A patient with bilateral parietal damage, and Balint's syndrome, named visual letters. These were presented individually or within four-letter strings. Solitary letters were identified very accurately. In the case of strings, more letters were correctly reported for words than for nonwords, and more for pronounceable than for unpronounceable nonwords. When required to read words as a whole, performance was better than predicted by letter-reports. These results extend the object-based limitation apparent in Balint's syndrome to the case of reading. The component letters of a string benefit when

Three hemifield tachistoscopic studies of a right-handed patient with acquired Broca's aphasia and deep dyslexia, but with intact visual fields, are presented to support the hypothesis of right hemisphere (RH) participation in deep dyslexic reading. A systematic comparison of this deep dyslexic with the disconnected RHs of two patients with complete cerebral commissurotomy disclosed a similar, but not identical, pattern of abilities. The results demonstrate partial reliance on the RH for accessing the meaning of single words, but not for phonological encoding in this patient.

A patient with intractable partial seizures was intensively studied before surgical removal of the epileptogenic focus. A subdural electrode array was surgically placed over the left temporoparietal cortex to better localize the epileptogenic focus and localize cortical function. In addition to speech and sensory findings, acalculia, agraphia, right-left confusion, and finger agnosia were transiently produced by electrical stimulation in the perisylvian area. These findings and their relationship to the controversy surrounding Gerstmann's syndrome are discussed.

A 82 year-old right-handed man, without any intellectual impairment, suffered from an acute neurological deficit consisting in letter-by-letter reading, right superior quadrant hemianopia with achromatopia in the lower quadrant, and anomia. Cerebral MRI showed an infarct involving the ventral structures of the left hemisphere sparing the splenium of the corpus callosum and the thalamus. Neuropsychological examination revealed that the patient easily identified the objects, the animals and the famous places he could not name: his comments attested normal visual recognition. Conversely, when he

The question as to whether a failure of recognition unrelated to impaired sensory processing or to disorder of naming can occur in the somato-sensory modality has been eagerly debated in the french neurology. Taking as an argument the fact that he had never observed a tactile agnosia in the absence of subtle sensory deficits Dejerine denied the localizing value of tactile agnosia (or asterognosis). Conversely Delay, 20 years later, identified tactile performances such as discrimination of texture and shapes, which he considered as a specific neocortical function, that were lost in parietal syn

Neurocysticercosis commonly presents with seizures, raised intracranial tension and dementia. The unusual location of the cysts may result in uncommon manifestations mimicking a host of neurological disorders. Ten patients with neurocysticercosis with rare clinical presentations have been described in this series. These include dorsal midbrain syndrome, isolated bilateral ptosis, papillitis, cerebral hemorrhage, painful cervical radiculopathy, progressive swelling of arm, paraplegia due to intramedullary cyst, third ventricular cyst, dystonia and nominal aphasia masquerading as transient ischa

As part of a 5-year study of the longitudinal effects of Alzheimer disease (AD) on language, discourse samples from three patients diagnosed with AD and three matched normal controls were analyzed. A picture-description task was used to elicit comparable samples of discourse. Subjects were asked to describe the "Easter Morning" picture by Norman Rockwell. Samples were tape recorded, transcribed, and scored using eight measures of discourse production. These measures successfully quantified the changes in quantity and quality observed in the discourse of AD patients over time. Results revealed

We report two head-injured patients whose knowledge of living things was selectively disrupted. Their semantic knowledge was tested with naming and verbal comprehension tasks and a verbal questionnaire. In all of them there was consistent evidence that knowledge of living things was impaired and that of non-living things was relatively preserved. The living things deficit emerged irrespective of whether the question tapped associative or perceptual knowledge or required visual or non visual information. In all tasks the category effect was still significant after the influence on the performan

A case of callosal disconnection syndrome caused by a penetrating stab wound of the brain is reported. A portion of the genu and the inferior splenium of the corpus callosum were sectioned by an ice pick without any other significant cerebral damage. We could detect incomplete disconnection left visual alexia by tachistoscopy. It would now appear that the inferior part of the splenium has a significant functional role in transmitting visual verbal codes from the right to the left cerebral hemisphere, but the remaining superior fibers of the splenium may carry some information, albeit less effi

Slow event-related potentials (ERP) were examined in healthy and aphasic subjects in two-stimulus designs comprising a word comprehension and a rhyming task. Aphasics, though selected to perform above chance level, made significantly more errors and responded more slowly than controls, although canonical correlations did not indicate a statistical relationship between performance measures and ERP amplitudes. A discriminant analysis of ERP amplitudes distinguished the groups for the slow wave (SW; 0.5-1.0 s post-S1 onset) in the word comprehension, for the SW and the initial contingent negative

We present evidence that semantic errors in object naming can arise not only from impairment to the semantic system but also from damage to input and output processes. Although each of these levels of disruption can result in similar types of semantic errors in object naming, they have different types of consequences for performance on other lexical tasks, such as comprehension and naming to definition. We show that the analysis of the co-occurrence of semantic errors in naming with different patterns of performance in other lexical processing tasks can be used to localise the source of semant

This case report documents the partial recovery, over a 12-month period, of pure alexia in an adult female following a left occipital infarction. Measures of speed and accuracy were obtained on an oral reading and a lexical decision task immediately postonset and then on 10 subsequent occasions. Explicit letter-by-letter reading was observed only during the first week poststroke but a significant effect of word length was seen in all testing sessions. Reading accuracy was relatively good at all stages and reading latency showed a remarkable decrease over time but did not reach normal reading r

This case study concerns an 18-year-old bilingual girl who suffered a radiation lesion in the left (dominant) thalamic and temporal region when she was 4 years old. Language and memory assessment revealed deficits in auditory short-term memory, auditory word comprehension, nonword repetition, syntactic processing, word fluency, and confrontation naming tasks. Both languages (English and Dutch) were found to be affected in a similar manner, despite the fact that one language (English) was acquired before and the other (Dutch) after the period of lesion onset. Most of the deficits appear to be r

The article reports the case of a patient who showed a selective inability in reading multi-digit numbers following recovery from aphasic disorders. Although his ability to read words, non-words, syntagms and sentences was almost preserved, he made errors with Arabic numerals and number words. Different types of errors with alphabetic material were also observed: he made only rare phonological substitutions with linguistic stimuli, whereas errors were almost always lexical in reading number words. A series of experiments showed that his ability to access number semantics was intact. In contras

In the course of increasing demands in acute stroke care in stroke units, criteria for efficient speech diagnosis, which can be applied in the first hours of acute stroke are necessary. In this study we investigated early recognition of speech and speech-associated disorders in acute stroke, and compared the neurologists' clinical diagnoses at admission with the results of a test battery (KAP--Kurze Aphasieprüfung). 90 of 104 consecutively recruited patients could be tested within the first three days. 53.8% (n = 49/N = 91) of the patients presented with language disorders in the clinical neur

A 72-year-old man experienced increasing difficulties in calculating and processing numbers. He made many syntactic errors when he read and wrote numbers, both in arabic and number-word forms. Transcoding between arabic numerals and number-words was severely impaired. His calculation abilities were grossly impaired. He could not perform even simple multiplications, but additions and subtractions were preserved. He was unable to recite multiplication tables or the alphabet. In contrast, numerosity judgment, magnitude comparisons, and semantic evaluation of numbers were intact. The patient's defi

We present a 2-year verbal and nonverbal follow-up of a crossed aphasic patient. The patient had suffered from widespread ischemic damage in the area of right middle cerebral artery, with a parieto-temporal lesion. Three months postonset he showed classical Wernicke's aphasia associated with oral, limb and constructional apraxia and left hemineglect. However, follow-up findings showed a complex, dynamic pattern entirely consistent with cognitive models of language and nonlanguage abilities. Current models of functional brain lateralizations could not satisfactorily account for such longitudina

A 43 year-old man with presumed progressive multifocal leukoencephalopathy developed difficulty recognizing objects and faces in the presence of adequate visual acuity and visual fields. His copying and matching of line drawings was intact, suggesting that his agnosia was associative. However, he had difficulty perceiving overlapping forms and drawings of single objects as integrated wholes. Unlike control subjects, he made fewer errors identifying silhouettes compared to line drawings with internal details. These alterations, together with his feature-by-feature descriptions of objects and co

Two patients had alexias after left occipital lobectomies. Case 1 was a 55-year-old man with a glioblastoma. At 4 months after surgery he could read slowly, but reading was neither efficient nor pleasant. Case 2 was a 19-year-old male who had a more restricted, medial occipital lobectomy for an encapsulated mesenchymal chondrosarcoma. The tumor did not invade brain initially, and the patient recovered efficient reading after 15 months. It is postulated that Case 2 was able to recover efficient reading because he still had a field of left ventrolateral occipitotemporal cortex connected to homol

Twenty-two subjects with presumed senile dementia of the Alzheimer's type (SDAT), twenty-two with dementia other than SDAT, twenty with aphasia, and seventeen healthy controls over 50 years of age were examined for intrusions and perseverations. Perseverations were defined as the immediate inappropriate repetition of a prior response whereas intrusions were defined as the inappropriate repetition of prior responses after intervening stimuli. Intrusions and perseverations occurred in demented and aphasic subjects, but were rare in controls. Intrusions occurred most often in SDAT or Wernicke aph
