Anatomical analysis of the posterior elbow joint capsule and small muscle bundle deep to the triceps brachii: a combined cadaveric and in vivo study.

BACKGROUND: Restricted elbow flexion sometimes persists even after stretching the triceps brachii. Although prior descriptions have noted that a small muscle bundle is located between the posterior elbow joint capsule and the triceps brachii, its anatomical relationship with surrounding muscles and dynamic changes during forearm rotation remain unclear. We hypothesized that the small muscle bundle separates from the triceps and contributes to forearm rotation. This study aimed to clarify the anatomical structure of the small muscle bundle and its dynamic changes during forearm rotation. METHO

Pitching Kinematics and Kinetics Related to the Elbow Joint Load in Young Female Baseball Players: A Comparison With Young Male Baseball Players.

BACKGROUND: Because the population of female baseball players has been increasing worldwide in recent years, it is important for female players to reduce the risk of elbow joint injuries, which occur frequently in baseball pitching. Although many biomechanical studies related to baseball pitching have clarified elbow joint loads, the participants in most of these studies were male baseball players, necessitating the expansion of investigations to female baseball players, particularly young female players. PURPOSE: The objective of the present study was to elucidate the pitching mechanics rela

Disseminated Tuberculosis of the Lungs, Brain, Pleurae, Mediastinal Lymphadenopathy, and Elbow Joint in an Immunocompetent Indian Female: A First-of-Its-Type Case.

Tuberculosis is usually seen in the lungs. However, the involvement of various extrapulmonary sites is due to the spread of the bacteria via blood, lymphatic, or direct inoculation. The present case is a rare presentation of tuberculosis in an Indian female who came with complaints of swelling in her right elbow joint, headache, and cough with expectoration. A diagnostic evaluation resulted in the isolation of Mycobacterium tuberculosis from the sputum samples and elbow joints, which was further supported by an exudative picture on the cerebrospinal fluid examination. The findings were support

Sonographic morphological and qualitative deficits in the elbow ulnar collateral ligament and ulnohumeral joint in throwing arms of asymptomatic collegiate baseball pitchers.

OBJECTIVE: The ulnar collateral ligament (UCL) supports the medial elbow against valgus torque and is commonly injured in baseball pitchers. Changes in UCL morphology and pathology occur with long-term pitching, with more severe findings at higher competition levels. We examined the bilateral differences and the relationship between UCL morphology, pathology, and ulnohumeral joint laxity in asymptomatic collegiate pitchers using ultrasound. MATERIALS AND METHODS: Division I college pitchers (n = 41) underwent ultrasound scans of their bilateral medial elbows, both at rest and in a valgus-stre

Sex differences in elbow and wrist joint loading during the cartwheel and round off with different hand positions performed by young gymnasts.

The aim of the study was to determine if sex differences exist in the key elbow and wrist joint injury risk factors during different cartwheel (CW) and round-off (RO) techniques performed by young male and female artistic gymnasts. Sixteen active young gymnasts (8 males and 8 females) performed 30 successful trials of CW and RO with three different hand positions (parallel (10), T-shape (10) and reverse (10)). Synchronised kinematic and kinetic data were collected for each trial. Two-way repeated measures ANOVA (3 × 2, technique × sex) and effect-sizes (ES) were used for statistical analysis.

External hinged fixation vs. internal joint stabilization for elbow instability: a systematic review and meta-analysis of functional outcomes and surgical complications.

BACKGROUND: Traumatic elbow instability can be managed with hinged external fixator (HEF) or internal joint stabilizer (IJS). While prior studies report device-related complications with both devices, a comprehensive analysis comparing range of motion (ROM), patient-reported outcome measures, and surgical complications is limited. This study aims to evaluate these outcomes to guide treatment decisions for complex elbow instability. METHODS: This systematic review and meta-analysis was conducted following Preferred Reporting Items for Systematic Reviews and Meta-Analyses guidelines. A comprehe

Fractures of the olecranon: an in vitro study of elbow joint stresses after tension-band wire fixation versus proximal fracture fragment excision.

BACKGROUND: Displaced fractures of the olecranon usually require operative treatment, by either open reduction with internal fixation (ORIF) or excision of the proximal fragment. However, the relative merits of these treatment options have not been fully delineated. One treatment outcome measure of joint function is residual intra-articular stress. The purpose of this study was to evaluate the effect of these two types of olecranon fracture treatment on humeroulnar joint stress. METHODS: Eight matched pairs of fresh frozen cadaveric upper extremities were thawed; stripped of skin, muscular, a

Medial elbow joint space gap after 100 pitches and early multi-sport participation in high school baseball players: a randomized controlled trial.

OBJECTIVES: To evaluate whether 100 consecutive pitches induce clinically meaningful increases in medial elbow joint space gap (MJS) in high school baseball players and to examine associations with player characteristics, including early multi-sport participation. METHODS: In a randomized controlled trial, 34 healthy male high school baseball players were allocated to a pitching group (n = 17) or a control group (n = 17). The pitching group threw 100 fastballs at maximal effort, whereas controls rested for an equivalent duration. MJS was measured by ultrasonography before and after pitching o

Drift-Free Joint Angle Calculation Using Inertial Measurement Units without Magnetometers: An Exploration of Sensor Fusion Methods for the Elbow and Wrist.

Joint angles of the lower extremities have been calculated using gyroscope and accelerometer measurements from inertial measurement units (IMUs) without sensor drift by leveraging kinematic constraints. However, it is unknown whether these methods are generalizable to the upper extremity due to differences in motion dynamics. Furthermore, the extent that post-processed sensor fusion algorithms can improve measurement accuracy relative to more commonly used Kalman filter-based methods remains unknown. This study calculated the elbow and wrist joint angles of 13 participants performing a simple

Management of a Rare Complex Triad Injury of the Elbow Joint: Radial Head Fracture, Triceps Tendon Avulsion, and Rupture of the Medial Collateral Ligament.

BACKGROUND: Isolated triceps tendon injuries are infrequent, and their combination with a medial ligament avulsion is even rarer. Eccentric force on the triceps tendon following a fall on the outstretched hand, combined with a valgus torque at the elbow joint, exposes the medial side to stretching forces, potentially disrupting the ulnar collateral ligament (UCL). Furthermore, compressive forces on the lateral side could result in injury to the radial head. Currently, there is no standardized surgical approach for managing such combined injuries. INDICATIONS: The complex triad injury presente

Supercharging Extra-Large Anterolateral Thigh Flaps for Single-Stage Resurfacing of Massive Burn Defects Over Upper Extremity Elbow Joints.

The resurfacing of soft tissue over extensor side of elbow joint after a full thickness burn injury is challenging due to greater amount of soft tissue needed. Local or regional flaps in the upper limb are not readily available and the use of free flap is the consensus. We want to present the first report of an extra-large anterolateral thigh (ALT) flap for a 1-stage coverage of an extensive elbow defect. The versatility of the ALT flap in design offers the surgeon the added option of supercharging the flap for increased survival when using a large flap for extensive wound coverage in the elbo

[Capsular arthroplasty of the elbow joint for the treatment of recurrent dislocation. Personal results in comparison with case reports in literature. Indication--methods--after care].

Four cases of recurrent dislocation of the elbow joint are reported. Instability of the postero-lateral ligaments were found in all patients, both clinically and during operation. In one case there was also instability of the medial ligament. Lateral instability is regarded as the primary lesion responsible for recurrent dislocation. Intraarticular pathological lesions such as osteochondral fractures were also diagnosed in our patients. All were treated by a capsuloligamentous repair as proposed by Osborne and Cotterill [8]. Based on follow-ups of 2 to nearly 5 years no recurrences were seen.

Total elbow and hip arthroplasties confer greater short-term risk of postoperative complications: a matched cohort analysis of the five major joint arthroplasties.

BACKGROUND: Total joint arthroplasty (TJA) is a common orthopedic procedure. OBJECTIVE: The primary objective of this retrospective cohort study was to investigate 30-day postoperative complication rates following five major types of TJA (shoulder, elbow, hip, knee, ankle). Independent risk factors for adverse outcomes were also assessed. METHODS: The American College of Surgeons (ACS) National Surgical Quality Improvement Program (NSQIP) database was filtered using current procedural terminology (CPT) codes to identify patients undergoing TJA from 2015 to 2020. Patients were divided into co

[Restoration of elbow joint flexion using pectoral muscle transfer in patients with arthrogryposis multiplex congenita. Part I: surgical method, rehabilitation and clinical results].

PURPOSE OF THE STUDY: In patients with type I arthrogryposis multiplex congenita, elbows are fixed in extension. The m. biceps brachii and m. brachialis are usually atrophic. The main objective of arm treatment is to restore flexion in the elbow that would enable the patient to reach the mouth with the hand as well as maintain active extension of the arm for hygienic purposes. This can be achieved by several techniques of muscle transfer. Of these, we selected and modified the method of Clark based on transfer of distal parts of the greater pectoral muscle. The aim of this study was to develop

Degenerative changes in the elbow joint after radial head excision for fracture: quantitative 3-dimensional analysis of bone density, stress distribution, and bone morphology.

HYPOTHESIS AND BACKGROUND: Some investigators speculate that excision may lead to elbow arthritis and associated problems; however, evidence supporting this theory is limited. It is hypothesized that radial head excision causes bone density changes as a result of asymmetrical stress distributions, consequently leading to osteophyte formation. In this study, we sought to quantitatively compare the 3-dimensional (3D) bone density and stress distributions between operative and nonoperative elbows in patients who underwent radial head excision. Furthermore, we aimed to quantify the bone morphologi
