Randomized, controlled, 48-week study of switching stavudine and/or protease inhibitors to combivir/abacavir to prevent or reverse lipoatrophy in HIV-infected patients.

OBJECTIVE: HIV-1 protease inhibitors (versus no protease inhibitors) and stavudine (versus zidovudine) are independently associated with a higher risk of lipoatrophy in HIV-infected patients. We sought to determine whether the revision of stavudine and/or protease inhibitor-containing regimens to combivir/abacavir would result in prevention and/or reversibility of lipoatrophy in HIV-1-infected patients. DESIGN: The investigation was a prospective, randomized, controlled, open-label study. SUBJECTS: The subjects included 37 HIV-1-infected individuals with stable undetectable HIV-1 loads who w

Diabetes Technologies in Ultra-Endurance Type 1 Diabetes: Qualitative Study.

BACKGROUND: Diabetes technologies-including continuous glucose monitoring (CGM), insulin pumps, and hybrid closed-loop systems-have profoundly transformed self-management in type 1 diabetes (T1D). While these technologies offer improved glycemic control and safety, their use in ultraendurance sports introduces specific cognitive, material, and organizational challenges that remain underexplored in digital health research. OBJECTIVE: This study aimed to explore how adults living with T1D experience and use diabetes technologies in ultraendurance sports, with particular attention to tensions be

Dapagliflozin-induced integrated improvements in left ventricular diastole, endothelial function, and arterial load: a randomized clinical trial.

BACKGROUND: Dapagliflozin has been shown in preclinical and clinical settings to improve arterial function and left ventricular (LV) diastolic performance, yet the interrelationship between these effects has not been established. OBJECTIVE: To determine whether improvements in endothelial function accompany-and relate to-early changes in LV diastolic function after short-term dapagliflozin in type 2 diabetes (T2D). METHODS: We conducted a prespecified secondary analysis of a prospective, open-label, active-controlled trial of patients with T2D on background metformin that were randomized to

Effects of multi-strain probiotics supplementation on body adiposity and metabolic profile in individuals with hypertension and overweight following an energy-restricted diet: A randomized clinical trial.

BACKGROUND & AIMS: Recent evidence suggests that gut microbiota has a potential role in the pathophysiology of obesity and other cardiovascular disease (CVD) risk factors, including hypertension, dyslipidemia, and type 2 diabetes. However, clinical trials evaluating the effects of probiotics supplementation on these outcomes have found inconsistent results, probably due to the wide heterogeneity in trial designs. In addition, there is a lack of studies investigating whether probiotics can enhance the beneficial effects of caloric restriction in individuals with increased risk of CVD as individ

High glycated albumin is an independent predictor of low response to clopidogrel in ACS patients: a cross-sectional study.

BACKGROUND: Glycated albumin (GA) is a marker of short-term glycemic control and is strongly associated with the occurrence of diabetes. Previous studies have shown an association between GA and the effect of clopidogrel therapy on ischemic stroke. However, limited information is available regarding this relationship in acute coronary syndrome (ACS) patients. In this study, we evaluated the effect of GA on platelet P2Y12 inhibition by clopidogrel in patients with ACS. METHODS: Consecutive Chinese patients with ACS who received loading or maintenance doses of clopidogrel in addition to aspirin

Gestational age and models for predicting gestational diabetes mellitus.

INTRODUCTION: Gestational diabetes mellitus (GDM) is generally identified by measuring elevated maternal glycemic responses to an oral glucose load in late pregnancy (> 0.6 term). However, our preliminary study suggests that GDM could be identified with a high predictive accuracy (96%) in the first trimester (< 0.35 term) by characteristic changes in the metabolite profile of maternal urine. (Koos and Gornbein, American Journal of Obstetrics and Gynecology 224:215.e1-215.e7, 2021). The gestational decrease in insulin sensitivity and the accompanying perturbations of the maternal metabolome sug

Early prediction of late-pregnancy hypertriglyceridemia in women with gestational diabetes: development and internal validation of a clinical risk model.

BACKGROUND: Women with gestational diabetes mellitus (GDM) are at high risk of developing hypertriglyceridemia (HTG) in late pregnancy, a condition associated with adverse maternal and neonatal outcomes. Early identification of high-risk individuals at GDM diagnosis is crucial for timely intervention. OBJECTIVE: To develop and internally validate a clinical risk prediction model for estimating the individualized risk of late-pregnancy HTG in women at the time of GDM diagnosis. METHODS: This single-center retrospective cohort study included 587 women with GDM. Predictor variables, available a

High-fructose diet-induced gut dysbiosis as a mechanistic driver of chronic inflammation and accelerated biological aging: a comprehensive review.

High-fructose diet (HFD) has emerged as a critical dietary risk factor for metabolic syndrome, non-alcoholic fatty liver disease (NAFLD), and cardiovascular disease. This review delineates the molecular mechanisms underlying the pathological cascade linking high-fructose intake, gut dysbiosis, chronic inflammation, and accelerated aging. At the metabolic level, excess fructose is rapidly phosphorylated by ketohexokinase (KHK), depleting intracellular ATP while driving overproduction of uric acid and reactive oxygen species (ROS). These events activate the NLRP3 inflammasome and NF-κB signaling

Distachionate and Its Nanocomposites Modulate Oxidative Stress, Inflammation, and Diabetes-Related Genes in High-Fructose/Streptozotocin Model.

Medicinal plants, known for their bioactive compounds, have long been utilized for the treatment of various diseases, including diabetes. A novel compound, distachionate (DST), was isolated as a yellowish powder from Breynia disticha, referred to as snowbush. It is known to possess pronounced antioxidant and anti-inflammatory activities mediated through the modulation of COX-2 and inflammatory cytokines. Based on its diverse medicinal uses of the parent plant and the lack of data for its effectiveness in diabetes, this study has been aimed at evaluating the antidiabetic efficacy of DST and DST

Biochemical and Metabolical Pathways Associated with Microbiota-Derived Butyrate in Colorectal Cancer and Omega-3 Fatty Acids Implications: A Narrative Review.

Knowledge regarding the influence of the microbial community in cancer promotion or protection has expanded even more through the study of bacterial metabolic products and how they can modulate cancer risk, which represents an extremely challenging approach for the relationship between intestinal microbiota and colorectal cancer (CRC). This review discusses research progress on the effect of bacterial dysbiosis from a metabolic point of view, particularly on the biochemical mechanisms of butyrate, one of the main short chain fatty acids (SCFAs) with anti-inflammatory and anti-tumor properties

Risk Factors and Prevention of Cancer and CVDs: A Chicken and Egg Situation.

Cardiovascular diseases and cancer are the two primary causes of mortality worldwide. Although traditionally regarded as distinct pathologies, they share numerous pathophysiological mechanisms and risk factors, including chronic inflammation, insulin resistance, obesity, and metabolic dysregulation. Notably, several cancers have been identified as closely linked to cardiovascular diseases, including lung, breast, prostate, and colorectal cancers, as well as hematological malignancies, such as leukemia and lymphoma. Additionally, renal and pancreatic cancers exhibit a significant association wi
