Multiagent Large Language Model Framework for Psychotherapy Fidelity Assessment in Motivational Interviewing and Cognitive Behavioral Therapy Training: Cross-Sectional, Simulation-Based Evaluation Study.

BACKGROUND: Psychotherapy training is difficult to scale because manual rating of motivational interviewing (MI) and cognitive behavioral therapy (CBT) sessions is time-intensive, requires trained raters, and is subject to rater variability. Large language models (LLMs) may support simulation-based training and rubric-guided scoring, but early-stage evidence is needed before such systems can be applied to real learners. OBJECTIVE: This study aimed to conduct a simulation-based evaluation of a multiagent LLM framework for generating and scoring stylized MI and CBT training encounters. METHODS

Deep Learning Model for Natural Language to Assess Effectiveness of Patients With Non-Muscle Invasive Bladder Cancer Receiving Intravesical Bacillus Calmette-Guérin Therapy.

PURPOSE: Collecting information on clinical outcomes (recurrence/progression) from complex treatment courses in non-muscle invasive bladder cancer (NMIBC) is challenging and time-consuming. We developed a deep learning natural language processing model to assess outcomes in patients with NMIBC using vast data from electronic health records (EHRs). METHODS: This retrospective study analyzed data from Japanese adults with NMIBC who started Bacillus Calmette-Guérin (BCG) induction therapy between April 2016 and June 2022. A Bidirectional Encoder Representations from Transformers (BERT) model was

Outcomes of a first point of contact speech language therapy clinic for patients requiring vocal cord check pre and post thyroid/parathyroid surgery.

INTRODUCTION: Speech Language Therapy First Point of Contact Clinic (SLT-FPOCC) models can assist assessment of low-risk patient populations referred to ear, nose and throat (ENT) services. To further improve ENT waitlist management and compliance with best-practice care, consideration of other low-risk populations that could be safely managed through this service model is needed. The aims of this paper are to evaluate the clinical and service outcomes of completing vocal cord check (VCC) assessments for patients' pre and post thyroid/parathyroid surgery within an SLT-FPOCC model and examine c

How Can Speech and Language Therapists be Supported to Make Decisions Regarding Communication Therapy for People With Dementia? A Realist Theory-Development Study.

INTRODUCTION: As of 2021, there were 57 million people with dementia worldwide, and there are 885,000 people with dementia in the UK. Communication difficulties occur in all forms of dementia. Increasing difficulty communicating thoughts and needs affects social interaction and personal relationships, with implications for individuals' and their relatives' quality of life. Speech and language therapists are trained to provide communication support to people experiencing communication difficulties. However, whether people with dementia are offered communication support from speech and language

Timing of transcranial direct current stimulation (tDCS) combined with speech and language therapy (SLT) for aphasia: study protocol for a randomized controlled trial.

BACKGROUND: Studies suggest that language recovery in aphasia may be improved by pairing speech-language therapy with transcranial direct current stimulation. However, results from many studies have been inconclusive regarding the impact transcranial direct current stimulation may have on language recovery in individuals with aphasia. An important factor that may impact the efficacy of transcranial direct current stimulation is its timing relative to speech-language therapy. Namely, online transcranial direct current stimulation (paired with speech-language therapy) and offline transcranial di

Establishing an Electronic Referral System from Speech and Language Therapy to Pharmacy to Improve Medication Administration in Patients with Dysphagia in an Acute Hospital Setting.

Dysphagia affects swallowing not only of food and drink, but also of orally administered medications. Altering solid or liquid dose formulations renders administration unlicensed and may have adverse effects. Medication administration in patients with dysphagia necessitates a multidisciplinary approach with no one profession holding all necessary expertise. This project aimed to improve the process of medication administration for patients with dysphagia in an acute hospital. Following a baseline audit of practice, an electronic referral system from Speech & Language Therapy (SLT) to Pharmacy

What are the primary influences on treatment decisions? How does this reflect on evidence-based practice? Indications from the discipline of speech and language therapy.

RATIONALE, AIMS AND OBJECTIVES: Four pillars of evidence underpin evidence-based behavioural practice: research evidence, practice evidence, patient evidence and contextual evidence. However, it is unknown which of these pillars or other factors are used by clinicians such as speech and language therapists (SLTS) when making treatment choices. The aim of this study was to identify the factors underpinning SLTs' treatment decisions and contextualize findings in terms of evidence-based practice (EBP). METHODS: Ethical approval was obtained for the electronic questionnaire-based study. A questio

LEGO therapy and the social use of language programme: an evaluation of two social skills interventions for children with high functioning autism and Asperger Syndrome.

LEGO therapy and the Social Use of Language Programme (SULP) were evaluated as social skills interventions for 6-11 year olds with high functioning autism and Asperger Syndrome. Children were matched on CA, IQ, and autistic symptoms before being randomly assigned to LEGO or SULP. Therapy occurred for 1 h/week over 18 weeks. A no-intervention control group was also assessed. Results showed that the LEGO therapy group improved more than the other groups on autism-specific social interaction scores (Gilliam Autism Rating Scale). Maladaptive behaviour decreased significantly more in the LEGO and S

Extracting adverse event nature, severity, timelines and resulting interventions from clinical notes of patients receiving CAR-T cell therapy using large language models.

Chimeric Antigen Receptor T-cell (CAR‑T) therapy, genetically engineered patient T cells targeting tumor antigens, has transformed care for hematologic malignancies but requires careful tracking of adverse events (AEs) often documented only in unstructured electronic health record (EHR) notes. We evaluated a Large Language Model (LLM)-based approach in UCSF's secure environment to extract AEs, dates, grades, and interventions within 30 days post‑infusion for six commercial CAR‑T products (2012-2023), benchmarking against two evaluators. Large Language Model-Powered Conversational Agent Delivering Problem-Solving Therapy (PST) for Family Caregivers: Enhancing Empathy and Therapeutic Alliance Using In-Context Learning.

BACKGROUND: Two widely used full-endoscopic lumbar interbody fusion (FE-LIF) techniques include the facet-preserving trans-Kambin approach, performed via a transforaminal corridor, and the facet-sacrificing endoscopic posterolateral transforaminal lumbar interbody fusion, which utilizes an interlaminar route. More recently, several authors have introduced an emerging transfacet endoscopic approach as an alternative, minimally invasive strategy for LIF. However, the majority of these studies have relied on direct decompression of neural elements, and the effectiveness of indirect decompression

Competent in evidence-based practice (EBP): validation of a measurement tool that measures EBP self-efficacy and task value in speech-language therapy students.

BACKGROUND: Worldwide speech-language therapy (SLT) students are educated in evidence-based practice (EBP). For students to use EBP in their future day-to-day clinical practice, they must value EBP as positive and must feel confident in using it. For curricula developers it is therefore important to know the impact their teaching has on these aspects of students' motivational beliefs. AIMS: To develop and validate a measurement tool to assess EBP task value and self-efficacy in SLT students. METHODS & PROCEDURES: A 20-item questionnaire was developed based on a review of the literature and a

"Then you are left on your own": barriers to guideline-based speech and language therapy for aphasia along the patient journey in the German-speaking healthcare context.

BACKGROUND: Although evidence-based recommendations for aphasia rehabilitation exist, guideline-based speech and language therapy is still not consistently implemented in routine care. Previous studies have described barriers to therapy participation from the perspective of people with aphasia, but less is known about how these barriers develop along the patient journey, especially in outpatient care. This study explored systemic and individual barriers to the uptake of speech and language therapy in aphasia care and identified implications for improving services. METHODS: An exploratory qual
