Surgical treatment of Brucella endocarditis.

Between April 1987 and October 1992, six cases of Brucella endocarditis were operated on in the authors' hospital for valve replacement. They were five men and one woman with a mean(s.d.) age of 52(15) years (range 30-71 years). Three patients were in New York Heart Association (NYHA) class III and three in class IV. Two patients had previous history of rheumatic fever, one was a drug abuser, two had peripheral embolism and one constrictive pericarditis. Most were living in rural areas. Echocardiographic diagnoses were: severe aortic regurgitation in two patients, mixed disease in two and doub

[Neurological complications in infectious endocarditis].

Despite considerable progress in both diagnostic studies and therapeutic management serious complications of infectious endocarditis have become rather more common. Next to intracardiac complications arterial embolization to the central nervous system is the second most common life-threatening event, which might lead to infarction, hemorrhage, mycotic aneurysm and/or metastatic infection with a wide spectrum of neurological symptoms and an overall very poor prognosis. The most effective prevention of neurological complications is the early diagnosis of infectious endocarditis with isolation of

Prevention of prosthetic valve endocarditis.

Prosthetic valve endocarditis (PVE) has been traditionally divided into early (EPVE) and late (LPVE) forms, the division being made at 60 days after operation. Recent actuarial studies suggest that the risk of EPVE continues up to 12 months after operation. This new insight must increase the emphasis on perioperative prevention, including those measures taken at the time of operation, such as antibiotic prophylaxis, and particularly, the prevention of postoperative nosocomial bacteraemia which other recent studies suggest is a much more significant factor than previously appreciated. The appli

[Infective endocarditis associated with pregnancy].

PURPOSE: To report the intra-hospitalar and late follow-up of patients with infective endocarditis (IE) acquired in the pregnancy or puerperium. METHODS: Eleven patients, between 1984-1992 according to the beginning of the IE episode (fever and other signals) were studied. Patients were divided in two groups: IE of pregnancy (7 cases), and puerperal IE (4 cases). RESULTS: In the pregnancy IE group, mitral valve was affected in 6 (85%), and aortic valve in 1 (15%). During the course of the treatment, two patients had to be operated on, respectively, in the 24th and 28th week of the pregnancy.

Myths of dental-induced endocarditis.

Although the latest (1990) American Heart Association recommendations for the prevention of bacterial endocarditis are by far the simplest yet, many physicians and dentists still do not comply with them. One of the reasons for this low compliance is that many clinicians rely on "myths" of dental-induced endocarditis prevention. To educate clinicians on endocarditis and its prevention, the myths of dental-induced endocarditis prevention are analyzed. Myth 1: For the most part, physicians and dentists are aware of and comply with American Heart Association guidelines on antibiotic prophylaxis fo

Risk factors for infective endocarditis.

The incidence of endocarditis is increased in patients with cardiac lesions causing turbulent flow. A 400-fold increase is observed in patients with prosthetic valves and a prior history of endocarditis. Other high risk lesions include rheumatic valvular disease, cyanotic congenital heart disease, and degenerative valve lesions. The smaller increased incidence in patients with mitral valve prolapse is important because of the high prevalence of mitral valve prolapse in the population. In addition, intravenous drug use and nosocomial bacteremia have emerged as important factors among patients w

Acute endocarditis following pneumococcal meningitis.

Acute mitral valve endocarditis, presenting as a new murmur and haematuria, complicated pneumococcal meningitis in a 20-month-old child with a normal heart. Awareness of this rare complication of pneumococcaemia and its early diagnosis, using cross-sectional echocardiography, improves the clinical outcome of a condition associated with a high mortality.

[Surgery for bacterial endocarditis. When?].

In 1993, infectious endocarditis (IE) remains a common and serious condition. Surgery has become an essential feature of treatment in many cases. The choice and optimal timing depend on many factors: the tolerance of the underlying cardiac disease is an important feature, surgery being indicated not only in cases of necessity (refractory cardiac failure) but also as treatment of choice in cases of episodic decompensation even if temporary when related to valvular dysfunction. In these conditions, if the lesion is severe aortic incompetence, surgery can be programmed in two or three weeks after

[Bacterial endocarditis: current bacteriological data].

The bacteria causing infective endocarditis have not changed significantly despite the diversity of potential portals of entry. Streptococci (viridans) cause 35 to 45% of cases of endocarditis. Group D streptococci of gastrointestinal origin cause about 20% of cases of endocarditis: Streptococcus bovis is the most commonly isolated species followed by the enterococcus. The beta-haemolytic streptococci of Groups A, B, C and G are rarely isolated. Seventeen to thirty per cent of endocarditis is caused by staphylococci, above all Staphylococcus aureus. These infections are mainly observed in pati

[Surgical treatment of infective endocarditis].

In 43 patients with infective endocarditis aortic valve involvement was seen in 37, mitral valve in 4 and both in 2. 81.39% of the patients had the disease secondary to chronic cardiac valve disease. Follow-up ranged from 1.5 months to 10 years (mean 3.27 years). Early death occurred in 5 patients (11.65%) and late death in 4. According to clinical states and cardiac function, the patients were divided into two groups: acute and chronic cardiac dysfunction. In the former group, operation was done in 9 to 14 days after administration of antibiotics, and in the latter, 6 weeks to 3 months after

Thirty out of 287 patients (10.4%) admitted to hospital for infective endocarditis between December 1970 and January 1990 had neurological complications. Twenty-three patients had native valve infectious endocarditis and 7 had prosthetic valve endocarditis. The clinical features were characterized by the frequency of aortic valve involvement (23 out of 30) and other complications, especially cardiac failure (16 cases) and peripheral vascular manifestations (7 cases). The commonest organism was the staphylococcus (53% of identified organisms) but the number of negative blood cultures was high (

33 patients with infective endocarditis were treated from 1980 to 1989. 31 of them were operated on for primary endocarditis complicated by congenital heart disease (5 patients) and valvular heart disease (26 patients). Two patients (6.5%) died postoperatively and 2 patients with primary endocarditis complicated by aortic insufficiency died without operation. In 10 patients with endocarditis secondary to open-heart surgery, 6 were reoperated upon but 5 of them died; in the remaining 4 who were not reoperated on died. We consider that surgical intervention for endocarditis, either primary or po

Septic embolism complicating infective endocarditis.

Embolic phenomena in patients with infective endocarditis may complicate the placement of a cardiac valvular prosthesis. To evaluate the vascular consequences of these emboli, a 15-year review of 102 patients undergoing valve replacement for proven infective endocarditis was undertaken. Thirty-one patients with 36 episodes of septic embolization were identified. Ten of these were separate extremity occlusive events. All patients with extremity emboli were admitted with pain; four had limb-threatening emboli. All patients grew gram-positive bacteria from their blood except a single Candida albi

[Infective endocarditis: a changing disease].

Infective endocarditis is best characterized as a disease in evolution. The list of patients at risk, which formerly included almost exclusively patients with rheumatic heart disease, is being continuously modified and expanded. Nowadays, patients with prosthetic heart valves, users of illicit intravenous drugs, and patients with mitral valve prolapse rather than patients with rheumatic heart disease account for the majority of cases of infective endocarditis. Moreover, due to the widespread use of indwelling atrial catheters for parenteral nutrition as well as for intensive cytotoxic therapy,

Valve repair in acute endocarditis.

Forty patients were operated on in the early phase of active endocarditis between 1980 and 1988. Indications for operation were heart failure (30 patients), severe valvular regurgitation (4), uncontrolled sepsis (2), septic emboli (3), and other (1 patient). Time between onset of endocarditis symptoms and operation ranged from 12 to 45 days (mean, 30 days). The aortic valve was involved in 3 patients; the mitral valve, in 28; both valves, in 7; and the tricuspid valve, in 2. There was no previous underlying valve pathology in 40%. Lesions found were cusp perforation (17 patients), annular absc

[Surgical therapy of bacterial endocarditis].

On the basis of an own clinical material of 26 patients and in comparison to the literature indications and results of the operative therapy in bacterial endocarditis were discussed. The progressive, conservatively not controllable cardiac insufficiency is the most frequent indication for operation. Without valve replacement it causes death in 60-90% of the patients. In contrast to this there is a postoperative early lethality of 10-30%. Among our own patients the early mortality was 19%. Ill patients operated on were in an advanced stage of the disease, seven patients had to be operated on un

Endocarditis due to Cardiobacterium hominis.

A case of subacute bacterial endocarditis was caused by Cardiobacterium hominis in a 55-year-old man who had a history of rheumatic fever. A survey of the literature showed few reports dealing with the isolation of these organisms from human sources. Microbiologically, the chief distinguishing features of C. hominis are its characteristic colonial morphotype, a positive reaction, and its production of indole.
