Chemotherapy in advanced non-small-cell lung cancer. The experience of Italian Cooperative Groups.

The clinical experience of Italian Cooperative Groups in the treatment of advanced non-small-cell lung cancer began in the early 1980's with several phase II trials on cisplatin-etoposide (PE) combination. PE was chosen and widely used also in neoadjuvant trials on the basis of several randomized studies showing a favourable safety-efficacy profile of this combination with respect to other chemotherapy regimens. Recently, in order to improve therapeutic results in NSCLC the Italian Oncology Group for Clinical Research compared PE versus mitomycin, ifosfamide and cisplatin (MIC) and mitomycin,

Clinical research on homeopathic preparations: protocol template for a series of systematic reviews.

BACKGROUND: The clinical benefits of homeopathic preparations (HPs) used in anthroposophic medicine and homeopathy remain a topic of debate. Systematic reviews (SRs), with or without meta-analyses (MAs), that assess the literature in line with scientific standards and account for the complex nature of these interventions are lacking for many health conditions. This project aims to conduct SRs to evaluate the efficacy, effectiveness, and safety of HPs for selected patient populations, using a pluralistic approach that considers internal and model validity. METHODS: This protocol follows PRISMA

Multitask learning of longitudinal circulating biomarkers and clinical outcomes: identification of optimal machine-learning and deep-learning models.

BACKGROUND: Many circulating biomarkers are assessed at different time intervals during clinical studies. Despite of the success of standard joint models in predicting clinical outcomes using low-dimensional longitudinal data (1-2 biomarkers), significant computational challenges are encountered when applying these techniques to high-dimensional biomarker datasets. Modern machine- or deep-learning models show potential for multiple biomarker processes, but systematic evaluations and applications to high-dimensional data in the clinical settings have yet to be reported. We aimed to enhance the

The rationale and study design of two phase II trials examining the effects of BI 685,509, a soluble guanylyl cyclase activator, on clinically significant portal hypertension in patients with compensated cirrhosis.

BACKGROUND: Clinically significant portal hypertension (CSPH) drives cirrhosis-related complications (i.e. hepatic decompensation). Impaired nitric oxide (NO) bioavailability promotes sinusoidal vasoconstriction, which is the initial pathomechanism of CSPH development. Activation of soluble guanylyl cyclase (sGC), a key downstream effector of NO, facilitates sinusoidal vasodilation, which in turn may improve CSPH. Two phase II studies are being conducted to assess the efficacy of the NO-independent sGC activator BI 685,509 in patients with CSPH due to various cirrhosis aetiologies. METHODS: T

Role of immunotherapy in stage III nonsmall cell lung cancer.

PURPOSE OF REVIEW: Despite aggressive treatment based on definitive chemoradiotherapy, 5-year overall survival in unresectable stage III nonsmall cell lung cancer remains poor (15-20%). The novel immunotherapy based on immune checkpoint inhibitors (ICIs) presents as the therapeutic 'Holly Grail' in lung cancer treatment. RECENT FINDINGS: Preclinical models provide evidence of immunotherapy-radiotherapy (IM-RT) synergy. The exposure to ionizing radiation turns tumor in an in-situ vaccine, primes the innate immune system, increases immunotherapy efficacy by overcoming the immunosuppressive micr

Comparative efficacy of phase 2-3 therapies for non-cirrhotic metabolic dysfunction-associated steatohepatitis: An updated network meta-analysis.

BACKGROUND: Metabolic-dysfunction-associated steatohepatitis has a rapidly expanding phase 2-3 drug pipeline, yet comparative evidence across mechanisms remains limited, and most trials are placebo controlled. METHODS: We performed a systematic review and frequentist network meta-analysis of phase 2-3 randomized trials in adults with non-cirrhotic disease. Primary endpoints were (1) resolution of steatohepatitis without worsening of fibrosis, (2) at least one-stage improvement in fibrosis without worsening of steatohepatitis (both biopsy based), and (3) at least a 30% relative reduction in li

Nonsteroidal anti-inflammatory drugs (NSAIDs) for acute renal colic.

BACKGROUND: Urolithiasis (urinary stones) is a common disease with an increasing incidence globally. It often presents with renal colic, which is characterised by acute and intense abdominal pain. The first step in the management of renal colic is pain control. Various medications, including narcotics, nonsteroidal anti-inflammatory drugs (NSAIDs), antispasmodics, and others, have been used for this condition. NSAIDs are amongst the most commonly used drugs for renal colic. They act by reducing inflammation and lowering the pressure inside the urinary collecting system. This review updates a p
