Medical clown intervention shortens length of hospitalization in children with pneumonia.

Community-acquired pneumonia (CAP) is a leading cause of hospitalization in children. The hospitalization duration depends on factors as child's well-being, vital signs, need for parenteral treatments, and development of complications. Medical clowns (MCs) are known to assist in reducing pain and alleviating anxiety and have been integrated into many aspects of hospital treatment routines. Our aim was to evaluate the effect of MC intervention on length of hospitalization in children admitted with CAP. A prospective quasi-randomized controlled trial allocated 51 children (2-18 years) hospitaliz

[Epilepsy with laughing seizures, hypothalamic hamartoma and precocious puberty. Contributions of MRI, computed EEG topography (CET) and ambulatory EEG (A-EEG)].

The concurrence of gelastic (laughing) seizures, hypothalamic hamartoma and precocious puberty constitutes a well defined epileptic syndrome in children; moreover mental retardation, neuropsychological deterioration and behavioral disorders have been often observed in these patients. In two cases we studied by means of MRI the appearance and the site of the hamartoma (in the posterior part of the hypothalamus with extension toward the third ventricle). The EEG study was performed by means of repeated recordings, of Computed EEG Topography (CET) and of Ambulatory EEG (A-EEG): in both patients d

Fragmented traces: Lesbianism and the Patronato's policies of confinement in late Francoist Spain.

Research on sexual dissidence under Francoism has often highlighted the repression of homosexual men and trans women, while the trajectories of lesbians remain underexplored. This absence has typically been explained through the notion of 'structural invisibility': women's same-sex relationships were both easier to conceal and less directly targeted by Francoist legal apparatus. Yet new documentary findings challenge this assumption. This article analyses a previously unknown set of medical and administrative files attributed to the Patronato de Protección a la Mujer, an institution jointly ru

Evaluating the Effectiveness of CHRIS'S Birds Assisted Therapy on Dental Anxiety among Pediatric Patients: A Pilot Study.

BACKGROUND: The heart of childhood nurtures an array of immature emotions. Through this rollercoaster of childhood emotions, from tears to laughter, it is the sole responsibility of a pediatric dentist to help the children navigate through their emotions with their empathetic actions. Whenever a child is anxious or fearful in the dental chair, the child may put forth his emotion through crying or taking up a flight response. Dental caries is a disease that is aggravated when a child refuses to cooperate with dental treatment. The tiny holes in the tooth, when unfilled, may lead to bigger probl

"We don't know how to talk": Adolescent meaning making and experiences of participating in research on violence in Romania, South Africa, and the Philippines.

BACKGROUND: Current evidence on adolescent participation in violence research has primarily measured distress, harm or upset using quantitative methods. There are relatively few studies which have employed qualitative methods to understand adolescent emotional experiences, and to articulate the experiences of participation from their own perspective. OBJECTIVE: This study aimed to assess adolescents' experiences of participating in research on violence in different contexts, namely Romania, South Africa, and the Philippines. METHODS: A purposive sample of adolescents (N = 53, 51 % female) we

Positive memory increases cataplexy-like behaviors in narcolepsy mice as revealed using conditioned place preference test.

BACKGROUND: Cataplexy is a loss of muscle tone that can lead to postural collapse, disturbing the daily life of narcolepsy patients; it is often triggered by positive emotions such as laughter in human patients. Narcolepsy model mice also show cataplexy, and its incidence increases in response to positive emotion-inducing stimuli such as chocolate and female courtship. Although such observation indicates a positive emotion-related nature of cataplexy in narcolepsy mice, they also show cataplexy without any apparent triggering stimulus ~ (spontaneous cataplexy). Therefore, we hypothesized that

Angelman syndrome: A review highlighting musculoskeletal and anatomical aberrations.

Angelman's syndrome (AS) is a genetic neurodevelopment disorder. The cause is a known abnormality involving the maternal inherited ubiquitin-protein ligase (UBE3A) gene. Clinical characteristics universal to the disorder are well documented in the literature and include developmental delay, seizures, ataxia, altered tone, severely impaired speech and intellect, as well as an overall happy demeanor, frequent bouts of laughter, and hypermotoric behavior. Associated with this disorder are several musculoskeletal aberrations. To date, a review of case studies reporting on these musculoskeletal cha

Degenerative pontine lesions in patients with familial narcolepsy.

BACKGROUND AND PURPOSE: Narcolepsy is characterized by chronic excessive daytime sleepiness with episodic sleep attacks. There are several associated symptoms of narcolepsy: cataplexy (bilateral muscle weakness without loss of consciousness provoked by an emotional trigger, e.g. laughter), sleep paralysis and hypnagogic-hypnopompic hallucinations. Most cases are sporadic; familial narcolepsy contributes to only 1-5% of all cases. While most cases of narcolepsy are idiopathic and are not associated with clinical or radiographic evidence of brain pathology, symptomatic or secondary narcolepsy ma

Treatment of cataplexy in Niemann-Pick disease type C with the use of miglustat.

Cataplexy is the sudden muscle weakness brought on by strong emotions, particularly joking, laughter, or anger. Cataplexy may involve only certain group of muscles or the entire voluntary musculature. In rare cases, symptoms of cataplexy can be seen during the course of some inherited diseases (Niemann-Pick type C (NPC), Prader-Willi syndrome, myotonic dystrophy, Norrie disease). We report the successful use of miglustat, a reversible inhibitor of the enzyme glucosylceramide synthase, approved for use in Gaucher's disease, and which catalyses the first step in the biosynthesis of most glycosph

Clinical and genetic aspects of Angelman syndrome.

Angelman syndrome is characterized by severe developmental delay, speech impairment, gait ataxia and/or tremulousness of the limbs, and a unique behavioral phenotype that includes happy demeanor and excessive laughter. Microcephaly and seizures are common. Developmental delays are first noted at 3 to 6 months age, but the unique clinical features of the syndrome do not become manifest until after age 1 year. Management includes treatment of gastrointestinal symptoms, use of antiepileptic drugs for seizures, and provision of physical, occupational, and speech therapy with an emphasis on nonverb

Angelman syndrome associated with oculocutaneous albinism due to an intragenic deletion of the P gene.

Angelman syndrome (AS) is a neurodevelopmental disorder characterized by mental retardation, speech impairment, ataxia, and happy disposition with frequent smiling. AS results from the loss of expression of a maternal imprinted gene, UBE3A, mapped within 15q11-q13 region, due to different mechanisms: maternal deletion, paternal UPD, imprinting center mutation, and UBE3A mutation. Deletion AS patients may exhibit hypopigmentation of skin, eye, and hair correlating with deletion of P gene localized in the distal part of Prader-Willi (PWS)/AS region. Our patient presented developmental delay, sev

Narcolepsy-cataplexy syndrome associated with DRB1*0806-DQB*0602 haplotype in a Caucasian patient.

Narcolepsy-Cataplexy (NC) is a neurological disorder associated with the human leukocyte antigen HLA DR2. This is a prerequisite for the disease in 95 to 98% of Caucasian patients. It has been demonstrated that the HLA DQB1*0602 allele is a better marker for narcolepsy than DRB1*1501 (DR2). We present a DR-negative and DQB1*0602-positive Caucasian Spanish patient with a very unusual genotype. A 20-year-old male presented with a 12-year history of excessive daytime sleepiness and sudden muscle weakness caused by laughter and disturbed nocturnal sleep. He had never presented hypnagogic hallucina

Four legs or two: up front with you--preventing tragedies by keeping children and pets out of the cargo areas of pickup trucks.

Dogs and children riding in the cargo area of pickup trucks are common sights on our nation's highways, country roads, and small town streets. This scene brings to mind idyllic childhoods filled with summer days spent bouncing down roads in the back of grandpa's pickup truck, wind catching the laughter from children's throats. For hundreds of families each year, this activity is the cause of heartache, traumatic injuries, and senseless deaths. More than 200 people die each year, half of whom are younger than 18 years of age, as a direct result of falls from cargo areas of pickup trucks. Injuri

Neuropsychiatric Symptoms After HHV-6 Encephalitis in an Immunocompetent Child: A Case Report.

BACKGROUND Human herpesvirus 6 (HHV-6) is a common pathogen known to cause roseola infantum in children and encephalitis in immunocompromised individuals. In immunocompetent children, HHV-6 encephalitis is rare and typically self-limiting. Although neurological symptoms such as seizures and altered consciousness have been reported, neuropsychiatric manifestations - particularly catatonia - are extremely uncommon. CASE REPORT A 4-year-old immunocompetent boy presented with sudden behavioral changes 1 week after a viral infection. Cerebrospinal fluid testing showed HHV-6 positivity. His symptoms

Association between lack of sexual interest and all-cause mortality in a Japanese general population: The Yamagata prospective observational study.

BACKGROUND: Sexual interest is essential for maintaining positive sexual relationships and sexual function, which have recently been recognized as important indicators of good health and quality of life. Here, we prospectively investigated associations between sexual interest and mortality in a community-based population. METHODS: This study enrolled 20,969 subjects (8,558 males and 12,411 females) aged ≥ 40 years who participated in annual health check-ups in Yamagata Prefecture. Sexual interest was assessed by a self-report questionnaire. Associations between sexual interest and increased a

Awakenings in rats by ultrasounds: A new animal model for paradoxical kinesia.

Paradoxical kinesia refers to a sudden transient ability of akinetic patients to perform motor tasks they are otherwise unable to perform. The mechanisms underlying this phenomenon are unknown due a paucity of valid animal models that faithfully reproduce paradoxical kinesia. Here, in a first experiment, we present a new method to study paradoxical kinesia by "awakening" cataleptic rats through presenting appetitive 50-kHz ultrasonic vocalizations (USV), which are typical for social situations with positive valence, like juvenile play or sexual encounters ("rat laughter"). Rats received system

Putting salient vocalizations in context: Adults' physiological arousal to emotive cues in domestic and external environments.

Salient vocalizations are automatically processed and distinguished from emotionally irrelevant information. However, little is known of how contextual, gender and attentional variables interact to modulate physiological responses to salient emotive vocalizations. In this study, electrocardiogram (ECG) was utilized to investigate differences in peripheral nervous activity of men and women to infant cry (IC), infant laughter (IL) and adult cry (AC) in two different situational contexts: the domestic environment (DE) and the outside environment (OE). As the mental state of listeners can affect t
