Facilitating transition of young people with long-term health conditions from children's to adults' healthcare services - implications of a 5-year research programme.

BACKGROUND: During transition from children's to adults' healthcare, young adults with long-term conditions may show delays in psychosocial development compared to their peers without long-term conditions, and deterioration of their conditions' medical control. METHODS: This paper integrates the findings, already published in 10 separate papers, of a 5-year transition research programme. IMPLICATIONS: There is an important role for funders (commissioners) of adults' services to fund transitional healthcare, in addition to funders of children's services who currently take responsibility. It is

Outcomes analysis of Internet-based CME initiatives for diagnosis and treatment of fibromyalgia patients: transition from education to physician behavior to patient health.

A well designed outcomes research study was performed in which 20 primary care physicians were selected to participate. Each physician had more than 30 fibromyalgia patients in their practice. The study design consisted of four phases. In phase one, physicians undertook a self-assessment of their practice. Phase two of the study involved diagnosis and treatment of a virtual case vignette. The third phase consisted of analysis of the data from phase two and providing feedback from an expert rheumatologist, and the fourth phase was to complete patient report forms for five patients in their prac

The sustainability of public health programs following donor transition: A comparative case study of HIV services and maternal and newborn care in Uganda.

UNLABELLED: Although there is emerging evidence on the impact of donor transition on health programs, there is little research comparing sustainability outcomes across health programs. We sought to compare drivers of health program sustainability concerning maternal and newborn care in Western Uganda after the end of the 'Saving Mothers Giving Life'(SMGL) project and HIV services in eastern Uganda following loss of PEPFAR support. METHODS: We report qualitative findings from a larger mixed-methods study. In-depth interviews were held with Ministry of Health officials (n = 11), district health

Electronic Health Literacy, Psychological Distress, and Quality of Life in Urological Cancer Patients: A Longitudinal Study During Transition from Inpatient to Outpatient Care.

Urological cancers are associated with reduced quality of life and high psychological burden, yet affected patients receive less psychosocial support than other cancer groups. Electronic health literacy (eHL) may facilitate independent access to resources, but its role for psychological outcomes and quality of life in this group is unclear. This study examined associations between eHL, psychological symptoms, and quality of life during transition from inpatient to outpatient care. A prospective, single-centre observational study was conducted. Eligible inpatients (urological cancer, Distress T

Health-Related Quality of Life in Adolescents and Young Adults with Congenital Heart Disease and Its Predictors, Including Subjective Transition Readiness to Adulthood.

Although international studies have examined the health-related quality of life (HRQoL) of adolescents and young adults (AYAs) with congenital heart disease (CHD), those focusing specifically on predictors including transition readiness to adulthood remain limited, particularly in Japan. This study aimed to evaluate HRQoL among AYAs with CHD and its predictors. This cross-sectional survey included 86 participants with CHD from 4 outpatient pediatric cardiology clinics and a parents' peer support group in Japan. The MOS Short-Form 36-Item Health Survey (SF-36) was conducted, and information was

An exploratory study of adolescents and emerging adults with spina bifida knowledge of their individual education program: Implications for health care transition planning.

PURPOSE: The purpose of this study was to explore adolescents and emerging adults (AEA) with spina bifida (SB) understanding of their Individualized Education Program (IEP) and to identify factors related to their knowledge about their own IEP. METHODS: Data were collected from January 2015 to July 2016 from 79 adolescents with SB. A ten-item questionnaire including demographics and questions specifically addressing IEPs was used. Qualitative analysis followed an iterative, emergent approach. Two experienced coders independently read and coded each of the three open-ended questions. It was de

Evolution of the "fourth stage" of epidemiologic transition in people aged 80 years and over: population-based cohort study using electronic health records.

BACKGROUND: In the "fourth stage" of epidemiological transition, the distribution of non-communicable diseases is expected to shift to more advanced ages, but age-specific changes beyond 80 years of age have not been reported. METHODS: This study aimed to evaluate demographic and health transitions in a population aged 80 years and over in the United Kingdom from 1990 to 2014, using primary care electronic health records. Epidemiological analysis of chronic morbidities and age-related impairments included a cohort of 299,495 participants, with stratified sampling by five-year age group up to

Associations Between Health Screening Rates and Visits to Their Primary Care Provider in Young Adults Transitioning From Pediatrics to Adult Medicine.

INTRODUCTION: This study examines associations between a young adult's visit to a designated primary care physician (PCP) in the 18 months after transitioning from pediatrics and their completion of preventive healthcare screenings. The period from 18 to 25 years is marked by transition and continued brain development, making it an ideal period for impactful intervention on healthy behaviors and outcomes. METHODS: Completion of screening for alcohol use, mental health issues, exercise habits, and tobacco use was measured in members upon turning 18 during 2020-2021. These members had transitio

Evaluation of a Model of Transitional Care After Preterm Birth on Parents' Mental Health and Self-Efficacy: A Randomized Controlled Pilot Trial.

BACKGROUND/OBJECTIVES: Parents of premature infants experience depression, anxiety, post-traumatic stress disorder, and increased stress, which can negatively impact parent-infant relationships and infant development. To reduce negative consequences and optimally support families, we developed the Transition to Home model (TtH). In this randomized controlled pilot trial (RCT), the feasibility of performing an experimental study to analyse the effects of TtH on parental mental health over time was evaluated. METHODS: The following domains were assessed: recruitment, follow-up and study burden,

E-cigarette Transitions Among US Youth and Adults: Results from the Population Assessment of Tobacco and Health Study (2013-2018).

Currently, the debate surrounding the regulation of e-cigarettes focuses mainly on the size of e-cigarettes' potentially beneficial effects (i.e., adult cessation) versus their unwarranted effects (i.e., initiation among tobacco-naïve adolescents). Therefore, we investigated the relative scale of e-cigarette use transitions in the United States. We reported cross-sectional weighted prevalence estimates of past-month e-cigarette use by ever cigarette use from Waves 1-4 of the Population Assessment of Tobacco and Health study (2013-2018) among youth (12-17 years) and adults (≥ 18 years). Porotic hyperostosis as a marker of health and nutritional conditions during childhood: studies at the transition between Imperial Rome and the Early Middle Ages.

Porotic hyperostosis, characterized by small and localized perforations on the surface of cranial bones, is considered a good indicator for assessing the health and nutritional status of past human populations. The most widely accepted theory at present indicates that anemias, either acquired or of genetic origin, are responsible for the bony lesions described as porotic hyperostosis. In this paper, the prevalence of these lesions in two skeletal samples from Latium (central Italy) was used to evaluate health and life conditions in Italy after the collapse of the Roman Empire. One sample belon

Estimating Chronic Kidney Disease Stage Transitions from Irregular Electronic Health Record Data Using an Expectation-Maximization Framework.

OBJECTIVE: To estimate chronic kidney disease (CKD) stage transition probabilities in patients with small renal masses (SRMs) using irregularly observed electronic health record (EHR) data, addressing challenges of interval censoring and irregular measurement intervals in real-world clinical practice. DATA SOURCES: We used EHR data from the institutional Small Renal Mass registry (2006-January 2026), capturing outpatient renal function data prior to any definitive treatment for SRM. CKD stages were defined using estimated glomerular filtration rate (eGFR) thresholds based on KDIGO guidelines.

Promoting Health Equity Through Tax-Funded Zero-Fare Public Transit: A Survey of Voters From Kansas City, MO.

Public transportation has positive impacts for both individual and community health, but funding and scalability are political challenges. Understanding civic processes, such as voting, is key to advancing health equity through large-scale policy change. The purpose of this study is to understand voter support for tax-funded public transportation. This study employed a cross-sectional survey of randomly sampled voters in Kansas City, Missouri, collected from October 2023 to January 2024. Of the 2,577 respondents, 79% participated in the election of interest. Two-sample proportion tests for pai

Multimorbidity patterns and phenotype transitions in patients with clinician-coded long COVID: a multicenter US electronic health record cohort study.

BACKGROUND: Long COVID is clinically heterogeneous, but longitudinal changes in documented chronic disease burden and transitions in multimorbidity phenotypes after infection are not well characterized in routine care. METHODS: We considered 425,614 patients with clinician-coded long COVID (ICD-10-CM U09.9) and a definable COVID-19 index date between October 1, 2021 and September 16, 2024 using deidentified electronic health record data from Epic Cosmos, a multicenter US network. Pre-index and post-index windows were defined as days - 365 to - 1 and days 91 to 455 relative to infection, respe

Transition to Retirement Impact on Physical Activity: Longitudinal Temporal Trajectories of Frequency and Intensity From the Survey of Health, Aging, and Retirement in Europe.

BACKGROUND: Major life course transitions such as retirement may substantially affect lifestyles. Despite the well-established benefits of physical activity (PA) in older ages, evidence on its relationship with retirement remains inconsistent. This study examines the impact of retirement on PA frequency and intensity in the Survey of Health, Ageing, and Retirement in Europe. METHODS: Longitudinal data from Survey of Health, Ageing, and Retirement in Europe waves 1 to 8 (2004-2020) were analyzed. The exposure was the time relative to retirement, modeled across preretirement and postretirement

Financing the transition: a roadmap for sustaining disease control in low- and middle-income countries after the 2025 contraction in development assistance for health.

In 2025, development assistance for health fell by about 21% in a single year, driven largely by a 67% reduction in United States financing, disrupting HIV, tuberculosis, malaria, immunization and maternal health programs across low- and middle-income countries. Modelling indicates millions of avoidable deaths by 2030 if the resulting gap is not filled, yet most recipient governments have not reprioritized budgets or raised domestic revenue in response. This brief argues that the contraction is structural rather than cyclical and sets out a sequenced transition-financing roadmap. It proposes p
