The numbers needed to treat for neurological disorders.

BACKGROUND: Numerous therapeutic interventions have been developed in the neurosciences. Clinicians need summary measures about efficacy of therapies that derive from the best available evidence, and that can be readily extrapolated to clinical practice. The number needed to treat (NNT) is intuitive and clinically applicable. We provide clinicians with a single source that summarizes important therapies in the main neurological and neurosurgical areas. METHODS: Critically appraised evidence about therapies in the neurosciences was obtained from meta-analyses in all neurosciences groups in the

Safety and feasibility of spinal anesthesia during simple and complex lumbar spine surgery in the extreme elderly (≥80 years of age).

OBJECTIVE: The extreme elderly population (≥80 years of age) is expected to triple globally by 2050 and surgeries in this age group are likely to increase. Spinal anesthesia has emerged as a safe and effective alternative to general anesthesia in lumbar surgery and may particularly benefit extreme elderly patients concerned with post-operative cognitive dysfunction, poor physiological reserves, and polypharmacy. However, literature supporting its use in this population is lacking and there are potential challenges such as degenerative anatomy and medical comorbidities. Here, we assess the safe

Postoperative Thoracic Epidural Analgesia: Adverse Events from a Single-Center Series of 3126 Patients.

PURPOSE: Thoracic epidural analgesia (TEA) has been shown to reduce postsurgical morbidity and mortality; nevertheless, major and minor complications can occur. We report our 10-year experience with TEA and incidence of complications. PATIENTS AND METHODS: Patients received continuous infusion TEA (0.2% ropivacaine and 2 µg ml-1 fentanyl) to control postoperative pain. Every 8 hours, the acute pain service recorded the analgesia regimen and occurrence of side effects. The initial infusion rate was tapered daily in response to improvement in pain symptoms or occurrence of side effects. RESULT

Ultrasound assisted lumbar intrathecal administration of nusinersen in adult patients with spinal muscular atrophy: A case series.

INTRODUCTION/AIMS: Intrathecal nusinersen administration can be challenging in certain adult spinal muscular atrophy (SMA) patients with difficult spinal anatomy who require imaging techniques (fluoroscopy or computed tomography scans) or invasive approaches (catheter placement, laminotomy) to identify the intrathecal space. We used ultrasound (US) assistance to access the lumbar intrathecal space in patients with SMA who experienced previous difficulties or failures with intrathecal dosing. METHODS: Eighteen adult patients with difficult spines were enrolled. We used US assistance, and we re

Continuous Spinal Anesthesia in Frail Patients Undergoing Orthopedic Hip and Knee Revision Surgery: Advantages, Indications, and Risk Management-A Single-Center Retrospective Experience.

Background: Frail patients undergoing hip and knee revision surgery represent a major anesthetic challenge because of advanced age and multiple comorbidities. Continuous spinal anesthesia (CSA) with titrated low-dose levobupivacaine may offer a potentially useful alternative to general anesthesia or single-shot spinal anesthesia in this high-risk population. Methods: A retrospective review was conducted of ASA II-III patients who underwent complex hip and knee revision surgeries between February and October 2024 under CSA. The technique was performed using a 25-gauge spinal catheter with incre

Seizure after IDDS implantation - unrecognized intracranial hypertension associated with intracranial metastases: A case report.

RATIONALE: The external intrathecal drug delivery system (IDDS) provides targeted analgesia and serves as an effective therapy for refractory cancer pain. Its known complications include post-dural puncture headache, device malfunction, granuloma formation, and others. Seizures induced by occult intracranial hypertension following IDDS implantation have rarely been reported. This case report describes a cancer-pain patient who developed seizures after IDDS implantation. PATIENT CONCERNS: A 66-year-old female with metastatic rectal cancer suffered from refractory lumbosacral and lower-extremit

A pilot study of novel ultrahigh-frequency dorsal root ganglia stimulation for chronic lower limb pain: Focusing on safety and feasibility.

OBJECTIVES: This single-arm, open-label, single-center observational pilot study assessed the safety and efficacy of ultrahigh-frequency dorsal root ganglia (UHF-DRG) stimulation in patients with chronic leg pain with or without low back pain. Such high-frequency electrostimulation had not been conducted in the human central nervous system previously. MATERIALS AND METHODS: The primary objective was to evaluate the safety of UHF-DRG stimulation (2 Hz pulses with 50 msec pulse-width and 500 kHz intrapulse sine waves, 5-min duration per stimulation) by identifying incident adverse events (AE) a

Single-dose spinal anaesthesia versus incremental dosing for lower limb vascular surgery.

BACKGROUND: In patients scheduled for vascular surgery, atherosclerotic disease is highly prevalent. Haemodynamic reactions are often aggravated when spinal analgesia is used in this population. No randomized studies have been conducted comparing single shot (SS) with continuous spinal analgesia (CSA) for vascular patients. We did a prospective randomized study comparing haemodynamics when SS versus CSA was performed. METHODS: Sixty patients were randomized to have SS or CSA. The power of the study was 0.90. Patients in the SS group received 17.5 mg bupivacaine and in the CSA group 5 mg was g

Intrathecal pain treatment for severe pain in patients with terminal cancer: A retrospective analysis of treatment-related complications and side effects.

OBJECTIVES: Two-thirds of patients with advanced cancer experience pain. Some of these patients have severe pain refractory to oral and parenteral medication, for whom intrathecal pain treatment could be an option. While intrathecal therapy is presently used with good results in clinical practice, the current evidence is limited. Hence, increased knowledge of intrathecal pain treatment is needed. This retrospective study aimed to assess complications and side effects related to intrathecal pain treatment in patients with terminal cancer. METHODS: A retrospective study on all patients who rece

Progressive systemic sclerosis: intrathecal pain management.

BACKGROUND AND OBJECTIVES: At present, there is no reliable method for long-term treatment of severe pain from progressive systemic sclerosis (PSS) associated with Raynaud's phenomena leading to ischemia and ulcerations of the extremities. Long-term continuous intrathecal (IT) buprenorphine/bupivacaine analgesia was used in such a case. METHODS: The patient was a 71-year-old woman in whom conservative treatment, including opioids, dorsal column stimulation, and epidural bupivacaine, had failed to provide satisfactory, long-term relief for her severe lower extremity pain. An 18-gauge Portex in

[The incidence of post spinal headache in a group of young patients].

Post-dural puncture headache (PDPH) is a significant and well-known complication of procedures that perforate the dura mater, e.g., clinical spinal anaesthesia (SpA). The exact mechanisms leading to PDPH are still not completely understood, although several factors, particularly the patient's age, modulate the incidence. In young patients (20-30 years) previous studies reported high occurrences of PDPH in 6%-16% of cases [5, 8], decreasing the value and acceptance of SpA in this patient group. This study was undertaken to reevaluate under reproducible study conditions the incidence of PDPH and

Safety and efficacy of intrathecal baclofen trials for the treatment of hypertonia: a retrospective cohort study.

OBJECTIVE: Intrathecal baclofen (ITB) is an effective treatment for refractory hypertonia in children. ITB has long been effective for the treatment of spasticity, and indications have naturally evolved to include dystonia and mixed pediatric movement disorders (PMDs). The established uses for ITB trials are insurance prerequisite, mixed tone, and family request. Despite agreement for ITB therapy by a multidisciplinary group of subspecialists in a complex PMD program, insurance companies often require an ITB trial be performed. A longitudinal cohort was identified to determine the safety and e
