The Use of Low-Dose Intermittent Levosimendan Infusion Therapy as Bridge-to-Left Ventricular Assist Device in Advanced Chronic Heart Failure Secondary to Isolated Cardiac Sarcoidosis.

Heart failure (HF) is a progressive condition that usually involves a debilitating late course with poor health-related quality of life and increasing mortality rate. In this report, we demonstrate the efficacy and safety of intermittent levosimendan infusion as a bridging therapy to left ventricular assist device use in a patient suffering from cardiac sarcoidosis who failed to respond to optimized medical therapy. Levosimendan was administered in an outpatient infusion-therapy facility every 2 weeks as a single intravenous infusion over 6 h at dose and rate of 0.2 μg/kg/min not proceeded by

Venetoclax plus azacitidine as genetic-driven bridge-to-transplant therapy for IDH2-mutated acute myeloid leukaemia (AML) refractory to intensive chemotherapy: proof-of-concept case reports.

Despite the greater biological understanding and the new drugs available, acute myeloid leukaemia (AML) patients who are refractory to intensive induction chemotherapy represents an unmet clinical need, especially in young/fit adults who are eligible for bone marrow transplantation. Since venetoclax/azacitidine (ven/aza) was introduced in AML management in 2020, survival of elderly/unfit patients has dramatically improved, especially in those carrying NPM1 or IDH2 mutations. However, the use of ven/aza in young and fit adults remains limited, raising ongoing debate about its potential role bey

Relapsed Refractory Hodgkin Lymphoma and Brentuximab Vedotin-Bendamustine Combination Therapy as a Bridge to Transplantation: Real-World Evidence From a Middle-Income Setting and Literature Review.

INTRODUCTION: Despite high cure rates with standard treatment, 30% patients with Hodgkin lymphoma develop relapsed or refractory (R/R) disease. Salvage therapy followed by autologous hematopoietic cell transplantation (HCT) is considered standard of care. Brentuximab Vedotin (Bv) in combination with Bendamustine (B) has been tested in the salvage setting with promising results. MATERIALS AND METHODOLOGY: We conducted a single centre retrospective chart review of patients who received BBv salvage therapy to determine its activity and safety in patients with R/R classical Hodgkin lymphoma (HL).

Extramedullary relapse of acute lymphoblastic leukemia treated with a CAR-T cell therapy bridge to unrelated cord blood transplantation: a case report and review of the literature.

Extramedullary relapse of acute lymphoblastic leukemia (ALL) is usually associated with poor prognosis. Chimeric antigen receptor T cell (CAR-T cell) therapy followed by allogeneic hematopoietic stem cell transplantation is beneficial for relapsed/refractory (r/r) B cell acute lymphoblastic leukemia (B-ALL). Here, we report a B-ALL patient with extramedullary relapse involving several organs, including multiple lymph nodes and the breast, kidney, uterus and pancreas. After treatment with CAR-T cell therapy, positron emission tomography/computed tomography (PET/CT) revealed that she went into r

Long-term preservation of functional capacity and quality of life in advanced heart failure patients with bridge to transplant therapy: A report from Japanese nationwide multicenter registry.

BACKGROUND: Under the revised heart allocation system in the United States, bridge to transplant (BTT) patients with left ventricular assist device (LVAD) have a longer waitlist period, as they are now lowly prioritized. However, little is known regarding the long-term trajectory of functional capacity (FC) and health-related quality of life (HR-QOL) among BTT-LVAD patients. METHODS: We retrospectively analyzed 442 consecutive patients with BTT-LVAD between April 2013 and May 2019 from a Japanese nationwide registry. FC (New York Heart Association [NYHA] functional class, peak oxygen uptake [

Evaluation of dentin bridge formation and postoperative pain reduction after direct pulp capping using photobiomodulation therapy, platelet-rich fibrin, and mineral trioxide aggregate - A randomized clinical trial.

OBJECTIVE: Direct pulp capping (DPC) preserves pulp vitality and stimulates reparative dentin formation. Photobiomodulation therapy (PBMT) may enhance healing especially with platelet-rich fibrin (PRF) and mineral trioxide aggregate (MTA). This study aims to assess the effects of PBMT, alone or with PRF or MTA, on postoperative pain after DPC and on dentin bridge thickness using cone-beam computed tomography (CBCT). MATERIALS AND METHODS: Sixty patients (17-45 years) with reversible pulpitis in permanent molars were randomly divided into three groups (n = 20 each): Group A: MTA with PBMT (A1)

Compression versus bridge negative pressure wound therapy combined with compression in non-healing venous ulcers: A randomized controlled trial with quality-of-life assessment and cost effectiveness.

ObjectivesNegative pressure wound therapy (NPWT) is a useful adjunct to optimization of wound healing. We aimed to evaluate the benefits of NPWT in the management of refractory non-healing venous leg ulcers (VLU), in conjunction with compression therapy (CT).MethodsA randomised controlled trial where 50 patients with chronic VLU were randomised to either NPWT with CT, or to CT alone, on a 1:1 ratio, for 12 weeks. Sample size was calculated to detect a minimum of 19 days with a standard deviation of 12 days and a power of 95% (α = 5%). Patients were followed-up for 1 year. Patients with mixed a

The Use of Cangrelor as Bridge Antiplatelet Therapy in a Patient with Recent Percutaneous Coronary Intervention for Acute Coronary Syndrome, Who Developed Esophageal Perforation After Transesophageal Echocardiography.

Dual antiplatelet therapy (DAPT) is a vital part of the pharmacological management in patients with coronary artery disease (CAD) undergoing percutaneous coronary intervention (PCI). While early discontinuation of DAPT increases ischemic risk, some patients on DAPT may require urgent surgery, necessitating its interruption. Cangrelor, an intravenous P2Y12 antagonist, provides strong platelet inhibition within minutes and platelet activity normalizes within one hour after the cessation of the drug. Bridging antiplatelet therapy with cangrelor has been increasingly studied as an alternative opti

The use of extracorporeal membrane oxygenation in patients with therapy refractory cardiogenic shock as a bridge to implantable left ventricular assist device and perioperative right heart support.

Implantation of left ventricular assist device (LVAD) as a bridge to recovery or transplantation is a widely accepted treatment modality. Preexisting organ dysfunction is thought to unfavorably affect patient survival after implantation of a ventricular assist device (VAD). We present our experience using extracorporeal membrane oxygenation (ECMO) in patients with cardiogenic shock to stabilized organ function prior to LVAD implantation. Between September 2006 and March 2008, five patients in cardiogenic shock with preexisting organ dysfunction (impaired liver and kidney function) were support

Bridging Evidence and Practice: A Community-Engaged Adaptation of Cognitive Behavioral Therapy for Autistic Youth With Anxiety Receiving Services in Community-Based Organizations.

Differences between research and community care settings contribute to a research-to-practice gap. Engaging community partners in intervention adaptation may better align mental health evidence-based practices (MH-EBPs) with community-based settings and intervention recipients. This study utilized a community-engaged intervention adaptation method (i.e., Adapted Method for Program Adaptation through Community Engagement [AM-PACE]) to develop a community-adapted version of Behavioral Interventions for Anxiety in Children with Autism (BIACA). A Community Advisory Board provided ongoing input abo

Bridging therapy with airway stent placement followed by immune checkpoint inhibitors in patients with lung cancer and malignant airway disorders: A retrospective study.

BACKGROUND: Airway stent placement is used to manage malignant airway obstruction, but often these patients cannot receive immune checkpoint inhibitors (ICIs) because of poor performance status (PS). We evaluated whether stenting could enable ICI therapy and improve outcomes. METHODS: We retrospectively reviewed patients with lung cancer who received anticancer therapy after airway stent placement from January 2016 to July 2025. Outcomes included 1-year overall survival (OS), median survival time (MST), ECOG PS changes, complications, immune-related adverse events, and stent removal. RESULTS

Staged non-bridging circular external fixation with sequential osteoporosis therapy for MRSA-infected distal radius non-union: a case report.

BACKGROUND: Infected distal radius non-union in elderly patients with severe osteoporosis is particularly challenging because infection eradication, stable fixation, and preservation of wrist function must be achieved simultaneously. Conventional internal fixation in an infected osteoporotic field carries a high risk of recurrent infection and fixation failure, whereas bridging external fixation may result in prolonged immobilization and functional deterioration. We report a case of methicillin-resistant Staphylococcus aureus (MRSA)-infected distal radius non-union treated with staged non-brid

Water-Soluble Ruthenium(II) p-Cymene Complexes Mitigating Intracellular S. aureus Infection: Bridging Antibacterial Défense and Anticancer Therapy.

The global rise of antimicrobial resistance and aggressive cancers like triple-negative breast cancer (TNBC) demands dual-functional therapies. We synthesized water-soluble ruthenium(II) p-cymene complexes (RuL1-RuL5) using quinoxaline-pyridyl ligands with strategic C6-substitutions. Structure-activity relationship studies identified halogenated RuL1 (-Br) as the most potent, showing selective activity against Gram-positive bacteria (MIC = 8 μg/mL vs Staphylococcus aureus). Mechanistic studies revealed membrane-targeted antibacterial action, supported by AFM, SEM, and fluorescence assays showi
