[Solitary rectal ulcer syndrome].

The solitary rectal ulcer syndrome is an infrequent entity consisting of a rectal lesion, caused by straining during defecation; it is characterized by specific histological changes. The condition is most frequent in adults between 30 and 50 years of age, but a few pediatric cases have been reported. Five patients (4 boys) are presented. All suffered from moderate rectal bleeding, straining, tenesmus and mucous discharge. One of them had clinical and endoscopic evidence of rectal prolapse. On defecography all children showed failure of pelvic floor relaxation during straining. Endoscopic evalu

Clinical value of transperineal ultrasound in evaluating the diagnostic grade of rectocele in Chinese women with obstructed defecation syndrome: An observational study.

We aimed to evaluate the effectiveness of transperineal ultrasound (TPUS) in diagnosing rectocele, rectal intussusception (RI), enterocele, perineal descent (PD), and cystocele in Chinese women with obstructed defecation syndrome (ODS), and to determine the grading of rectocele via TPUS. Between January 2019 and December 2021, 107 obstructed defecation syndrome patients, with a mean age of 49.76 years, received TPUS and defecation proctography (DEP). Both methods were used to diagnose anorectal angle, rectocele, RI, enterocele, and PD, while cystocele and uterine prolapse were diagnosed only t

X-ray video defaecography is superior to magnetic resonance defaecography in the imaging of defaecation disorders.

AIM: The aim of this work was to study the technical success and diagnostic capability of magnetic resonance defaecography (MRD) compared with video defaecography (VD). METHOD: Sixty four women with defaecation disorders underwent both MRD and x-ray VD over 1 year. The assessment by two radiologists in consensus was retrospective and blinded. The technical success of straining and evacuation was evaluated subjectively. The presence of enterocele, intussusception, rectocele and dyssynergic defaecation was analysed according to established criteria, with VD as the standard of reference. RESULT

Faecal incontinence in adults.

Faecal incontinence is a distressing condition, which adversely affects the quality of life of adults of all ages. It is often preventable and easily treatable once a diagnosis has been made. Nurses should provide support and information for patients and their families and ensure they understand the causes of faecal incontinence and the treatment regimens available.

Rome Constipation Symptoms Augmented by Painful Defecation Predicts Specific Subtypes of Refractory Constipation.

INTRODUCTION: Functional defecation disorders (FDD) are a prevalent etiology of refractory constipation. The diagnosis of FDD requires specific physiology testing, including anorectal manometry (ARM) and balloon expulsion test (BET). The aims of our study were to evaluate whether the complaint of painful defecation added to the Rome III symptoms questionnaire could help to differentiate subtypes of refractory chronic constipation. MATERIALS AND METHODS: One hundred and ninety-eight constipated patients (Rome III Criteria) who had failed a 30-day fiber/laxative trial were enrolled. Before ente

A radiomics nomogram utilizing T2-weighted MRI for accurate diagnosis of rectocele.

BACKGROUND: Rectocele (RC) is a common pelvic organ prolapse (POP) that can cause obstructed defecation and reduced quality of life. Magnetic resonance defecography (MRD) offers high-resolution, radiation-free visualization of pelvic floor anatomy but relies on time-consuming, observer-dependent manual measurements. Our research constructs a nomogram model incorporating intra-ROI and habitat radiomics features to improve MRD-based RC diagnosis. METHODS: We retrospectively analyzed 222 MRD patients (155 training, 67 testing). Clinical features were selected via univariate and multivariate logi

Influence of body posture on defecation: a prospective study of "The Thinker" position.

PURPOSE: We hypothesized that bending the upper body into what we have termed "The Thinker" position facilitates defecation. This study aimed to assess the influence of "The Thinker" position on defecation. METHODS: This is the prospective single-group study. Patients who could not evacuate the paste in normal sitting position on cinedefecography between January and June 2013 were enrolled in this study. Cinedefecography was first performed in the sitting position; if the patient was unable to evacuate the paste, images were obtained in "The Thinker" position. Patients who were able to evacua

Diagnostic-Therapeutic Care Pathway in Chronic Constipation: AIGO (Italian Association of Gastroenterologists and Gastrointestinal Endoscopists) Position Paper.

Chronic constipation (CC) is one of the most common disorders of gut-brain interaction, affecting more than 11% of adults in Western countries, with higher prevalence in women and in the elderly. Despite its significant impact on quality of life, most patients self-manage their symptoms, while only a minority seek medical attention from general practitioners (GPs) or specialists. Proper assessment not only often requires a multidimensional approach but also accurate diagnostic and therapeutic pathways that define the exact role of GPs and specialists. This paper describes a comprehensive Diagn

Pelvic floor dysfunction in patients with gestational diabetes mellitus.

In this editorial, we comment on an article by Wang et al. Recent literature shows an increase in research on pelvic organ prolapse (POP). Although the true incidence of POP remains uncertain, its impact on quality of life is substantial. Anatomical studies report high incidence rates, surpassing those observed in symptom-based surveys. Weakness of the endopelvic fascia is a primary anatomical risk factor for POP. Additionally, gestational diabetes mellitus (GDM) has emerged as a growing concern, as poor glycemic control increases complications for both mother and fetus. GDM and POP are interc

Outlet type constipation in adult patients treated with type A botulinum toxin: a cohort study.

PURPOSE: Chronic constipation is a common symptom. Constipation due to pelvic floor disorders remain a therapeutic challenge. Biofeedback therapy is considered as the first-choice treatment for pelvic floor disorders, whenever dedicated expertise is available. Type A botulinum toxin has been used to selectively weaken the external anal sphincter and puborectalis muscle in constipated patients. METHOD: Eighty-two patients with chronic outlet obstruction constipation were treated with 100 units type A botulinum toxin, injected into the puborectalis muscle and the external anal sphincter. From simplicity to precision: the balloon expulsion test in the diagnosis of anorectal disorders.

INTRODUCTION: Anorectal disorders, such as chronic constipation and fecal incontinence, significantly impact patients' quality of life. Accurate diagnosis is essential for effective treatment, and the balloon expulsion test (BET) has emerged as a simple yet valuable tool in evaluating anorectal function. Despite its simplicity, variations in technique and interpretation have led to ongoing debates regarding its diagnostic precision. To support this review, a PubMed search was conducted using the terms balloon, expulsion, test, evacuation, and disorders, focusing on the most relevant articles p

[Surgical treatment of severe constipation].

Severe constipation may be treated surgically provided precise evaluation of colon-transit-time and rectal evacuation can be performed. Colon-transit-time may be studied by straight X-ray of the abdomen after oral intake of small plastic markers, and rectal evacuation may be evaluated by defecography. In patients, where Hirschsprung's disease with a short aganglionic segment is suspected, ano-rectal manometry with evaluation of the recto-anal reflex must be performed. Furthermore, electromyography of the external anal sphincter and puborectal muscle during simulated defecation will be necessar

Anal sphincter morphology in patients with full-thickness rectal prolapse.

PURPOSE: The aim of this study was to assess the morphologic change of the anal canal in patients with rectal prolapse. METHODS: The endoanal ultrasound scans of 18 patients with rectal prolapse were compared with those of 23 asymptomatic controls. The thickness and area of the internal anal sphincter and submucosa were measured at three levels. RESULTS: Qualitatively, patients with rectal prolapse showed a characteristic elliptical morphology in the anal canal with anterior/posterior submucosal distortion accounting for most of the change. Quantitatively, internal anal sphincter (IAS) and s

Evaluating patients with anorectal incontinence.

Fecal incontinence is a devastating disability, and although this condition is widely accepted as a problem in the elderly, it is now becoming apparent that much younger age groups are also frequently affected. Thorough assessment of anorectal incontinence is very important to choosing the most appropriate treatment. Careful history-taking and physical examination can identify the cause of most cases of incontinence and are essential in every patient. Several incontinence scoring systems have been proposed to provide an objective measure of a subject's degree of fecal incontinence, but only on

Large rectal diverticulum in the setting of pelvic organ prolapse treated with robotic ventral mesh rectopexy: a case report.

BACKGROUND: Rectal diverticula are a very rare occurrence compared to diverticula of the colon. They are reported to account for only 0.08% of all diverticulosis. Diverticula of the rectum can be caused by congenital or acquired factors. The majority are asymptomatic, diagnosed incidentally, and require no treatment. The low incidence of rectal diverticulosis may be attributed to the unique anatomical structure and physiological environment of the rectum. However, complications can arise and may necessitate surgical or endoscopic treatment. CASE DESCRIPTION: We report the case of a 72-year-ol

Defecographic findings in patients with anal incontinence and constipation and their relation to rectal emptying.

PURPOSE: The aim of this study was to examine defecographic findings in patients with anal incontinence and constipation and to compare these findings with rectal emptying. METHODS: One hundred seventy-five preoperative defecographies documented on videotape in patients with either anal incontinence or constipation were retrospectively reviewed. The examinations were evaluated with respect to anatomic abnormalities of the rectum or anal canal. The results were compared with a semi-quantitative assessment of rectal emptying as it appeared on the video sequence after one minute of strain. RESU

Pelvic floor investigations for anal incontinence: Are they useful to predict outcomes from conservative treatment?

AIMS: There are several options for treating anal incontinence (AI), with limited success rate in long-term follow-up. Patients' selection is important to avoid unnecessary investigations and therapies. The aim of this review is to assess the utility of pelvic floor investigations to predict success from conservative treatment in AI. METHODS: Baseline demographics, severity scores, and pelvic floor investigations of 490 patients with AI symptoms were retrospectively reviewed. Patient-reported outcomes were used to define success of conservative treatment. RESULTS: Bivariate analysis showed t

Abdominal Sacrocolpopexy for Repair of Pelvic Organ Prolapse After Radical Cystectomy.

INTRODUCTION: Most case series describing surgical repair for pelvic organ prolapse (POP) after radical cystectomy (RC) focus on transvaginal repairs. We present our experience of POP after RC repaired by abdominal mesh sacrocolpopexy (ASC) with long-term follow-up. METHODS: Two women with previous RC with ileal conduit diversion underwent open ASC for repair of apical prolapse with concomitant enterocele. Prolapse severity was assessed using the POP quantification staging system, whereas pelvic imaging was performed with magnetic resonance imaging defecography. RESULTS: One patient had no P

Use of Sacral Nerve Stimulation for the Treatment of Overlapping Constipation and Fecal Incontinence.

BACKGROUND Fecal incontinence and constipation are common gastrointestinal complaints, but rarely occur concurrently. Management of these seemingly paradoxical processes is challenging, as treatment of one symptom may exacerbate the other. CASE REPORT A 51-year-old female with lifelong neurogenic bladder secondary to spina bifida occulta presented with progressive symptoms of daily urge fecal incontinence as well as hard bowel movements associated with straining and a sensation of incomplete evacuation requiring manual disimpaction. Pelvic floor testing showed poor ability to squeeze the anal
