Management of severe spinal cord injury following hyperbaric exposure.

There is an increasing body of evidence that drainage of lumbar cerebrospinal fluid (CSF) improves functional neurological outcome after reperfusion injury to the spinal cord that occasionally follows aortic reconstructive surgery. This beneficial effect is considered owing to lowering of the CSF pressure thereby normalising spinal cord blood flow and reducing the 'secondary' cord injury caused by vascular congestion and cord swelling in the relatively confined spinal canal. Whilst lacking definitive proof, there are convincing randomised controlled trials (RCTs), cohort data and systematic re

Adaptations for marine habitat and the effect of Triassic and Jurassic predator pressure on development of decompression syndrome in ichthyosaurs.

Decompression syndrome (caisson disease or the "the bends") resulting in avascular necrosis has been documented in mosasaurs, sauropterygians, ichthyosaurs, and turtles from the Middle Jurassic to Late Cretaceous, but it was unclear that this disease occurred as far back as the Triassic. We have examined a large Triassic sample of ichthyosaurs and compared it with an equally large post-Triassic sample. Avascular necrosis was observed in over 15% of Late Middle Jurassic to Cretaceous ichthyosaurs with the highest occurrence (18%) in the Early Cretaceous, but was rare or absent in geologically o

The use of contrast echocardiography for the detection of cardiac shunts.

Recently, debate has erupted about the clinical significance of cardiovascular shunts. Several major health problems such as stroke and migraine have been associated with patent foramen ovale (PFO) with right-to-left shunt (RLS). The nature of the relationship between these syndromes and PFO is not clearly understood. Technical advances have led to more therapeutic options including device closure of PFO, hence prevention of such a PFO-related stroke has become feasible. Therefore, optimal diagnosis of PFO has become of greater clinical importance. Contrast echocardiography with non-transpulmo

Breath-Hold Diving - The Physiology of Diving Deep and Returning.

Breath-hold diving involves highly integrative physiology and extreme responses to both exercise and asphyxia during progressive elevations in hydrostatic pressure. With astonishing depth records exceeding 100 m, and up to 214 m on a single breath, the human capacity for deep breath-hold diving continues to refute expectations. The physiological challenges and responses occurring during a deep dive highlight the coordinated interplay of oxygen conservation, exercise economy, and hyperbaric management. In this review, the physiology of deep diving is portrayed as it occurs across the phases of

MRI screening of dysbaric osteonecrosis in hyperbaric-chamber inside attendants.

Inside attendants are medical staff who accompany patients during hyperbaric oxygen treatments. Dysbaric osteonecrosis (DON) is a well-known consequence of hyperbaric exposure. The aim of this study was to evaluate DON in inside attendants using magnetic resonance imaging (MRI). The bilateral shoulder, hip and knee joints of 12 inside attendants (four men, eight women; mean age 29 years; age range 22 - 36 years) were investigated. The mean +/- SD duration of employment as an inside attendant was 3.8 +/- 3.0 years (range 1 - 9 years) and the mean +/- SD number of hyperbaric exposures was 198 +/

First-ever Ischemic Stroke after a Flight in a Patient with Prior Poliomyelitis.

Survivors of poliomyelitis sometimes travel by air with mobility assistance. However, prolonged seating during long-haul flights may also possibly produce stroke events on polio-inflicted patients. A 48-year-old polio-inflicted male suffered a stroke after an extended flight. A two-dimensional echocardiography was normal without detected patent foramen ovale or dyskinetic segment. The venodynamic variables were all within normal limits. MR Imaging studies revealed acute cerebral infarction in the distribution of the right middle cerebral artery and posterior watershed area. Hematological exami

[Hyperbaric oxygen therapy (HBO): current standing].

For more than thirty years hyperbaric oxygen therapy (HBO) has been an important and ultimate therapeutic tool in special indications. Hyperbaric oxygen improves tissue oxygenation, stimulates important mechanisms in wound healing and exerts beneficial effects on other biochemical and cellular processes. The properties of hyperbaric oxygen have built the rationale for its use as therapy of choice in patients with severe carbon monoxide poisoning, decompression sickness and arterial gas embolism, and as adjunctive therapy for the treatment of osteoradionecrosis, necrotizing fasciitis and compro

Ear damage due to diving.

Diving injury may affect all parts of the ear during all types of diving. Divers are regularly exposed to noise and 120 dB(A) is measured during ventilation of a hyperbaric chamber. Ear canal squeeze, possibly with drum perforation, may give a transient conductive hearing loss. Middle ear squeeze, possibly with drum perforation, also gives a transient conductive loss. Inner ear barotrauma, possibly with perilymph fistula, most often results in transient vertigo and lasting sensorineural high tone loss, often resembling a noise-induced loss. Decompression sickness and gas embolism can also dama

Diving practices in technical divers' community and behaviour towards self-reported unusual symptoms.

INTRODUCTION: The use of gas mixtures containing helium for deep recreational diving is increasingly common, involving complex logistics and decision-making compromises. The characteristics and inherent risks of this practice remain poorly documented. This study aims to provide an epidemiological inventory of practices and diving-related incidents within the technical diving community. METHODS: An international online survey was disseminated on social networks targeting certified trimix divers. It collected demographic data, diving experience, and dive management practices, along with self-re

[The reactions of the cardiorespiratory system to exposure to a hyperbaric environment in the simulation of free-ascent conditions].

In 44 adult rabbits simulating 360 to 700-m ascent, bradyarrhythmia, auricular extrasystoles, partial antegrade block, dyspnea and transitory apnoea were found. Changes in the R-R histograms as well as autocorrelation function and spectrum of the cardiointervalogram showed overstrain and exhaustion of the cardiac rhythm control system at the end of decompression. Mechanisms of the cardiorespiratory system's responses to hyperbaric ambient conditions are discussed.

Persistent value of air-augmented radiograph in neonatal high gastrointestinal obstruction, despite more modern techniques.

Air is a safe and effective natural contrast agent in neonatal high gastrointestinal (GIT) obstruction. Successful early decompression often results in plain abdominal radiographs of low diagnostic yield. We present a series of neonates with high GIT obstruction in whom air-augmented abdominal radiographs (AAAR) were performed instead. Fourteen neonates presented with suspected high GIT obstruction. In 12 sick babies, obstruction was confirmed and the level of obstruction was determined. The other two neonates required additional positive contrast upper GIT studies. These confirmed small bowel

Massive cecal dilation: pseudoobstruction versus cecal volvulus?

Seven patients with acute and progressive abdominal distension secondary to massive cecal and right colon ileus are analyzed. Five had pseudoobstruction of the colon and two had cecal volvulus. Two of the patients with pseudoobstruction and one with cecal volvulus died from preexisting diseases. Pseudoobstruction of the colon is not a rare complication of elderly, sick, bedridden patients. Differential diagnoses include cecal and sigmoid volvulus and acute gastric dilation. Initial conservative therapy is warranted if no peritoneal signs are present. If the cecal diameter is more than 12 cm, c

Real-Time Monitoring of Tissue Oxygenation During Hyperbaric Oxygen Exposure Using In Vivo EPR Oximetry.

PURPOSE: Hyperbaric oxygen (HBO) therapy increases blood oxygen levels by exposing patients to 100% oxygen under elevated pressure and is used clinically for conditions such as decompression sickness, carbon monoxide poisoning, chronic wounds, and impaired tissue healing. However, real-time precise and quantitative measurement of tissue oxygen tension (pO2) under hyperbaric conditions remains technically very challenging. METHODS: We developed a specialized small-animal hyperbaric chamber compatible with in vivo electron paramagnetic resonance (EPR) oximetry using customized OxyTrack oxygen d

Musculoskeletal complaints among professional divers.

BACKGROUND: Previous publications have indicated a high risk of musculoskeletal complaints among professional divers. This study aims to investigate which factors influence professional divers' risk of musculoskeletal complaints. MATERIALS AND METHODS: Based on data gathered from a postal questionnaire sent to Norwegian inshore divers in 2011, the prevalence of musculoskeletal complaints, strain injuries and joint pain among divers with different certification levels, work-related tasks and decompression sickness (DCS) experiences were analysed. RESULTS: The risk of musculoskeletal complaint

Physiological impact of patent foramen ovale on pulmonary gas exchange, ventilatory acclimatization, and thermoregulation.

The foramen ovale, which is part of the normal fetal cardiopulmonary circulation, fails to close after birth in ∼35% of the population and represents a potential source of right-to-left shunt. Despite the prevalence of patent foramen ovale (PFO) in the general population, cardiopulmonary, exercise, thermoregulatory, and altitude physiologists may have underestimated the potential effect of this shunted blood flow on normal physiological processes in otherwise healthy humans. Because this shunted blood bypasses the respiratory system, it would not participate in either gas exchange or respirato

Bubbles or microparticles: which are the main initial cause of decompression illness?

INTRODUCTION: There are two leading schools of thought on the mechanism that causes decompression illness (DCI). The old school attributes DCI to decompression bubbles, while the new school, mostly led by S. Thom, attributes DCI mainly to microparticles (MP). MICROPARTICLES: Microparticles were found to be elevated at high pressures before decompression and, more so, after decompression. Decompression MP activate neutrophils and platelets, enhance inflammation, stimulate proinflammatory cytokines, and proto-coagulant activity. The predominant presence of endothelial MP is obvious. It was sugg

Risks from Breathing Elevated Oxygen.

INTRODUCTION: Effects of breathing gas with elevated oxygen partial pressure (Po₂) and/or elevated inspired oxygen fraction (FIo₂) at sea level or higher is discussed. High FIo₂ is associated with absorption problems in the lungs, middle ear, and paranasal sinuses, particularly if FIo₂ > 80% and small airways, Eustachian tubes, or sinus passages are blocked. Absorption becomes faster as cabin altitude increases. Pulmonary oxygen toxicity and direct oxidative injuries, related to elevated Po₂, are improbable in flight; no pulmonary oxygen toxicity has been found when Po₂ < 55 kPa [418 Torr; 100

Recurrent cutaneous decompression illness after PFO device implantation: a case report.

It is well known that patency of the foramen ovale (PFO) is a risk factor for decompression illness (DCI) in scuba divers, even if they adhere to the currently accepted and used decompression tables (termed "an undeserved hit"). The correction of the PFO through percutaneous closure after DCI is an ongoing discussion. We describe a case of a diver who redeveloped a cutaneous DCI due to a recurrent PFO after initial treatment with an occluder device four years earlier.

Vascular hyperpermeability in pulmonary decompression illness: 'the chokes'.

Decompression illness (DCI) develops during or after diving. Pulmonary decompression illness ('Chokes') is rarely seen because the affected individual usually dies in the water. We encountered a rare and interesting case. A 60-year-old man complained of leg pain after diving. Despite rapid transfer to a nearby hospital, advanced respiratory failure and shock had set in. He was then transferred to our hospital for hyperbaric oxygen therapy (HBOT). On account of his poor general condition, we initially treated him in the intensive care unit without HBOT, where he showed extreme hyperpermeability
