Carbonic anhydrase inhibitors for prevention of space motion sickness: an avenue of investigation.

Space motion sickness (SMS) is an important medical problem facing NASA's space shuttle program. Two theories that have been advanced to explain SMS are the "fluid shift theory" and the "vestibulo-ocular sensory conflict theory". The "fluid shift theory" pre-supposes an active or passive shift of body fluid to the central nervous system (CNS) and vestibulo-auditory mechanisms. In contrast, the "sensory conflict theory" hypothesizes that unfamiliar accelerational and gravitational inputs from the middle ear conflict with visual inputs and lead to SMS. Carbonic anhydrase inhibitors (CAHI) are kn

Acute vestibulopathy.

Acute vestibulopathy is characterized by the acute or subacute onset of vertigo, dizziness or imbalance with or without ocular motor, sensory, postural or autonomic symptoms and signs, and can last for seconds to up to several days. Acute vestibular lesions may result from a hypofunction or from pathological excitation of various peripheral or central vestibular structures (labyrinth, vestibular nerve, vestibular nuclei, cerebellum or ascending pathways to the thalamus and the cortex). This update focuses on new aspects of the aetiology, pathophysiology, epidemiology, and treatment of (i) acut

[Human type II collagen induced autoimmune inner ear lesions in guinea pigs].

Guinea pigs were immunized with human type I, type II or type III collagen. Inner ear lesions were induced in some guinea pigs by immunizing them with human type II collagen. Histopathologic changes included endolymphatic hydrops, organ of Corti lesions and endolymphatic duct contained a large number of inflammatory cells. These histologic changes resembled those observed in patient with Meniere's disease. We didn't discover any inner ear lesions in guinea pigs by immunizing them with human type I and type III collagen, although their serum antibody titers were high and antibody presented in p

Poststapedectomy otitis media and meningitis.

A patient developed acute otitis media, labyrinthitis, and meningitis 16 months after a stapedectomy operation, and 14 similar cases have been collected from the literature. Poststapedectomy otitis media carries an increased risk of labyrinthitis and meningitis and requires prompt antibiotic therapy and regular observation until cured. Should labyrinthitis occur, with or without meningitis, fistula repair must be undertaken as soon as the infection is eliminated. All stapedectomy patients require regular supervision, and must report for urgent treatment if symptoms of otitis media or perilymph

Fluctuating hearing loss following immune reaction in the endolymphatic sac of guinea pigs.

This study has investigated immune injuries to the inner ear auditory system of guinea pigs. Following secondary antigen challenge to the endolymphatic sac, the mean hearing threshold significantly increased in the early phase from day 1 to day 3 and thereafter recovered. In the early phase, hearing threshold significantly increased simultaneously to the elevation of perilymph antibody levels. The size of hydrops was not the only factor that causes an increase in hearing loss as well as in AP/SP ratio. Scale-out hearing loss was seen in 2 animals with severe degeneration of the stria vasculari

Statistical analysis of otosclerosis surgery performed by Jean Marquet.

The early and long-term hearing results of 1,681 primary otosclerosis operations performed by the same surgeon, Jean Marquet, were reviewed retrospectively and analyzed with very strict statistical standards. Significantly better short- and long-term results were achieved with the stapedotomy technique compared to total stapedectomy, mainly at the higher frequencies (4 and 8 kHz) important for speech discrimination. Whatever technique was used to open the footplate (micropick, microdrill, or laser), no statistical audiometric difference could be found. The results were equal whether or not the

Biochemical mechanisms affecting susceptibility to noise-induced hearing loss.

In magnesium (Mg)-deficient rats and guinea pigs, noise-induced hearing loss (NIHL) was found to be correlated to the decrease of Mg in serum and perilymph. Also, in noise-exposed humans, NIHL increased with decreasing serum Mg. During the process of mechanoelectrical transduction within the hair cells in the inner ear, membrane permeability of K+ and Ca2+ will transiently increase. Mg deficiency may additionally increase membrane permeability and, therefore, energy-dependent K+ and Ca2+ turnover. The increased release of catecholamines in Mg deficiency may affect the hair cells, either direct

Mean intracranial pressure monitoring by a non-invasive audiological technique: a pilot study.

Intracranial pressure is normally transmitted to the perilymph of the cochlea via the cochlear aqueduct. The relationship between perilymphatic pressure, indirectly measured by tympanic membrane displacement, and mean intracranial pressure defined either clinically or by direct measurement has been examined in 58 patients (aged 5-77 years), with hydrocephalus, benign intracranial hypertension, intracranial tumours, subarachnoid haemorrhage and head injuries. The most consistent results were obtained in young patients with hydrocephalus and benign intracranial hypertension. However, the techniq

The membrane rupture theory of Menière's disease--is it valid?

The Membrane Rupture Theory states that the acute attack of Menière's disease occurs when endolymph, with its high potassium ion concentration, escapes into the perilymph and surrounds the first-order neuron. Initially, this causes partial depolarization of the nerve and an increased resting discharge, followed by complete depolarization. Clinically, one would expect an initial irritative nystagmus (toward the affected side) followed by a paralytic nystagmus (toward the unaffected side). To confirm this, the guinea pig perilymphatic space was perfused with an artificial endolymph solution. The

Revision stapes surgery: problems with some solutions.

A review of 217 consecutive revision stapes operations performed during a ten-year period revealed that the surgeon encounters more pathological variables than he does during primary operations. Hence, the technical solutions are less stereotyped and the net results less predictable. Prosthesis displacement, with or without incus tip erosion, was the most common primary cause of failure (82%). However, oval window problems such as footplate refixation, perilymph fistula, otosclerotic regrowth, and lateralized oval window membrane in 60% of cases often complicated prosthesis displacement. First

7-Tesla sodium magnetic resonance imaging of the inner ears in unilateral Ménière's disease and endolymphatic hydrops: an exploratory study.

BACKGROUND: Whilst delayed post-gadolinium MRI has led to a shift in the diagnostic paradigm of Meniere's Disease (MD), there remains a strong desire to develop a non-contrast enhanced MRI technique to detect and monitor MD. The endolymphatic space (ES) undergoes hydropic expansion in Ménière's Disease (MD) and the concentration of sodium ions in the endolymph is at least 10 times lower than that in the perilymph. It was hypothesised that the lower sodium (23Na) concentration in the endolymph relative to the surrounding perilymph would result in a differential reduction in 23Na-MRI signal in i

A three-dimensional finite element model of round window membrane vibration before and after stapedotomy surgery.

Piston stapes prostheses are implanted in patients with refractory conductive or mixed hearing loss due to stapes otosclerosis to stimulate the perilymph with varying degrees of success. The overclosure effect described by the majority of researchers affects mainly low and medium frequencies, and a large number of patients report a lack of satisfactory results for frequencies above 2 kHz. The mechanics of perilymph stimulation with the piston have not been studied in a systematic manner. The objective of this study was to assess the influence of stapedotomy surgery on round window membrane vib

Practical imaging for Ménière's disease.

Ménière's disease (MD) is a chronic disorder of the inner ear characterized by vertigo, hearing loss, tinnitus, and aural fullness. The pathophysiology of MD involves endolymphatic hydrops, an abnormal accumulation of endolymph fluid, although the exact cause remains unclear, potentially involving genetic, environmental, and autoimmune factors. Recent advancements in magnetic resonance imaging have significantly enhanced diagnostic capabilities. This technique uses gadolinium-based contrast agents to differentiate between endolymph and perilymph. Imaging techniques such as 3-dimensional fluid-

The effect of changes in perilymphatic K+ on the vestibular evoked potential in the guinea pig.

To investigate the effect on the functioning of the vestibular system of a rupture of Reissner's membrane, artificial endolymph was injected in scala media of ten guinea pigs and vestibular evoked potentials (VsEPs), evoked by vertical acceleration pulses, were measured. Directly after injection of a sufficient volume to cause rupture, all ears showed a complete disappearance of VsEP, followed by partial recovery. To investigate the effect of perilymphatic potassium concentration on the vestibular sensory and neural structures, different concentrations of KCl were injected directly into the ve

Reparative granuloma related to perilymphatic fistula.

Reparative granuloma is defined as an 'exaggeration of the normal reparative process' (Schuknecht) after stapes surgery, often resulting in a destruction of the labyrinth. It is ascribed to a foreign body reaction, yet there are insufficient histological data to prove this and although rare, the problem is still with us. We encountered two patients in whom the history contains elements evoking a perilymphatic fistula. Cerebrospinal fluid causes irritation of the middle ear mucosa. This is demonstrated in an exemplary way in case of a spontaneously developed cerebrospinal fluid leak from a defe

[Leakage of cerebrospinal fluid. Diagnosis and treatment].

Leakage of cerebrospinal fluid may occur after fracture of the skull base or after surgery on the skull base, nose, or paranasal sinuses. Further causes of cerebrospinal fluid leakage are tumours and malformations. It may also occur spontaneously. We describe four case reports illustrating diagnostic and surgical problems. Cerebrospinal fluid oto- and rhinorrhoea should be suspected in cases of recurring purulent meningitis. Immunologic demonstration of beta(2)-transferrin in secretions from the nose and ear is a diagnostic sign of cerebrospinal fluid or perilymph. Digital subtraction cisterno

Management of the open labyrinth.

Labyrinthine fistulas occur in approximately 5% of cholesteatoma cases, but the management of this difficult problem remains controversial. This study assessed the preoperative presentation and outcome in 37 patients operated on for cholestatoma complicated by labyrinthine fistula. Therapy involved removing the matrix from each fistula and reconstructing the bony wall of the labyrinth with bone dust, fibrin glue, and perichondrium. Corticosteroids were added to the management protocol in more recent cases. A fistula classification scheme was introduced to standardize the reporting of the exten

[Mechanics of the ear ossicles in static pressures. II. Impaired joint function and surgical reconstruction of the ossicle chain].

Following the presentation of ossicle chain mechanics in the normal ear, the air pressure-induced movements of the ossicles with artificially ankylosed incudomalleal joints were studied in the same static temporal bone-experiments. In this case, exaggerated inward-outward excursions also prevail at the incus and stapes. This isodirectional motion, first described by Helmholtz, and on which is based our concept of middle-ear function until now, can also be seen in insufficiently moistened temporal bone preparations. It is not inconceivable that former studies were conducted with insufficiently

Combined effects of noise and kanamycin. Cochlear pathology and pharmacology.

Concurrent administration of aminoglycoside antibiotics and acoustic exposure is reported to result in a cochlear damage level exceeding that predicted from the effects of either agent given alone. To investigate this interaction, guinea pigs were given daily subcutaneous injections of kanamycin sulfate at 200, 300, or 400 mg/kg followed by a ten-hour noise exposure (115 or 45 dB) for seven consecutive days. Drug levels in plasma and perilymph were measured during the 24 hours after injection and during the seven-day period of drug and noise exposures. Electrophysiological and morphological ev
