Human umbilical cord blood-derived CD34-positive endothelial progenitor cells stimulate osteoblastic differentiation of cultured human periosteal-derived osteoblasts.

The aim of this study was to examine the effects of human umbilical cord blood-derived CD34-positive endothelial progenitor cells (CD34+ EPCs) on osteoblastic differentiation of cultured human periosteal-derived osteoblasts (POs). CD34+ cells from human umbilical cord blood were sorted to purify more EPCs in characterization. These sorted cells showed CD31, VE-cadherin, and KDR expression as well as CD34 expression and formed typical tubes in Matrigel. These sorted cells were referred to as human cord blood-derived CD34+ EPCs. In in vivo bone formation using a miniature pig model, the newly fo

Ultrasound-Guided Muscle Enhancement and Support Therapy (MEST): Protocol Standardization and a Pilot Study in Refractory Chronic Pain After Total Knee Arthroplasty.

PURPOSE: Muscle enhancement and support therapy (MEST) using bidirectional barbed polydioxanone filaments has been introduced for knee osteoarthritis, but the original technique relies on surface anatomical landmarks without imaging guidance. This study aimed to establish a standardized ultrasound-guided protocol for MEST targeting the vastus medialis obliquus (VMO) and report preliminary clinical outcomes in patients with refractory chronic knee pain following total knee arthroplasty (TKA). PATIENTS AND METHODS: This single-center retrospective pilot study included three patients (five treat

In Vitro and In Vivo Evaluation of a Composite Electrospun Matrix as a Dermal Scaffold: Cell Behavior and Full-Thickness Wound Repair.

An effective dermal scaffold is expected to provide a three-dimensional porous architecture that supports fibroblast adhesion, infiltration, and proliferation; evolve structurally to meet the progressive physiological needs of the entire healing process; and absorb without adverse tissue response during tissue repair. This study evaluates a fully synthetic absorbable composite electrospun matrix comprising three polymer components within a single fiber network, each contributing a distinct yet complementary function. Poloxamer 188 provides immediate wettability and conformability; PLGA undergo

Biomechanical comparison of bone staple fixation methods with suture material for median sternotomy closure using 3D-printed bone models.

AIM: To compare the biomechanical properties of three different sternal closure techniques in a 3D-printed bone model of a sternum from a 30-kg dog. METHODS: Median sternotomy was performed on a total of 90 three-dimensional (3D) copies of a polycarbonate (PC) model of a sternum, generated from the CT images of the sternum of a 30-kg German Shepherd dog. Three different methods were used to repair the sternotomies: polydioxanone suture (group PDS, n = 30), stainless steel bone staples (group SS, n = 30), and nitinol bone staples (group NS, n = 30). Each repair method was tested by applying te

Anterior cruciate ligament remnant-preserving and re-tensioning reconstruction: a biomechanical comparison study of three different re-tensioning methods in a porcine model.

BACKGROUND: With the developments in the arthroscopic technique, anterior cruciate ligament (ACL) remnant-preserving reconstruction is gradually gaining attention with respect to improving proprioception and enhancing early revascularization of the graft. To evaluate the mechanical pull-out strength of three different methods for remnant-preserving and re-tensioning reconstruction during ACL reconstruction. METHODS: Twenty-seven fresh knees from mature pigs were used in this study. Each knee was dissected to isolate the femoral attachment of ACL and cut the attachment. An MTS tensile testing

[Pancreatojejunostomy in pancreatic head resection using a resorbable monofilament for internal drainage of the anastomosis - clinical experience and perioperative results].

BACKGROUND: Pancreatic fistulas are relevant in perioperative outcome, morbidity and mortality after pancreatic head resection. We analysed the potential benefit of an internal pancreatic duct draining technique by a resorbable monofilament suture if performing a two-layer duct-to-mucosa pancreatojejunostomy. PATIENTS AND METHODS: From 2006 to 2010, 139 pancreatic head resections were performed in our department (124 pylorus-preserving, 15 Whipple). Indications for surgery were malignancies (n = 97), chronic pancreatitis (n = 24) or others (n = 18). In 64 cases, internal drainage of the pancr

Comparison of skin adhesive and absorbable intracutaneous suture for the implantation of cardiac rhythm devices.

AIMS: Wound healing is a major determent in the post-surgical course of patients (pts) after pacemaker (PM) and implantable cardioverter defibrillator (ICD) implantation. Insufficient closure may lead to serious complications with pocket infections leading to the device's explantation as the worst case scenario. In addition to the different types of suture and suture clips, a novel topical skin adhesive containing 2-octyl-cyanoacrylate is commercially available. METHODS AND RESULTS: Over a period of 18 months, we prospectively assigned all cases of PM, ICD, and loop recorder implants either t

Effectiveness of Absorbable Plates for the Treatment of Nasal Septal Cartilage Fractures.

BACKGROUND: Nasal bone fractures are the most common type of facial fracture. Nasal bone fractures often occur in combination with septal cartilage fractures, because the nasal septal cartilage acts as a vertical strut and provides structural support for the nose and bilateral nasal airway. However, the treatment for nasal septal cartilage fracture remains controversial, and if untreated, nasal septal cartilage fracture can lead to various complications, such as nasal obstruction and posttraumatic nasal and septal deformity. This study aimed to evaluate the effectiveness and safety of our proc

A composite semiresorbable armoured scaffold stabilizes pulmonary autograft after the Ross operation: Mr Ross's dream fulfilled.

OBJECTIVES: Use of resorbable external reinforcement of the pulmonary autograft during the Ross operation has been suggested, but the differential regional potential for dilation of the aorta, mainly regarding the neo-root and the neo-Valsalva sinuses, represents an unresolved issue. Auxetic materials could be useful in preventing dilation given their favorable mechanical properties. We designed a composite semiresorbable armoured bioprosthesis constituted by polydioxanone and expanded polytetrafluoroethylene and evaluated its effectiveness as a pulmonary autograft reinforcement device in an a

Hook plate fixation for acromioclavicular joint separations restores coracoclavicular distance more accurately than PDS augmentation, however presents with a high rate of acromial osteolysis.

INTRODUCTION: Hook plate fixation of acromioclavicular (AC) joint separations carries the disadvantage of compulsory implant removal, occasional implant fatigue and secondary loss of reduction. This study compares the clinical and radiological outcome of a new polyaxial angular stable hook plate (HP) with absorbable polydioxansulfate (PDS) sling. MATERIALS AND METHODS: Between 2002 and 2009, out of a consecutive series of 81 patients with symptomatic Rockwood type V lesions 52 patients received clinical and radiographic follow-up (HP: n = 27; PDS: n = 25). HP patients were prospectively analy

Arthroscopic Reverse Bankart Surgical Repair.

BACKGROUND: Posterior glenoid labrum lesions occur in only 2% to 10% of all cases of shoulder instability, yet these injuries may cause significant shoulder dysfunction in an athletic population. Moreover, these injuries frequently require surgical intervention and present a unique surgical challenge. INDICATIONS: Indications for arthroscopic posterior labral repair include symptomatic posterior labral tears identified on magnetic resonance imaging with or without contrast, that failed nonsurgical management (ie, lifestyle modification, nonsteroidal anti-inflammatory drug, and physical therap

Chordee repair utilizing a novel technique ensuring neurovascular bundle preservation.

Penile chordee, with and without hypospadias, is amenable to surgical correction. The Nesbit technique of dorsal plication of the ventral tunica albuginea is effective in correcting most cases of corporal disproportion. A hazard with this approach is the potential inclusion of the dorsal neurovascular bundle, with resultant erectile and sensory dysfunction. We developed a simple technique using the Freer elevator to isolate the neurovascular bundle prior to plication. This ensures that no injury occurs to the neurovascular bundle during plication. Since 1994, 37 boys with chordee have been rep

Comparison of Wound Outcomes Between Short-Stitch and Long-Stitch Fascial Closure Following Emergency Exploratory Laparotomy: A Prospective Comparative Study.

Background Emergency exploratory laparotomy is associated with a high incidence of postoperative wound complications, including surgical site infection (SSI) and surgical wound dehiscence (SWD). The fascial closure technique, particularly the stitch length and suture length-to-wound length (SL:WL) ratio, plays a crucial role in wound healing. While short-stitch (SS) techniques have demonstrated superiority in elective surgeries, evidence in emergency settings remains limited. Methods This prospective, observational, comparative study was conducted over 18 months at a tertiary care teaching hos

The Lasso Suture: Ex Vivo Testing of an Alternative to the High-Tension Deep Dermal Stitch.

SUMMARY: Wound dehiscence, with an estimated occurrence rate greater than 4% in plastic surgery, is generally underreported, and can be an indicator of increased mortality and remission rates. The authors developed the lasso suture as a stronger alternative to the current standard patterns. The lasso suture takes less time to perform than the standard high-tension wound repair method. The authors dissected caprine skin specimens to create full-thickness wounds for suture repair using simple interrupted, vertical mattress, horizontal mattress, and deep dermal with running intradermal (DDR) sutu

Failure of coracoclavicular artificial graft reconstructions from repetitive rotation.

PURPOSE: To assess how suture type and suture construct in an augmented Weaver-Dunn reconstruction affect coracoclavicular sling failure and rotary stability. METHODS: Fifteen cadaveric shoulders were tested in rotation about the long axis of the clavicle with 10 lb of simulated arm weight. The clavicle was rotated 50 degrees about its long axis, and the applied torque was recorded. Next, modified Weaver-Dunn reconstruction was conducted. Two types of coracoclavicular sling (opposed drill holes through the clavicle and complete loop around the clavicle) were tested by use of 3 different sutur

VectorLift: Anatomically Guided Thread-Lifting Technique With Reduced Pain and Downtime - Results from a Real-World Non-Randomized Comparative Study.

BACKGROUND: Thread lifting has emerged as a minimally invasive alternative to surgical facial rejuvenation, providing mechanical tissue repositioning with shorter recovery times. Advances in biomaterials and thread design have improved safety and lifting capacity; however, variability in clinical outcomes persists due to differences in vector design, thread type, tissue manipulation, and anesthesia techniques. These factors may influence complication rates, procedural tolerance, and postoperative downtime, contributing to ongoing concerns regarding the predictability and reproducibility of thr
