An unusual case of sub-condylar bilateral fracture and bilateral post-traumatic temporomandibular ankylosis.

A case of bilateral sub-condylar fracture with wide stump dislocation associated with a central facial trauma, fracture-intrusion of the rhino-orbital-maxillary complex and a parasymphyseal mandibular fracture, is reported. After surgery and inter-maxillary fixation an unusual temporo-mandibular ankylosis developed. Maximum mouth opening, lateral and protrusive movements were severely limited. Surgical treatment of ankylosis was requested and performed. The originality of this case lies in the atypical lateral dislocation of condylar neck fractured stumps to the zygomatic arches and in the lat

Fracture patterns in endodontically treated teeth restored with or without horizontal fiber posts: An in vitro study.

CONTEXT: Endodontically treated maxillary premolars are more susceptible to fracture because of the loss of tooth structure following caries removal and endodontic procedures. Various composite restorative systems and reinforcement strategies, such as horizontal fiber posts (HFPs), have been proposed to improve fracture behavior and promote more favorable failure patterns. AIM: The aim of this study was to evaluate fracture patterns in endodontically treated maxillary premolars restored with different composite systems, with and without horizontal fiber posts (HFP). MATERIALS AND METHODS: Si

A five-year follow-up of a root fracture in a ten-year-old boy.

The management of horizontal root fracture still presents a challenge to clinicians. The case of a 10-year-old boy with a horizontal root fracture in the left maxillary central incisor (tooth number 21) is described here. The fracture was present at the junction of the apical and the middle thirds. We report both the emergency care and the 5-year follow-up. At the same time, an orthodontic treatment was planned for a retained maxillary cuspid (tooth number 23), first composed of a high-pull headgear and then by a fixed multi-bracket appliance. Today, the tooth number 21 remains vital and sympt

Endoscopic transmaxillary repair of orbital floor fractures: a minimally invasive treatment.

Although endoscopic transmaxillary repair of orbital floor fractures is a minimally invasive treatment, controversy remains regarding the method for supporting the orbital floor after elevation of the orbital contents. To date, a urethral balloon catheter has been widely used. However, it can be difficult to leave the catheter in place for a long time period due to the inconvenience, and prolapse of the orbital contents may recur in the case of its premature removal. This study described the techniques for endoscopic reduction and use of a balloon for orbital floor fractures. From June 2006 th

Impact of conservative and traditional endodontic accesses on the strength of maxillary zirconia crowns.

PURPOSE: Performing endodontic procedures on existing crowns is common when replacing the crown is not feasible. Traditional triangular access (TTA) is standard for maxillary molars, but more conservative designs are reported in the literature. However, research comparing the fracture resistance of maxillary molar zirconia crowns using traditional and novel endodontic access sealed with composite resin remains limited. The purpose of this in vitro study was to evaluate the fracture resistance of maxillary molar zirconia crowns with various endodontic cavity designs: traditional triangular acce

Management of multiple and displaced mandibular fractures in a pediatric patient sans mandibular immobilization, sans open reduction and internal fixation.

Maxillofacial fractures in the pediatric population are generally managed by conservative approaches such as soft diet and medication or semi-invasive procedures namely inter-maxillary fixation or circum-mandibular wiring. These approaches are preferred over any invasive treatment to minimize injury to the growing skeleton and tooth germs. Displaced fractures that cause functional problems such as restricted mouth opening, malocclusion or impaired breathing, mandate open reduction and internal fixation. However, surgical management is associated with morbidity related to general anesthesia, ri

The association of Le Fort midfacial fractures with frontobasal injuries: a 17-year review of 125 cases, reflections on biomechanics, classifications and treatment.

The frequency of midface and frontobasal fractures has increased over the past 40 years despite the improvement and stringent regulation implemented on modern safety equipment (belts, helmets…). This observation might be correlated with the progress of radiodiagnosis tools. Literature was reviewed according to Prisma guidelines. We searched for reviewed articles, published between January 2000 and December 2017, through Medline (Pubmed) online databases and ScienceDirect, using the following MeSH Keywords: "Le Fort classification", "Le Fort fracture", "Frontobasal fracture", "skull base fractu

Role of CBCT in Prediction of Oro-antral Communication Post Third Molar Extraction: A Retrospective Study.

OBJECTIVE: Oro-antral communication (OAC) is one of the most frequently encountered complications during third molar extraction. Various radiographic factors, like excessive maxillary sinus pneumatization, long periods of edentulism, periapical lesions, etc., have been considered high-risk factors for OAC. However, a panoramic radiograph has not proven to be accurate in predicting the chances of OAC. Through this retrospective study, we evaluated the efficacy of a CBCT in predicting the incidence of OAC after maxillary third molar extraction. MATERIALS AND METHODS: We conducted a retrospectiv

Zygomaticomaxillary fracture fixation: a prospective comparative evaluation of two-point versus three-point fixation.

PURPOSE: Zygomatic bone has a higher risk of sustaining injuries in the maxillofacial skeleton. On fracturing, zygomatic bone separates from the four neighbouring bones at its articulations. Treatment for zygomaticomaxillary fractures has evolved a long way since 3000 BC. With the advent of miniplates for midface fracture, controversies still exist regarding the stability of zygoma following 1, 2, and 3 points for fixation. The study aims to compare and determine the most effective technique for the reduction of zygomaticomaxillary fractures and the ability to retain the fractured zygoma in a

Mesenchymal phosphaturic tumour: early detection of recurrence.

The case of a recurrent phosphaturic mesenchymal tumour of the maxillary sinus 10 years after the first surgical excision is reported. The neoplasm first presented with paraneoplastic osteomalacia causing a pathological femur fracture. A right maxillary sinus tumour was identified and treated thereafter. The patient had no local symptoms and serum electrolytes returned to normal after surgical removal of the tumour. However, 10 years later, the patient's urine Ca and P levels increased and an octreoscan detected a new tumour in the right maxillary sinus. Early diagnosis prevented the effects o

Esthetic enhancement of a traumatized anterior tooth with a combination of forced eruption and tooth alignment: a case report.

Exposing sound structure of a subgingivally fractured tooth using orthodontic extrusion is considered to be a conservative way to re-establish biologic width without sacrificing esthetics or jeopardizing periodontal support of neighboring teeth. When a misaligned tooth is traumatically involved, a more comprehensive approach combining tooth extrusion and re-alignment may be necessary for a successful restorative outcome. This case report describes a successful esthetic management of a patient with complicated crown-root fracture on the maxillary right central incisor and pre-existing malocclus

Critical Degree of Orbital Floor Displacement Drives Operative Repair of Zygomaticomaxillary Complex Fractures: Findings from a 10-Year Retrospective Study.

Among zygomaticomaxillary complex (ZMC) fractures presenting to a tertiary urban academic center, the authors hypothesized the presence of both clinical and radiographic predictors of operative management. The investigators conducted a retrospective cohort study of 1,914 patients with facial fractures managed at an academic medical center in New York City between 2008 and 2017. The predictor variables were based on both clinical data and features of pertinent imaging studies, and the outcome variable was an operative intervention. Descriptive and bivariate statistics were computed and the p-va

Etiology and pattern of zygomatic bone fracture among patients with facial injuries attending Muhimbili National Hospital, Tanzania.

The zygomatic bone is frequently fractured following maxillofacial injuries due to its prominence. Fracture of this bone has functional, cosmetic, and psychological effects. Proper management is therefore crucial. Data on the etiology and pattern of zygoma fracture will aid in planning management and setting appropriate preventive measures. The study aimed to determine the etiology and pattern of zygomatic bone fractures among patients with facial injuries attending Muhimbili National Hospital. We carried out a prospective hospital-based cross-sectional study where a consecutive sampling of pa

Facial bone fractures in children.

From the aforementioned study, the following conclusions can be stated: (1) Patients ranged from 2 to 15 years of age, with no one age group being more susceptible to facial fractures. This fact is contrary to previous findings. (2) No true orbital blow-out fractures were found in patients less thn 7 years of age, primarily because of the lack of maxillary sinus development in that age group. The most rapid development of the maxillary sinus occurs between the ages of 7 to 15 years. (3) In patients from 2 to 7 years of age, fractures occurred at an even ratio of boys and girls. From 8 to 15 ye
