Expanding the Phenotypic Spectrum of SLC1A4-Related Spastic Tetraplegia: A Case With Novel Multisystem Features.

Spastic tetraplegia, thin corpus callosum, and progressive microcephaly (SPATCCM) is a rare autosomal recessive neurodevelopmental disorder caused by pathogenic variants in the SLC1A4 gene. 24 cases have been reported worldwide. We describe a 30-month-old boy born to consanguineous parents, presenting with global developmental delay, seizures, progressive microcephaly, spastic tetraplegia, feeding difficulties, and recurrent infections. Genetic testing confirmed a homozygous pathogenic variant in SLC1A4 (p.Arg457Trp). Notably, our patient exhibited novel systemic phenotypic features, including

The role of 3D printing in chest wall reconstruction.

Technology is advancing fast, and chest wall surgery finds particular benefit in the broader availability of three-dimensional (3D) reconstruction and printing. An increasing number of reports are being published on the use of these resources in virtual 3D reconstructions of chest walls in computed tomography (CT) scans, virtual surgeries, 3D printing of real-size models for surgical planning, practice, and education, and of note, the manufacture of customized 3D printed implants, changing the fundamental conception from a surgery that fits all, to a surgery for each patient. In this review, w

Diced autologous rib cartilage for primary treatment or refinement of minor chest wall deformities.

BACKGROUND: Various methods of corrective thoracoplasty for congenital anterior chest wall deformities have been described. To date, no studies have described the use of diced cartilage as a surgical procedure in thoracoplasty. The authors present their experience with 23 patients for whom diced cartilage grafts were used as an innovative approach for thoracoplasty. METHODS: A retrospective review of 23 patients was performed. From January of 2004 to September of 2010, 18 patients underwent repair of pectus excavatum, three patients underwent repair of pectus arcuatum, and two patients underw

X-Linked Spinal Muscular Atrophy 2 due to a Synonymous Variant in the UBA1 Gene in a Family with Novel Findings from Turkey.

Spinal muscular atrophy, X-linked 2 (SMAX2) is a rare type of spinal muscular atrophy characterized by muscle weakness, hypotonia, areflexia, myopathic face, tongue fibrillations, contractures, bone fractures, and cryptorchidism. Variants of the UBA1 gene lead to SMAX2. The UBA1 gene encodes a protein that activates the ubiquitin pathway which is responsible for protein degradation. Here, we describe a family presenting with hypotonia, muscle weakness, areflexia, contractures, weak cry, in association with other anomalies including myopathic face, scoliosis, tongue fibrillations, and cryptorch

Metatropic dysplasia in children.

Metatropic dysplasia is a rare genetic condition characterized by progressive dwarfism. Metatropic dysplasia is defined as a short limb skeletal dysplasia characterized by dumbell like configuration of long bones, a narrow but normal length of thorax and occasionally a coccygeal appendage similar to a tail. Children born with this condition show different signs and symptoms throughout the childhood. An eight years old boy was admitted in the Paediatric ward of Bangbandhu Sheikh Mujib Medical University (BSMMU), Dhaka with the complain of swelling in the back for 7 1/2 years. On examination the

Intentional Lower Pole Rotation of Anatomic Breast Implants in Chest Wall Deformities.

Several methods have been described for the correction of congenital thoracic wall deformities. Our aim was to investigate the feasibility and clinical results of using standard anatomic breast implants with modified anatomic positioning according to the defect in congenital thoracic wall deformities. Between 2014 and 2015, 5 patients diagnosed with pectus excavatum (PE, n = 4) or pectus carinatum (PC, n = 1) and breast asymmetry or hypoplasia were evaluated. In all patients, a submammary incision and dual-plane subpectoral placement of texturized, anatomic implants were performed. In patients

Decorin expression, straw-like structure, and differentiation of human costal cartilage.

Costal cartilage is much understudied compared with the load-bearing cartilages. Abnormally grown costal cartilages are associated with the inherited chest wall deformities pectus excavatum and pectus carinatum resulting in sunken and pigeon chests, respectively. A lack of understanding of the ultrastructural and molecular biology of costal cartilage is a major confounder in predicting causes and outcomes of these disorders. This study analyzed the structure of marginal human costal cartilage (ribs 6-10) through scanning electron and atomic force microscopes and identified the presence of stra

Anaesthetic management of a patient with tumour-induced osteomalacia for an orthopaedic procedure.

Tumour-induced osteomalacia is a metabolic disorder that causes hypophosphataemia, resulting in bone mineralisation defects. If not treated definitively with tumour resection, this produces life-threatening anatomical changes. The anaesthetic management is tailored to address tumour location, perioperative positioning, as well as airway and pulmonary concerns. We report the anaesthesia delivery for the surgical excision of a tibial mass in a man in his late 30s with oncogenic osteomalacia. He had cervical spine deformity, pectus carinatum, severe kyphosis, lower limb atrophy with flexion contra

Marfan Syndrome With Recurrent Lower Left Posterior Toothache as the First Symptom: A Report on a Rare Case.

Marfan syndrome (MFS) is a life-threatening connective tissue disorder that affects multiple organs and systems. We report a case of MFS with recurrent lower left posterior toothache as the first symptom. A 23-year-old Asian man walked into the dental emergency room with a chief complaint of recurrent spontaneous and intermittent toothache in his lower left posterior tooth region, mimicking acute symptomatic pulpitis. He self-reported a relatively healthy medical status without any hereditary disease. However, his disproportionately elongated body structure, high myopia, and positive wrist sig

Case Report: A Synonymous Mutation in NF1 Located at the Non-canonical Splicing Site Leading to Exon 45 Skipping.

Synonymous mutations are generally considered non-pathogenic because it did not alter the amino acids of the encoded protein. Publications of the associations between synonymous mutations and abnormal splicing have increased recently, however, not much observations available described the synonymous mutations at the non-canonical splicing sites leading to abnormal splicing. In this pedigree, the proband was diagnosed Neurofibromatosis type I due to the presence of typical cafe' au lait macules and pectus carinatum. Whole-exome sequencing identified a synonymous mutation c.6795C > T (p.N2265N)

Safe intubation in Morquio-Brailsford syndrome: A challenge for the anesthesiologist.

Morquio-Brailsford syndrome is a type of mucopolysaccharidoses. It is a rare disease with features of short stature, atlantoaxial instability with risk of cord damage, odontoid hypoplasia, pectus carinatum, spine deformities, hepatomegaly, and restrictive lung disease. Neck movements during intubation are associated with the risk of quadriparesis due to cervical instability. This, along with the distortion of the airway anatomy due to deposition of mucopolysaccharides makes airway management arduous. We present our experience in management of difficult airway in a 3-year-old girl with Morquio-

Are Adolescents With Chest Wall Deformity Potential Primary Spontaneous Pneumothorax Patients?

INTRODUCTION: Chest wall deformities often become apparent with growth spurts during adolescence. Alterations in the structure of the chest wall can result in the development of pulmonary bullae and blebs, which may lead to spontaneous pneumothorax. There are limited data on this subject in the literature. In this study, it is aimed to investigate the presence of bullae and bleb in patients with anterior and posterior chest wall deformities. METHODS: Computed tomography scans of adolescent idiopathic scoliosis, pectus carinatum, and pectus excavatum patients were analyzed. The presence of bul
