Long-term results of implant-supported over-dentures retained by double crowns: a practice-based retrospective study after minimally 10 years follow-up.

BACKGROUND: Different concepts regarding the number of implants and attachment systems for the preparation of implant-supported over-dentures (IODs) have been discussed. Nonetheless, long-term results for double-crown-retained IODs with an observational period of more than 10 years are still rare in the literature. OBJECTIVE: The aim of this practice-based study was to retrospectively evaluate the long-term clinical outcome (success/survival rates, technical/biological complications) of IODs retained by double crowns. MATERIAL AND METHODS: In a private practice, 36 non-smoking edentulous pat

Clinical and microbiologic effects of chemical versus mechanical cleansing in professional supportive implant therapy.

The aim of the present study was to compare the cleansing properties of mechanical supportive care for dental implants with the use of an etching gel. Sixteen patients underwent a 5-month clinical trial with monthly recalls. These patients, wearing maxillary complete dentures and mandibular overdentures supported by a bar device on 4 implants, were treated in a split-mouth study design. Test and control therapy were randomly assigned to left and right sides of the mandible. At the test side, 35% phosphoric etching gel (pH 1) was applied in the peri-implant sulcus. After 1 minute, the sulcus wa

Mini-implant mandibular overdentures under a two-step immediate loading protocol: A 4-6-year retrospective study.

OBJECTIVES: This retrospective study to evaluate the treatment outcomes of mandibular mini-implant overdentures (MIODs) placed under a two-step immediate loading protocol. BACKGROUND: The mini-implant overdenture emphasises the advantages of simplicity using flapless surgery and immediate loading. However, some mini-implant have lowe initial stability. MATERIALS AND METHODS: A total of 30 participants who used mandibular MIODs and maxillary removable complete dentures (RCDs) over 4 years were included. Four one-piece mini-implants (<3 mm in diameter) were placed by a flapless surgical approa

Platform switching in two-implant bar-retained mandibular overdentures: 1-year results from a split-mouth randomized controlled clinical trial.

OBJECTIVES: The concept of platform switching is widely applied in current implant dentistry; however, the influence on peri-implant bone-level alterations (ΔIBL), especially in the field of implant overdentures (IODs), remains inconclusive. Therefore, the present study aimed to test the alternative hypothesis that there is an equivalent ΔIBLs at platform-switching and platform-matching implant abutments in 2-implant bar-retained IODs. MATERIALS AND METHODS: Two interforaminal implants were placed in 32 subjects, who were randomly assigned to either an immediate- or a 3-month post-placement l

[A consideration of determining the removal or retention of teeth in the patient with a few remained teeth].

The alveolar bone of edentulous ridge is resorbed after the teeth extraction and the subsequent denture insertion. For the periodontal structure, the stress transmitted along the longitudinal axis is favorable, but the horizontal stress is destructive. Under the overdenture application which the retained teeth have the reduced clinical crown, it is possible to prevent the alveolar bone resorption and to improve the involved teeth. However it is difficult for the inclined teeth to get the successful results. In a case, having a few remained teeth contained the embedded root stump and the overde

Scalp-Split Technique: A Concept of Bone Splitting and Expansion for Thin Alveolar Ridges to Place Dental Implants.

Bone expansion is when the existing bone at the implant site is split and separated to create a gap to widen it. This article introduces a novel concept of bone expansion for the ultrathin alveolar ridges to place dental implants with zero wastage of existing natural bone in long-term edentulous arches. Instead of using twist drills and piezo surgery kits, the bone is split with a surgical scalpel blade to prevent natural bone wastage. The split helps to expand the bone horizontally by using expanders to accommodate the smaller diameter implants. This technique was applied to 12 individuals to

Experimental and numerical investigations of fracture and fatigue behaviour of implant-supported bars with distal extension made of three different materials.

The aim of this study was to investigate experimentally the fatigue and stability of three bar materials with distal extension at the molar region and to numerically analyse the biomechanical properties of the bar materials connected to overdentures in a patient individual model. A milled bar was designed for the mandible on four implants in the canine and second premolar region. Three bar materials were investigated: titanium (Ti), cobalt chromium (CoCr), and polyetherketonketon (PEKK). Firstly, static and fatigue tests were performed based on EN ISO 14801 in a commercial permanent loading se
