Radiographic changes in the operative treatment of acute acromioclavicular joint dislocation - tight rope technique vs. K-wire fixation.

BACKGROUND: Operative treatment of higher degree acromioclavicular joint luxation is common. A new option is made available by the tight rope technique. It claims to provide adequate outcome with the use of a minimally invasive technique. First clinical studies justified its medical use, but the equivalence to established surgical methods remains unclear. We therefore analyzed radiographic data from patients that were treated with the tight rope system (TR) and compared them to those treated with K-wires (KW) fixation. MATERIAL/METHODS: RETROSPECTIVE STUDY WITH INCLUSION CRITERIA: surgery for

In vivo monitoring of the degradation process of bioresorbable polymeric implants using magnetic resonance imaging.

The degradation of a bioresorbable poly(L-lactide) osteosynthesis plug after fixation of a transferred coracoid bone block using the Bristow-Latarjet procedure was examined by means of magnetic resonance (MR) imaging. There were 15 patients with a mean follow-up time of 38 months (range 34-42 months). The gross geometry of the biomaterial plug remained unaltered on the MR images. The bone marrow signal reached the implant surfaces in all cases, and no signs of liquid phase around the implant could be discerned. The implants themselves were visible as homogeneous low signal intensity (SI) black

Mapping out the axillary nerve: A cadaveric study.

INTRODUCTION: The axillary nerve is a crucial peripheral branch arising from the posterior cord of the brachial plexus. It provides both motor and sensory innervation to the shoulder girdle, enabling essential movements such as abduction and external rotation-functions that are crucial for daily activities and various athletic movements. Despite its significance, detailed documentation of the axillary nerve's branching patterns remains limited. This study aimed to examine the branching configurations of the axillary nerve in a South African cadaveric sample. MATERIALS AND METHODS: Thirty adul

A complex glenoid fracture following high-energy trauma: Case report and literature review.

RATIONALE: Glenoid fractures, representing approximately 10% of scapular fractures, are uncommon but clinically significant due to their frequent association with high-energy trauma. Without timely and adequate management, such injuries may lead to complications including nonunion, osteoarthritis, and chronic instability, especially when fracture patterns are complex or extend into the scapular body. This report highlights the unique diagnostic and therapeutic challenges of managing an Ideberg type Vb glenoid fracture. PATIENT CONCERNS: A 32-year-old male motorcyclist presented with significa

Morphological variability of the pectoralis minor muscle. Study in human fetuses.

BACKGROUND: The pectoralis minor muscle is located in the anterior thoracic wall. Typically, is constituted by a single belly originating from the 3rd to the 5th rib and inserted into the coracoid process near the origins of the biceps brachii shorth head and of the coracobrachialis muscle. The current study, on human fetuses, aims to detect all morphological muscle variants and to create a new classification system. MATERIAL AND METHODS: Classical dissection of the thoracic wall and the upper limb was bilaterally performed on 25 (13 male and 12 female) human formalin-fixed fetuses aged 18-38

Total glenoid fractures.

INTRODUCTION: Fractures affecting the entire glenoid fossa are termed comminuted or total glenoid fractures. However, there are no detailed studies of total glenoid fractures, and only brief mentions can be found in the literature. METHODS: The group comprised 12 patients (mean age, 39 years), who sustained 13 fractures of the glenoid fossa. In all the fractures, all parts of the glenoid fossa were separated from the scapular neck or body. In total 5 patients (6 fractures) were treated non-operatively and 7 patients were operated on. The method of treatment was based on displacement of the fr

Accuracy of breast cancer axillary lymph node treatment plans based on 2-dimensional imaging: what we should know before interpreting 2-dimensional treatment-planning era studies.

OBJECTIVE: To investigate axillary 2-dimensional treatment planning accuracy. METHODS: Computed tomography (CT) simulation data for 16 breast cancer cases taken after level I-II axillary dissection were analyzed. An additional 6 patients underwent CT simulation using the historical 90-degree position (HP), and the standard-bore CT position (CT-P). Two physicians identified the lateral and medial borders of the coracoid process (CCP) on digitally reconstructed radiography (DRR). The DRR-identified x coordinates were compared with the CT-measured x coordinates. x coordinates differences between

The anatomy of the brachial plexus as displayed by magnetic resonance imaging: technique and application.

Full field of view coronal chest magnetic resonance imaging (MRI) routinely displays bilateral images of the brachial plexus, surface anatomy, and anatomic structures. Eighty patients had chest radiographs correlated with surgery for thoracic outlet syndrome. The PA chest film findings correlated with the surgical findings: smaller thoracic inlet on the concave side of the cervicothoracic spine scoliosis, shorter distance between the dorsal spine of the second or third thoracic vertebral body to the concavity of the first ribs, asymmetric clavicles and coracoid processes, synchondrosis of the

Prevention and management of early treatment failures in elbow injuries.

The appropriate management of early treatment failures in patients with elbow injuries requires the identification of the cause of failure. In this work, six types of elbow injury are considered: (i) identification of early failed reduction of a dislocation or fracture-dislocation of the elbow should lead to a repeat reduction procedure, testing for elbow instability, and ligament repair, followed by the use of a hinged external fixator to allow early mobilisation. Differentiating an isolated dislocation from a dislocation combined with a fracture of the coracoid process is crucial. Re-implant

Does the choice of the optic portal influence the radiographic and early functional results in acute acromioclavicular disjunctions?

Exposure of the coracoid process during arthroscopic stabilization of acute acromioclavicular disjunctions (ACDs) can be achieved either by passing an extra- articular optical portal through the subacromial space or by an intra-articular optical route through the glenohumeral joint with opening of the rotator interval. The objective of our study was to compare the impact on the functional results of these two optical routes. This was a retrospective, multicentre study that included patients operated on for an acute acromioclavicular disjunction arthroscopically. The treatment consisted of surg

Outpatient Latarjet surgery for gleno-humeral instability: Prospective comparative assessment of feasibility and safety.

BACKGROUND: Some surgical procedures are rarely done on an outpatient basis. The primary objective of this study was to assess the safety of outpatient surgical shoulder stabilisation using the Latarjet procedure. HYPOTHESIS: The Latarjet procedure is safe when performed on an outpatient basis provided the patients are managed according to a specifically designed programme starting at the decision to undergo surgery and ending at the end of the early postoperative period. PATIENTS AND METHODS: Consecutive patients with unidirectional anterior shoulder instability managed in 2013-2014 by prim
