Preventing escalation in customer service: immediate motivation and 4-week outcomes in a workplace deployment of LLM role-play training.

Many customer support employees encounter aggressive customer attitudes, including overt dissatisfaction, unreasonable demands, and verbal abuse. Such attitudes do not always stem solely from customers: inadequate employee responses can amplify dissatisfaction and escalate initially manageable issues, making skills that prevent escalation critical. Recent advances in large language models (LLMs) enable role-play training with virtual interlocutors. However, role-play systems in which virtual customer attitudes shift in response to the trainee's utterances under pedagogically grounded rules rem

Evolution of chronic diseases and multimorbidity clusters among the deceased in rural northeast South Africa (2012-2022): insights from verbal autopsy data.

INTRODUCTION: Rural South Africa is undergoing an epidemiological transition characterised by rising mortality from chronic non-communicable diseases (NCDs) alongside that from persistent infectious diseases. We aimed to investigate trends in the prevalence of chronic disease and multimorbidity clustering among deceased individuals in rural northeast South Africa from 2012 to 2022. METHODS: Verbal autopsy data for 5,892 deceased individuals collected from the Agincourt Health and socio-Demographic Surveillance System (HDSS) study area in Bushbuckridge, Mpumalanga Province, from 2012 to 2022 w

Exploring the impact of the adaptive disclosure technique on reducing the severity of post-traumatic stress disorder symptoms in veterans: a study protocol for a randomized controlled trial.

BACKGROUND: Post-traumatic stress disorder (PTSD) presents significant mental health challenges for veterans, leading to substantial physical, cognitive, and psychological difficulties, including heightened risks of social issues and suicidal ideation. Adaptive Disclosure (AD) is a novel, emotion-focused Cognitive Behavioral Therapy (CBT) approach developed by the military for combat-related PTSD. This technique involves exposure therapy and an empty-chair technique, allowing patients to verbalize unresolved issues. Despite its short, six-session duration, AD appears well-tolerated and effecti

Effectiveness of newborn infant parasympathetic evaluation (NIPE) index for guiding intraoperative fentanyl administration in children under 2 years: a randomized controlled trial.

BACKGROUND: Assessing pain in infants and neonates is challenging due to their inability to communicate verbally. While validated subjective tools exist, they rely on interpreting the child's behavior, leading to potential inconsistencies and underestimation of pain. Based on heart rate variability, the newborn infant parasympathetic evaluation (NIPE) index offers a more objective approach to pain assessment in children under 2 years. Although promising, research on its effectiveness during surgery under general anesthesia remains limited and inconclusive. OBJECTIVE: This study compared the e

Financial Health of People Living With Dementia and Their Informal Care Partners: Protocol for a Mixed Methods Study.

BACKGROUND: There is a growing body of academic literature focusing on the significant financial burdens placed on people living with cancer, but little evidence exists on the impact of rising costs of care in other vulnerable populations. This financial strain, also known as financial toxicity, can impact behavioral, psychosocial, and material domains of life for people diagnosed with chronic conditions and their care partners. New evidence suggests that populations experiencing health disparities, including those with dementia, face limited access to health care, employment discrimination, i

Effect of initial levothyroxine dose on neurodevelopmental and growth outcomes in children with congenital hypothyroidism.

OBJECTIVES: We designed a multicentre open prospective randomized trial to evaluate the risk-benefit profile of two different initial treatment schemes with levothyroxine (L-T4), 10-12.5 μg/kg/day vs 12.6-15 μg/kg/day, on growth and neurodevelopmental outcomes in children with congenital hypothyroidism (CH) detected by neonatal screening to identify the best range dose to achieve optimal neurocognitive development. DESIGN PATIENTS AND METHODS: Children detected by neonatal screening were randomly assigned to receive an initial L-T4 dose of 10-12.5 μg/kg/day (Low) or 12.6-15 μg/kg/day (High).

Efficacy of a digital cognitive behavioral therapy for insomnia in people with low back pain: a feasibility randomized co-twin and singleton-controlled trial.

BACKGROUND: Digital cognitive behavioral therapy for insomnia (CBT-i) in people with low back pain (LBP) may be efficacious in improving both sleep and pain; and twin trial designs provide greater precision of treatment effects by accounting for genetic and early environmental factors. We aimed to determine the feasibility of a trial investigating the efficacy of a digital CBT-i program in people with comorbid symptoms of insomnia and LBP, in twins and people from the general community (singletons). METHODS: Thirty-two twins (16 pairs) and 66 singletons with comorbid symptoms of insomnia and

Effects of recombinant human erythropoietin on cognition and neural activity in remitted patients with mood disorders and first-degree relatives of patients with psychiatric disorders: a study protocol for a randomized controlled trial.

BACKGROUND: Bipolar disorder (BD) and unipolar disorder (UD) are associated with cognitive deficits and abnormal neural activity in a "cognitive control network." There is an increased prevalence of cognitive dysfunction in psychiatric patients' first-degree relatives, which constitutes a risk factor for psychiatric illness onset. However, there is no treatment with enduring pro-cognitive efficacy. We found preliminary evidence for beneficial effects of eight weekly doses of recombinant human erythropoietin (EPO) on cognition in BD in a recent randomized controlled trial (RCT). The present RCT

Peripheral CCK and ß-Endorphin as Potential Neuroendocrine Markers of Nocebo Effects: Evidence from a Multiple-Threat Pain Paradigm.

INTRODUCTION: Nocebo effects are shaped by negative treatment-related expectations and experiences and can contribute to adverse treatment outcomes. They have been linked to neuroendocrine alterations, yet reliable peripheral biomarkers remain elusive. Although cholecystokinin (CCK) has long been implicated in nocebo hyperalgesia, direct evidence in humans remains limited. β-Endorphin (β-END) is another promising candidate given its established role in endogenous pain modulation and placebo analgesia. The present study aimed to investigate whether peripheral CCK and β-END responses are associa

Complex ADHD in a Child With Tic Disorder.

Jaxon is a healthy 12-year-old boy who is referred to your clinic for medication management. He was diagnosed with ADHD using a validated questionnaire at age 9 years. He is currently prescribed OROS methylphenidate 54 mg capsules once daily in the morning, which he has taken for 3 years. This has generally worked well to improve his attention, focus, task persistence, and task completion. However, his parents now report increasing disruptive behaviors over the past 18 months, both at home and at school. There have been no major stressors over this period nor significant life changes or trauma

"They talked to me rudely". Women perspectives on quality of post-abortion care in public health facilities in Kenya.

BACKGROUND: Access to safe abortion is legally restricted in Kenya. Therefore, majority women seeking abortion services in such restrictive contexts resort to unsafe methods and procedures that result in complications that often require treatment in health facilities. Most women with abortion-related complications end up in public health facilities. Nevertheless, evidence is limited on the quality of care provided to patients with abortion complications in public health facilities in Kenya. METHODS: Data for this paper are drawn from a qualitative study that included interviews with 66 women

Sex differences in structural brain asymmetry of children with autism spectrum disorders.

Previous studies have confirmed the sex difference of gray matter asymmetry in typically developing controls and the abnormal gray matter asymmetry in autism spectrum disorders. However, whether and how sex differences of gray matter asymmetry exist in autism spectrum disorders remains studied. This paper analyzes the above issues and explores correlations between gray matter asymmetry and autistic symptoms. Data from 72 children (36 males and 36 females) with autism spectrum disorders and 72 typically developing-controls (36 males and 36 females) between 8 and 14 years were included and obtai

Representational Organization of Novel Task Sets during Proactive Encoding.

Recent multivariate analyses of brain data have boosted our understanding of the organizational principles that shape neural coding. However, most of this progress has focused on perceptual visual regions (Connolly et al., 2012), whereas far less is known about the organization of more abstract, action-oriented representations. In this study, we focused on humans' remarkable ability to turn novel instructions into actions. While previous research shows that instruction encoding is tightly linked to proactive activations in frontoparietal brain regions, little is known about the structure that

Students' Perceptions of Trigger Warnings in Medical Education.

UNLABELLED: Phenomenon: Trigger warnings are verbal statements or written warnings that alert students in advance to potentially distressing material. Medical education includes numerous subjects frequently identified as triggers, such as abuse, rape, self-injurious behaviors, eating disorders, drug and alcohol addiction, and suicide. Thus, exploring medical students' perceptions of trigger warnings may provide a valuable perspective on the use of these warnings in higher education. APPROACH: As part of a larger descriptive, cross-sectional survey study on medical education, we assessed 1st-

Selective cognitive and psychiatric manifestations in Wolfram Syndrome.

BACKGROUND: Wolfram Syndrome (WFS) is known to involve diabetes mellitus, diabetes insipidus, optic nerve atrophy, vision loss, hearing impairment, motor abnormalities, and neurodegeneration, but has been less clearly linked to cognitive, sleep, and psychiatric abnormalities. We sought to determine whether these abnormalities are present in children, adolescents, and young adults with WFS compared to age- and gender-matched individuals with and without type 1 diabetes using standardized measures. METHODS: Individuals with genetically-confirmed WFS (n = 19, ages 7-27) were compared to age- and
