The extraoral examination from the perspective of the patient.

PURPOSE: Literature suggests that the extraoral examination frequently is not offered to patients in the dental setting. The main purpose of this study was to learn more about the patients' perspective regarding the extraoral examination as performed during a dental hygiene appointment. Also of interest was the frequency with which the subjects reported having had an extraoral examination performed on them at a prior dental or medical visit. The importance of the role and responsibility of the dental hygienist in regards to performing the extraoral examination was discussed as a means of demon

Neurodevelopmental/neuroradiologic recovery of a child infected with HIV after treatment with combination antiretroviral therapy using the HIV-specific protease inhibitor ritonavir.

BACKGROUND: Neurodevelopmental impairment has been identified in children infected with human immunodeficiency virus (HIV). The frequency and spectrum of neurologic impairment are greater in children than those reported for adults. In children, HIV is known to enter the central nervous system early in the course of the disease. The presentation of pediatric neuro-acquired immune deficiency syndrome ranges from static (eg, nonprogressive developmental delay) to progressive encephalopathy (eg, acquired microcephaly, pyramidal tract signs, and spasticity). It has been demonstrated that antiretrov

Rapid serial processing in patients with multiple sclerosis: the role of peripheral deficits.

This study compared speed of information processing in patients with relapsing-remitting or secondary progressive multiple sclerosis (MS) and healthy controls using the Stroop Test and a Picture Naming Test (PNT). While both tests evaluated processing speed within a format calling for rapid serial processing of stimulus information, the PNT included trials designed to impose greater verbal-motor and ocular-motor challenges by using novel rather than repeated pictures and by presenting the pictures in distributed locations rather than always centered on the screen. The results confirmed that a

Comparative assessment of oral hygiene awareness and periodontal knowledge among caregivers of children with and without intellectual disabilities: a cross-sectional comparative study.

OBJECTIVES: Individuals with intellectual disabilities (ID) experience disproportionately higher rates of periodontal disease. While biological vulnerability contributes, the oral health knowledge and awareness of primary caregivers is increasingly recognized as a critical modifiable determinant. This study aimed to explore potential differences in the periodontal knowledge, oral hygiene awareness, personal hygiene practices, and child-directed supervision behaviors of caregivers of children with ID versus caregivers of neurotypical children. METHODS: A cross-sectional comparative study was c

Neurodevelopment at Age 9 Years Among Children Born at 32 to 36 Weeks' Gestation.

IMPORTANCE: Although children born moderate to late preterm (MLP; 32-36 weeks' gestation) have more neurodevelopmental problems compared with children born early term or later (≥37 weeks' gestation), detailed understanding of affected domains at school age is lacking. Little is known of risk factors for poorer development. OBJECTIVE: To examine whether being born MLP compared with being born early term or later is associated with neurodevelopmental outcomes at age 9 years and to describe factors associated with poorer neurodevelopment in children born MLP. DESIGN, SETTING, AND PARTICIPANTS:

'Am I being over-sensitive?' Women's experience of sexual harassment during medical training.

Despite larger numbers of women in medicine and strong statements against gender discrimination in written policies and the medical literature, sexual harassment persists in medical training. This study examines the everyday lives of women and men resident physicians to understand the context within which harassment unfolds. The narratives explored here reveal how attention is deflected from the problem of sexual harassment through a focus on women's 'sensitivity'. Women resist by refusing to name sexual harassment as problematic, and by defining sexual harassment as 'small stuff' in the conte

Diffusion model of pain language and quality of life in orofacial pain patients.

AIMS: To address the following questions: (1) Which words are preferred by different groups of orofacial pain patients to describe their pain experience? (2) Is it possible, based on such descriptions, to obtain a clinical differential diagnosis in these patients? (3) Is there any relationship between the verbal description of pain and self-rated quality of life (QOL)? (4) Can a pattern of modulation of pain language by affective variables (diffusion model) be recognized in orofacial pain patients, as it has in other chronic pain patients? and (5) If so, what might be the clinical usefulness o

Locus-Coeruleus Norepinephrine Functioning as a Predictor of Childhood Mental Health (LOCUS-MENTAL): Protocol for a Longitudinal Study.

BACKGROUND: Mental health disorders (MHDs) remain a leading cause of the global burden of diseases. Early identification of neurobiological mechanisms mediating a risk for MHDs is key to reducing a lifetime burden. Recent findings emphasize the locus coeruleus-norepinephrine (LC-NE) system as a neuromodulator of arousal translating acute stress responses into neuronal excitability. We propose that individual differences in LC-NE functioning can explain a differential susceptibility to psychological adversity, which mediates the development of transdiagnostic psychopathology in early childhood.

Comparison of One-Year Post-Operative Evolution of Children Born of COVID-19-Positive Mothers vs. COVID-19-Negative Pregnancies Having Congenital Gastrointestinal Malformation and Having Received Proper Parenteral Nutrition during Their Hospital Stay.

Background: The long-term effects on neonates born of COVID-19-positive pregnancies are still unclear. Congenital gastrointestinal malformations (CGIMs) often require urgent surgical intervention and antibiotic coverage. We aim to compare the health status at one-year post-surgical follow-up of cases of CGIM born of COVID-19-positive pregnancies to those of non-COVID-19 pregnancies. Methods: We conducted a comparative, observational study of 41 patients who underwent surgery at our hospital for congenital gastrointestinal malformations in 2022. They were initially treated with antibiotics and

Hydroxyurea maintains working memory function in pediatric sickle cell disease.

Pediatric patients with sickle cell disease (SCD) have decreased oxygen-carrying capacity in the blood and reduced or restricted cerebral blood flow resulting in neurocognitive deficits and cerebral infarcts. The standard treatment for children with SCD is hydroxyurea; however, the treatment-related neurocognitive effects are unclear. A key area of impairment in SCD is working memory, which is implicated in other cognitive and academic skills. N-back tasks are commonly used to investigate neural correlates of working memory. We analyzed functional magnetic resonance imaging (fMRI) of patients

Maternal Phenylketonuria: Long-term Outcomes in Offspring and Post-pregnancy Maternal Characteristics.

Maternal phenylketonuria (MPKU) is a well-recognized complication of PKU and one of the most potent teratogenic syndromes of pregnancy. Virtually all offspring from untreated pregnancies in women with classic PKU have intellectual disabilities and microcephaly. Congenital heart disease and intrauterine growth retardation occur many times more often than expected in the general population. Control of maternal blood phenylalanine during pregnancy prevents most if not all of these complications. Previous studies demonstrated the benefits of treatment in terms of birth parameters and early develop

GPs' perspectives of type 2 diabetes patients' adherence to treatment: A qualitative analysis of barriers and solutions.

BACKGROUND: The problem of poor compliance/adherence to prescribed treatments is very complex. Health professionals are rarely being asked how they handle the patient's (poor) therapy compliance/adherence. In this study, we examine explicitly the physicians' expectations of their diabetes patients' compliance/adherence. The objectives of our study were: (1) to elicit problems physicians encounter with type 2 diabetes patients' adherence to treatment recommendations; (2) to search for solutions and (3) to discover escape mechanisms in case of frustration. METHODS: In a descriptive qualitative

Caregiver-reported outcomes with real-world use of cannabidiol in Lennox-Gastaut syndrome and Dravet syndrome from the BECOME survey.

PURPOSE: Plant-derived highly purified cannabidiol (CBD) reduced the frequency of seizures associated with Lennox-Gastaut syndrome (LGS) and Dravet syndrome (DS) and improved the overall condition of patients in placebo-controlled phase 3 clinical trials. Anecdotal reports also suggest a positive effect on nonseizure outcomes. In this study, we aimed to identify, through a caregiver survey which nonseizure outcomes were most likely to change in these patients. METHODS: The BEhavior, COgnition, and More with Epidiolex® (BECOME) was a 20-minute, cross-sectional, online survey that was developed

Dynamic Reconfiguration of Cognitive Networks and Recovery From Microlesion Effects in Parkinson's Disease: Insights From a Longitudinal fNIRS Study.

BACKGROUND: Bilateral subthalamic nucleus deep brain stimulation (STN-DBS) significantly improves motor symptoms in advanced Parkinson's disease (PD). However, the perioperative "microlesion effect" (MLE) is often associated with cognitive dysfunction, notably declines in verbal fluency (VFT). The dynamic neural mechanisms underlying cognitive network impairment during the MLE phase and functional reorganization following DBS stimulation remain poorly understood. AIMS AND METHODS: This study employed longitudinal task-based functional near-infrared spectroscopy (fNIRS) to prospectively track

Why primary caregivers fail to report pediatric pain: A qualitative study.

BACKGROUND: In many East Asian hospital settings where family members are closely involved in bedside care, caregivers play a crucial role in recognizing and reporting children's pain during hospitalization. Yet they often fail to inform healthcare providers, leading to inadequate interventions and prolonged suffering. While previous studies focused on whether pain is reported, the psychological mechanisms underlying non-reporting remain poorly understood, limiting the development of effective support strategies. OBJECTIVE: To explore why primary caregivers failed to report pediatric pain to
