or deterioration of a previously diagnosed psychiatric illness. Whatever the history or aetiology, the occurrence of psychiatric disturbance during this period can have profound, long-term implications not only for the mental health of the mother, but also for her relationship with her child and other family members, as well as for the child's development. The treatment of psychiatric disturbance at this time can present difficult dilemmas and should always involve a carefu
