Lessons Learned in the Response to Multiple Cases of Exertional Hyperthermia at an Urban Half Marathon.

OBJECTIVE: Exertional heat stroke (EHS) occurs when core body temperature exceeds 40°C (104°F) with central nervous system dysfunction and has been identified as a leading cause of morbidity and mortality among endurance athletes. With rapid identification and treatment, survival approaches 100%. This study describes the clinical presentation and course of seven patients transported by emergency medical services from a half marathon with EHS. METHODS: The 2022 Cambridge Half Marathon occurred during unusually warm weather, with a did not finish rate of 2%. Retrospective review was performed o

Analysis of over 1 million race records shows runners from East African countries as the fastest in 50-km ultra-marathons.

The 50-km ultra-marathon is a popular race distance, slightly longer than the classic marathon distance. However, little is known about the country of affiliation and age of the fastest 50-km ultra-marathon runners and where the fastest races are typically held. Therefore, this study aimed to investigate a large dataset of race records for the 50-km distance race to identify the country of affiliation and the age of the fastest runners as well as the locations of the fastest races. A total of 1,398,845 50-km race records (men, n = 1,026,546; women, n = 372,299) were analyzed using both descrip

Effects of extreme mountain ultramarathon on stress-related biomarkers from multiple biological matrices in adult male runners.

Prior investigations concerning finishers of the Tor des Géants (TDG) have demonstrated oxi-inflammatory response. MicroRNAs (miRNAs) have been identified as prospective biomarkers for oxi-inflammatory and stress responses. This study addressed the acute responses concerning salivary miRNAs, circulating redox markers and urinary exercise-induced markers during the 2019 TDG. It included seven healthy male participants who successfully completed the race. Biological specimens (blood, saliva, and urine) were collected 1-2 days prior to the race and immediately post-completion, to assess redox sys

[Diseases and overuse injuries of the lower extremities in long distance runners].

Running is one of the most popular sports worldwide, with running events attracting hundreds of thousands of runners of all age groups. Running is an effective way to improve health but is also associated with a high risk of injuries. Up to 50% of regular runners report having more than one injury each year. Some injuries are caused by an accident but most are caused by overuse. The most frequent diagnoses are patellofemoral pain syndrome, tibial stress syndrome (shin splint), Achilles tendinopathy, iliotibial band friction syndrome (runner's knee), plantar fasciitis and stress fractures of th

The Dynamics of Cardiovascular Biomarkers in non-Elite Marathon Runners.

The number of recreational/non-elite athletes participating in marathons is increasing, but data regarding impact of endurance exercise on cardiovascular health are conflicting. This study evaluated 79 recreational athletes of the 2016 Barcelona Marathon (72% men; mean age 39 ± 6 years; 71% ≥35 years). Blood samples were collected at baseline (24-48 h before the race), immediately after the race (1-2 h after the race), and 48-h post-race. Amino-terminal pro-B type natriuretic peptide (NT-proBNP, a marker of myocardial strain), ST2 (a marker of extracellular matrix remodeling and fibrosis, infl

Correlation properties and respiratory frequency of ECG-derived heart rate variability during multiple race-pace running intervals in female and male long-distance runners.

Aim was to evaluate alterations of the non-linear short-term scaling exponent alpha1 of detrended fluctuation analysis (DFAa1) of heart rate (HR) variability (HRV) as a sensitive marker for assessing global physiological demands during multiple running intervals. As a secondary analysis, agreement of ECG-derived respiratory frequency (EDR) compared to respiratory frequency (RF) derived from the metabolic cart was evaluated with the same chest belt device. Fifteen trained female and male long-distance runners completed four running bouts over 5 min on a treadmill at marathon pace. During the la

Valuing an urban marathon as a public sport service: evidence from the Seoul international marathon.

This study examined the nonmarket value of the Seoul International Marathon (SIM) as part of the city's sport infrastructure and public sport service provision, using the Contingent Valuation Method (CVM) to estimate Seoul residents' willingness to pay for the event. An online survey of 1,250 adult residents employed a double-bounded dichotomous-choice format in a hypothetical tax-based donation scenario. A logit model was used to explore which factors shape willingness to pay, and bid level, interest in marathon events, satisfaction with related infrastructure, event perception, occupation, a

Improved Function in a Runner With Hereditary Spastic Paraparesis With Use of Extracorporeal Shockwave Therapy: Personal Clinical Experience.

A 59-yr-old male marathon runner presented with recent diagnosis of hereditary spastic paraplegia in the setting of gait deviation and spasticity. He noted asymmetric wear pattern of his right shoe and toe drag, with recent development of left lower limb pain and cramping attributed to spasticity. He elected to proceed with radial extracorporeal shockwave treatment targeting the affected muscles. The night following initial treatment, he was able to run 2 mins per mile faster over a 4-mile run with resolution of toe drag. He completed six sessions of radial extracorporeal shockwave treatment a

Not All Runners Cross the Same Finish Line: Sociodemographic Inequalities in COVID-19 Recovery After a Mass Sporting Event.

BACKGROUND: The COVID-19 pandemic significantly disrupted patterns of physical activity and participation in mass sporting events, with recreational runners in Latin America among the most affected. In Colombia, pre-existing inequalities in access to sport further exacerbated these impacts. Nevertheless, evidence on post-COVID-19 impact and recovery experiences among regional runners remains limited. OBJECTIVE: We examined the sociodemographic profiles, athletic experience, and perceptions of COVID-19-related impact and recovery among participants in the 2023 Medellín Marathon, and to assess

Ultramarathon is an outstanding model for the study of adaptive responses to extreme load and stress.

Ultramarathons comprise any sporting event involving running longer than the traditional marathon length of 42.195 km (26.2 miles). Studies on ultramarathon participants can investigate the acute consequences of ultra-endurance exercise on inflammation and cardiovascular or renal consequences, as well as endocrine/energetic aspects, and examine the tissue recovery process over several days of extreme physical load. In a study published in BMC Medicine, Schütz et al. followed 44 ultramarathon runners over 4,487 km from South Italy to North Cape, Norway (the Trans Europe Foot Race 2009) and reco

Morbidity Among Athletes Presenting for Medical Care During 3 Iterations of an Ultratrail Race in the Himalayas.

INTRODUCTION: Although ultratrail races are increasing in popularity, there is a dearth of data regarding illnesses and medical care at these events. Data about injuries and illnesses for races taking place in the Himalayas, where the nearest medical facility can be hundreds of miles away, are even harder to find. This study aimed to describe the injuries and illnesses befalling the participants of a 7-stage 212 km (132 mi) trail race at high altitude. METHODS: Ethical approval was obtained from Nepal Research Health Council. A retrospective study of the record of medical encounters among the

Sacral stress fracture in a young-adult, long-distance runner: an underestimated cause of low back pain.

Sacral stress fractures (SSFs) in physically active young patients are frequently misdiagnosed due to the lack of specificity of signs and symptoms. Over the last years, these injuries have been described as rare, although some studies report that the incidence of an SSF in athletes may be as high as 20%.We describe a case of a male long-distance runner in his late 20s with a 1-month undiagnosed SSF. The patient complaints included insidious right low back and buttock pain without trauma that started after running a marathon. MRI revealed an extensive area of bone marrow oedema in the right sa

Acetabular labral tears with underlying chondromalacia: a possible association with high-level running.

PURPOSE: The use of hip arthroscopy has helped delineate intra-articular pathology and has enabled clinicians to further elucidate the factors responsible for injuries, such as running. The subtle development of degenerative changes may be a result of repetitive impact loading associated with this sport. This study presents a population of runners with common pathologic acetabular changes. TYPE OF STUDY: Case series. METHODS: Eight high-level runners with an average age of 36 years (range, 19 to 45 years) were seen for complaints of increasing hip pain with running without any history of mac
