The third part of our 3-years' continuous investigation on the clinical effect of cefaclor (CCL) against urinary tract infections was reported herein. CCL, a daily dose of 0.75 g t.i.d., has been applied for the treatment of (I) 69 cases with the uncomplicated acute cystitis in the women, and (II) 44 cases with the complicated. The clinical efficacy and safety of cefaclor, a new oral cephalosporin, was compared with that of amoxycillin in the treatment of lower urinary tract infections in 61 patients with chronic non-urinary tract disease. Thirty-six patients received cefaclor and 25 received amoxycillin doses of 250 mg every 8 hours for 7 to 10 days. Bacteriologic investigations were performed on 100 children with acute otitis media by culturing the fluid from a myringotomy site. Patients ranged in age from 7 months to 14 years, and 91 were younger than 6 years old. Sixteen cases of severe osteomyelitis and septic arthritis caused by staphylococci, streptococci, gonococci, and a variety of gram-negative bacilli were treated with 4 to 8 g of parenteral cefazolin per day; nine received subsequent therapy with oral cephalexin or ampicillin. We report a case of osteomyelitis of the proximal femur caused by Lancefield group G streptococcus in a 71-year-old otherwise healthy man. The organism has rarely been identified as the cause of osteomyelitis. The subacute nature of the symptoms and the radiological appearance of the femur in this patient mimic bone tumour. The patient was successfully treated with conservative methods, including a prolonged period of oral antibiotics. We stress the importance of histological and bacteriological evidence in avoiding misdiagnosing patients with equivocal clinical and radiological presentation.

A 72-year-old woman with Sjögren's syndrome manifested acute fulminating respiratory failure and was treated with high-dose corticosteroids. Autopsy revealed overwhelming pulmonary infection by Blastomyces dermatitidis with no evidence of extrapulmonary dissemination. Hyphal forms, usually not observed in vivo, were noted in premortem sputum samples as well as in sections of lung obtained at autopsy. It is proposed that the immunocompromised state of the patient played a pathogenetic role in the fulminating clinical course as well as in the production of filamentous forms of B. dermatitidis.

Thirty patients suffering from bacteriogenic skin diseases were given cefaclor, in most cases simultaneously with classical external dermatological therapy. The results of therapy were satisfactory in 29 of the 30 patients. Only in one relatively severe case of erysipelas did the disease continue to spread despite the daily doses of 3 x 500 mg of oral cefaclor prescribed by the dosage regimen. Side-effects observed consisted of diarrhoea in one patient, and a transient slight increase of SGOT or alkaline phosphatase in another six patients.
