Patterns of urethral injury and immediate management.

PURPOSE OF REVIEW: The management of urethral trauma remains controversial. The purpose of this review is to provide a concise account of the patterns of urethral injury and the current trends of its immediate management. RECENT FINDINGS: Recent studies have identified types of pelvic fractures associated with a higher risk of urethral injury. Recognition of these fractures coupled with timely radiographic imaging has facilitated the earlier diagnosis of urethral injury and ensured that serious long-term sequelae are minimized. In highly selected cases of complete anterior and posterior ureth

State of the Art: Single-Port Robot-Assisted Extraperitoneal Transvesical Surgery-Technique, Indications, Outcomes, and Pitfalls.

BACKGROUND: The transvesical route provides direct access to the bladder and prostate whereas avoiding peritoneal entry. With the advent of the da Vinci Single-Port (SP) platform, the extraperitoneal (EP) transvesical procedure has reemerged as a minimally invasive option for benign, oncologic, and reconstructive urologic procedures. However, guidance regarding setup, patient selection, outcomes, and technical challenges remains limited. OBJECTIVES: To provide a procedure-oriented overview of the SP robot-assisted EP transvesical procedure, focusing on access, indications, outcomes, and techn

The new gold standard for surgical management of BPH: an institutional experience with 1000 HoLEPs.

INTRODUCTION: Holmium Laser Enucleation of the Prostate (HoLEP) is a size-independent, endoscopic management option for benign prostatic hyperplasia (BPH). HoLEP offers a distinct advantage for patients who are at high-risk for bleeding whilst preserving prostatic tissue for pathology analysis, unlike photoselective vaporization. Further, HoLEP avoids the need for cystotomy, unlike simple open and robotic prostatectomy, by using intravesical morcellation. We report our experience with the first 1000 HoLEP procedures at our institution. MATERIALS AND METHODS: We performed a retrospective revie

Modified paravaginal repair combined with sacrospinous ligament suspension for pelvic organ prolapse: a randomized controlled study.

BACKGROUND: Native tissue repair surgery for pelvic organ prolapse is associated with a high recurrence rate, and accumulating evidence has highlighted adverse events related to mesh use. This study aimed to assess the efficacy and safety of a modified paravaginal repair technique combined with sacrospinous ligament suspension for treating pelvic organ prolapse. METHODS: This randomized, non-inferiority, endpoint assessor-blinded study included patients with pelvic organ prolapse. Participants were assigned to either the intervention group, who underwent the modified paravaginal repair with s

Transverse versus vertical skin incision for planned cesarean hysterectomy: does it matter?

BACKGROUND: To investigate differences in perioperative outcomes by type of skin incision, transverse versus vertical, for planned cesarean hysterectomy for placenta accreta spectrum (PAS). METHODS: A retrospective cohort study of all women who underwent a planned cesarean hysterectomy for abnormal placentation at a single academic medical center over 5 years. The Student's t-test was used for continuous variables and Fisher's exact test compared categorical variables. Continuous data were presented as median and compared using the Wilcoxon-rank sum test. RESULTS: Forty-two planned cesarean

Maternal morbidity associated with multiple repeat cesarean deliveries.

OBJECTIVE: Although repeat cesarean deliveries often are associated with serious morbidity, they account for only a portion of abdominal deliveries and are overlooked when evaluating morbidity. Our objective was to estimate the magnitude of increased maternal morbidity associated with increasing number of cesarean deliveries. METHODS: Prospective observational cohort of 30,132 women who had cesarean delivery without labor in 19 academic centers over 4 years (1999-2002). RESULTS: There were 6,201 first (primary), 15,808 second, 6,324 third, 1,452 fourth, 258 fifth, and 89 sixth or more cesare

Staging laparotomy for endometrial carcinoma: assessment of peritoneal spread.

To evaluate surgical staging procedures in women with endometrial carcinoma, we examined the techniques used to assess the peritoneal cavity in 295 clinical stage I patients treated between 1985 and 1993. These patients were felt to be at increased risk for extrauterine disease because of significant myometrial invasion, high-grade (2 or 3), or variant histology (papillary serous, clear cell, or mixed). Patients had a mean of two intraperitoneal samples taken: 224 patients (76%) had at least an omental biopsy and peritoneal cytology. Additional peritoneal biopsy sites included pericolic gutter

Invasive cervical cancer complicating intrauterine pregnancy: treatment with radical hysterectomy.

OBJECTIVE: Radical hysterectomy and bilateral pelvic lymph node dissection have become the mainstay of treatment for early-stage cervical cancer because of both a high success rate and acceptable morbidity. However, those cervical lesions that occur concomitant with an intrauterine pregnancy have historically been treated with irradiation. We report the morbidity and results of radical hysterectomy and bilateral pelvic lymph node dissection for the treatment of early-stage cervical cancer complicating intrauterine pregnancy. METHODS: Between 1955-1991, 13 patients were treated with radical hy

Robot-assisted vesico-vaginal fistula repair: technical nuances.

INTRODUCTION:: Vesico-vaginal fistula (VVF) is a rare event in Western countries and are mainly consequent to iatrogenic injuries (1, 2). When conservative management fails, surgical repair is needed, although timing and surgical approach (open or minimally invasive (3)) are still controversial (4, 5). Herein we present a step-by-step description of robot-assisted vesico-vaginal fistula repair. MATERIAL AND METHODS:: From 2015 to 2018 six patients underwent robotic vesico-vaginal fistula repair. Pre-operative cystoscopy was performed to identify the fistulous tract. The ureters were stented.

Outcomes of Iatrogenic Genitourinary Injuries During Colorectal Surgery.

OBJECTIVE: To describe, categorize, and determine the outcomes of repairs of genitourinary (GU) injuries that occur during colorectal surgery. Presently, little is known regarding these injuries or the long-term outcomes of their repair. METHODS: We performed a retrospective review of patients undergoing colorectal surgery between 2003 and 2013 who experienced iatrogenic GU injuries requiring surgical repair. GU repair failures were defined as development of urine leak, urinary fistula, or anastomotic stricture requiring secondary GU intervention. Possible risk factors associated with repair

What is the role of reassessment laparoscopy in the management of gynecologic cancers in 1995?

One hundred fifty-four patients with a diagnosis of ovarian, primary peritoneal, or fallopian tube carcinoma underwent 181 reassessment procedures to detect persistent or recurrent disease between January 1, 1989 and December 31, 1994 at Cedars-Sinai Medical Center. One hundred four laparoscopic procedures were performed. Eleven of these procedures were converted to laparotomy due to severe adhesions. Therefore, a total of 88 reassessment laparotomies were performed during the study period. Fifty-seven of 93 laparoscopies and 69 of 88 laparotomies were done as second-look procedures. There was

Posterior urethral stricture repair following trauma and pelvic fracture.

Posterior urethral injuries typically arise in the context of a pelvic fracture. The correct and appropriate initial treatment of associated urethral rupture is critical to the proper healing of the injury. In this paper, we provide a comprehensive review of the literature with special emphasis on the various treatments available: open or endoscopic primary realignment, immediate or delayed urethroplasty after suprapubic cystostomy, and delayed optical urethrotomy.
