The most common cause for catching or snapping in the finger is stenosing tenosynovitis, that is, trigger finger. Although less common, snapping can also occur as a result of extensor mechanism injury. Among these injuries, sagittal band rupture is most common and leads to snapping at the metacarpophalangeal joint. Snapping at the proximal interphalangeal (PIP) joint is rare with only 4 reported cases; reported mechanisms of PIP joint snapping include retinacular ligament injury or tendon impingement. We present a unique case of painful finger snapping at the PIP joint as a result of longitudi

OBJECTIVE: Nonsurgical treatment of Dupuytren's disease using collagenase Clostridium histolyticum (CCH).
INDICATIONS: Metacarpophalangeal (MP) joint (20-100°) and proximal interphalangeal (PIP) joint (20-80°) contractures.
CONTRAINDICATIONS: Pregnancy, previous hypersensitivity to collagenase or excipients, anticoagulant use within 7 days prior to treatment.
INJECTION TECHNIQUE: CCH injected directly into the Dupuytren's cord weakening the contracted cord. After injection, the patient returns the following day to allow CCH to lyse the collagen within the cord. An extension force is then ap

BACKGROUND: Australian Sign Language (Auslan) relies heavily on precise finger movements, alongside wrist, forearm, and elbow coordination. Prior studies have examined upper limb biomechanics in Auslan communication but have not quantified the specific range of motion (ROM) required of the fingers.
PURPOSE: To determine the finger ROM necessary for effective Auslan communication, providing evidence to guide rehabilitation strategies for patients reliant on Auslan.
STUDY DESIGN: Biomechanical analysis METHODS: Two right-handed native Auslan communicators were recruited for biomechanical analy

Dupuytren's disease is a common condition in hand surgery. The fifth finger is frequently affected, pre- senting the highest recurrence rate after surgical treatment. The ulnar lateral-digital flap is used when a skin defect prevents direct closure after fasciectomy of the fifth finger at the level of the MP joint. Our case series comprises of 11 patients undergoing this procedure. Their mean preoperative extension deficit was 52° at the MP joint and 43° at the PIP joint. Full extension of the MP joint and a mean of 8° extension deficit at the PIP joint was achieved with surgery. All patients

We measured the alignment of the nails in the hands of 100 normal adults with the interphalangeal joints extended and the metacarpophalangeal (MCP) joints at 0 degrees and 90 degrees. All fingers were naturally supinated (i.e. rotated towards the thumb), the index and little being the most supinated. When examining individual hands in MCP joint extension, only 17% of hands had all nails parallel. With the MCP joints flexed, this improved to 56%. If the little finger was excluded, this improved to 78%. When comparing matching fingers from the two hands, 76% of little, 83% of ring, 77% of middle

Amputation of the thumb is a severe handicap. In an emergency situation, thumb amputation must be treated by means of reimplantation when possible. If reimplantation cannot be performed or fails, several methods of thumb reconstruction can be used according to various factors. These include the number of surviving fingers and the level of the thumb amputation. Pollicization is the first choice for amputations proximal to the metacarpophalangeal joint when four and even three fingers are present. It is the easiest and safest operation that supplies the best results both from the motor and senso

PURPOSE: To report a congenital anomaly of the middle finger.
METHOD: Nine patients (16 digits) are reported with congenital flexion deformity of the metacarpophalangeal (MCP) joint of the middle finger. Three patients (4 digits) had isolated deformities to the middle finger and in 6 the deformity was part of congenital ulnar drift (CUD) of the hand. Three patients had Freeman-Sheldon syndrome, 2 had nonsyndromic CUD, and 1 had arthrogryposis multiplex congenita. In CUD patients the middle finger had substantially greater flexion deformity of the MCP joint in comparison with other digits. Sev

Eleven cytogenetically documented patients with the fragile X syndrome were evaluated for hyperextensibility of the finger joints and the presence of other manifestations of connective tissue dysfunction. All of the patients had hyperextensibility of several finger joints and many had features such as flat feet, highly arched palate and the ability to voluntarily dislocate finger joints. These traits, particularly hyperextensibility of the joints, are a useful aid in the clinical diagnosis of the fragile X syndrome.

This paper presents the design and implementation of an Anti-Finger Roll-Up (AFRU) mechanism for a cable-driven grasp assistance exoskeleton. Finger rollup describes the premature flexion of the distal and proximal interphalangeal joints relative to the metacarpophalangeal joint as a result of a cable pulling directly on the fingertip. This leads to unstable pinching grasps or grasps that place the target object outside of the grasp envelope. This device provides a solution to finger roll-up by emulating an increase in joint stiffness in the distal interphalangeal and proximal interphalangeal j

Finger fillet flaps have been used to treat a variety of complex hand deformities providing stable soft tissue coverage and preventing pathologic contractures. Fillet flaps have not been reported in the coverage of segmental extensor tendon deficit in an adjacent digit. A 20-year-old man involved in a motor vehicle crash sustaining a 30% total body surface area burn, primarily to left arm and bilateral lower extremities. In particular, his left index finger extensor mechanism was disrupted with exposure of the proximal interphalangeal (PIP) joint. In addition, the middle finger had a segmental

A 49-year-old woman with rheumatoid arthritis who underwent replacement arthroplasty of second to fifth left metacarpophalangeal joints with silastic implant seven years ago presented with a complaint of mild pain and discomfort on the replaced joint of index finger. Ulnar deviation had relapsed, with severe swan neck deformities. Computed tomography examination demonstrated that the tip of the stem of the silicon implant penetrated the second metacarpal. We confirmed that finding surgically, and we performed a revision surgery successfully with autogenous bone grafting from distal radius. As

We introduced a technique with a volar transfer of the lateral band using the transverse retinacular ligament for swan-neck deformity caused by volar plate injury of the PIP joint. A 61-year-old woman injured her 5th finger and was diagnosed with a volar plate injury of the PIP joint. She presented with snapping of the finger together with the appearance of a swan-neck deformity, and surgery was performed. Dorsally located lateral bands were transferred towards the volar aspect of the finger, and their position was maintained using the transverse retinacular ligament. Improvements in the snapp

PURPOSE: Claw finger deformity occurs during attempted finger extension in patients whose intrinsic finger muscles are weakened or paralyzed by neural impairments. The deformity is generally not acutely present after intrinsic muscle palsy. The delayed onset, with severity progressing over time, suggests soft tissue changes that affect the passive biomechanics of the hand exacerbate and advance the deformity. Clinical interventions may be more effective if such secondary biomechanical changes are effectively addressed. Using a computational model, we simulated these altered soft tissue biomech

BACKGROUND: Recurrent severe Dupuytren contracture of the small finger's proximal interphalangeal (PIP) joint is a difficult problem. Further surgery carries high risk of complications and poor outcome. Patients are often offered finger amputation. We have devised a novel surgical procedure consisting of middle phalanx monoblock resection and ligament reconstruction to create a new functioning interphalangeal joint.
METHODS: Two patients requesting small-finger amputation because of severe PIP joint contracture after multiple treatments for Dupuytren contracture were offered and accepted this

CASE: Charcot arthropathy (CA) is a progressive degenerative joint disease typically affecting lower extremity weight-bearing joints, with only a few cases reported in the fingers. We present 2 cases of interphalangeal joint CA: the long finger distal interphalangeal joint in a 73-year-old man with severe carpal tunnel syndrome and the ring finger proximal interphalangeal joint of a 71-year-old woman with diabetic neuropathy.
CONCLUSION: Two cases of CA of the digits were treated with splinting with resolution of symptoms and no wound complications.

Rheumatoid arthritis (RA) causes persistent synovitis and arthritis, resulting in joint deformity and destruction throughout the body. As RA medications have evolved over the past 30 years, the surgical indications and techniques for RA joint deformities have changed. The aim of this review article is to summarize the recent trend of surgery for rheumatoid hand/finger deformities in previous reports and to present our recent surgical methods and outcomes for these deformities. A typical hand and finger deformity caused by RA is ulnar deviation, which is mainly caused by joint laxity and disloc

X-rays of 20 finger joints, 10 foot joints and 2 wrists of 50 female patients with seropositive rheumatoid arthritis of 10-15 years' duration from 1962, 1972, 1982 and 1992 were evaluated blindly for erosions. A highly significant decline was observed in erosions in finger joints. Erosions had also declined in wrists and in foot joints, though less than in fingers. This may indicate improvement in the long-term outcome of rheumatoid arthritis.

PURPOSE: To assess the longitudinal radiographic osseous changes of the wrist and hand other than interphalangeal joints in rheumatoid disease.
METHODS: Serial wrist and hand x-rays in 96 patients with long-standing rheumatoid disease were reviewed. The average number of years between initial and most recent x-rays was 15.1. The Larsen scoring system was used to assess the degree and severity of joint involvement. We identified patterns of involvement in the wrist, thumb, and finger metacarpophalangeal (MCP) joints.
RESULTS: The radioscaphoid and radiolunate joints had the earliest and most
