Physician perspectives on chronic pain management: barriers and the use of eHealth in the COVID-19 era.

BACKGROUND: Chronic pain is a highly prevalent and disabling condition which is often undertreated and poorly managed in the community. The emergence of COVID-19 has further complicated pain care, with an increased prevalence of chronic pain and mental health comorbidities, and burnout among physicians. While the pandemic has led to a dramatic increase in virtual health care visits, the uptake of a broader range of eHealth technologies remains unclear. The present study sought to better understand physicians' current needs and barriers in providing effective pain care within the context of COV

Printed educational materials directed at Ontario family physicians do not improve adherence to guideline recommendations for diabetes management: a pragmatic, factorial, cluster randomized controlled trial [ISRCTN72772651].

BACKGROUND: Printed educational materials (PEMs) have long been used to inform clinicians on evidence-based practices. However, the evidence for their effects on patient care and outcomes is unclear. In Ontario, despite widely available clinical practice guidelines recommending antihypertensives and cholesterol-lowering agents for patients with diabetes, prescriptions remain low. We aimed to determine whether PEMs can influence physicians to intensify prescribing of these medications. METHODS: A pragmatic, 2 × 2 factorial, cluster randomized controlled trial was designed to ascertain the effe

BRAZILIAN PHYSICIANS' PRACTICES ON THE MANAGEMENT OF SYMPTOMS SUGGESTING GASTROESOPHAGEAL REFLUX DISEASE: A MULTIDISCIPLINARY SURVEY.

BACKGROUND: Clinical guidelines are available to steer decisions regarding diagnosis, management and treatment of gastrointestinal disorders. Despite this, variations in physician's practices regarding gastroesophageal reflux disease (GERD) symptoms are well described in the literature. OBJECTIVE: To describe practices of physicians from different specialties on the management of patients with typical symptoms of GERD (heartburn and regurgitation) in a Brazilian sample. METHODS: National online survey enrolling a sample of general practitioners, gastroenterologists, cardiologists and otolary

Primary care physician practices in the diagnosis, treatment and management of men with chronic prostatitis/chronic pelvic pain syndrome.

To describe practice patterns of primary care physicians (PCPs) for the diagnosis, treatment and management of chronic prostatitis/chronic pelvic pain syndrome (CP/CPPS), we surveyed 556 PCPs in Boston, Chicago, and Los Angeles (RR=52%). Only 62% reported ever seeing a patient like the one described in the vignette. In all, 16% were 'not at all' familiar with CP/CPPS, and 48% were 'not at all' familiar with the National Institutes of Health classification scheme. PCPs reported practice patterns regarding CP/CPPS, which are not supported by evidence. Although studies suggest that CP/CPPS is com

As mammography use increases, are some providers omitting clinical breast examination?

OBJECTIVE: To explore use of clinical breast examination (CBE) among women receiving mammography. METHODS: A retrospective cohort analysis of 100 women aged 50 years or older with at least one bilateral mammogram. Chart review documented demographic information, severity of illness, and performance of CBE (from 1 year prior to 18 months after the mammogram). RESULTS: The mean age of the 100 women was 63 years. They were predominantly unmarried (60%), nonwhite (58%), and not currently employed (57%). Three quarters (76%) had mammography and CBE (comprehensive screening), while the remaining 2

Primary care doctors' management behavior with respect to epilepsy in Kerala, southern India.

Although a majority of persons with epilepsy in developing countries are diagnosed, treated, and followed up by primary care doctors, few efforts have been made to examine their understanding with respect to epilepsy management. Through a questionnaire survey, we gathered information about the epilepsy management behavior of 500 primary care doctors distributed across the south Indian state of Kerala. Very few of them ever had diagnosed focal seizures, and the majority of them overutilize EEGs, prescribe continuous antiepileptic drug (AED) prophylaxis for febrile convulsions, use relatively ex

Knowledge, attitudes, and practice of abortion among women and doctors in Armenia.

OBJECTIVE: To assess knowledge and attitudes towards abortion among women and obstetricians/gynaecologists in Armenia, in order to facilitate the introduction of medical abortion. METHODS: Ninety-nine doctors and 400 women in three cities in Armenia were purposively selected to participate in face-to-face interviews using structured survey instruments. RESULTS: Most women knew that abortion is legal under any (31%) or certain (50%) circumstances. The majority of women had heard of medical abortion, but had superficial or incorrect knowledge of the method. Thirty percent of women expressed a

Strategies for managing uncertainty and complexity.

OBJECTIVES: To identify strategies involved in the diagnosis and treatment plans of primary care problems that are uncertain and complex. METHODS: In this exploratory study we observed primary care physicians encountering standardized patients who portrayed typical primary care problems involving uncertainty and complexity. First, we analyzed 10 tapes of nine physicians with a range of clinical experience (first-year residents through faculty physicians) interacting with four standardized patient cases (headache, back pain, hypertension, and abdominal pain). We analyzed the 10 tapes to determ

An ethnography of chronic pain management in primary care: The social organization of physicians' work in the midst of the opioid crisis.

BACKGROUND: This study reports on physicians' experiences with chronic pain management. For over a decade prescription opioids have been a primary treatment for chronic pain in North America. However, the current opioid epidemic has complicated long-standing practices for chronic pain management which historically involved prescribing pain medication. Caring for patients with chronic pain occurs within a context in which a growing proportion of patients suffer from chronic rather than acute conditions alongside rising social inequities. METHODS: Our team undertook an ethnographic approach kno

Risks and responsibilities in prescribing opioids for chronic noncancer pain, part 2: best practices.

Opioids are increasingly prescribed to provide effective therapy for chronic noncancer pain, but increased use also means an increased risk of abuse. Primary care physicians treating patients with chronic noncancer pain are concerned about adverse events and risk of abuse and dependence associated with opioids, yet many prescribers do not follow established guidelines for the use of these agents, either through unawareness or in the mistaken belief that urine toxicology testing is all that is needed to monitor compliance and thwart abuse. Although there is no foolproof way to identify an abuse

Repeat prescribing policy in New Zealand general practice: making it better.

INTRODUCTION Repeat prescribing is common in New Zealand general practice. Research also suggests that repeat prescribing is a process prone to error. All New Zealand general practices have to comply with requirements to have a repeat prescribing policy, with the details of the policy to be designed by the practice. AIM To inform the development of practice policy, research was undertaken with experienced general practitioners to identify and mitigate risk in the process. METHODS At the 2019 annual conference of the Royal New Zealand College of General Practitioners, a workshop was held with 5

[Revision of the Dutch College of General Practitioners practice guideline 'Children with fever'].

- Fever in children is usually caused by benign viral infection.- Approximately 1-10% of children with fever has or develops a serious infection such as pneumonia, meningitis or pyelonephritis.- It is more important to timely recognize symptoms that may indicate a potentially serious disease course than to make an exact diagnosis.- Children < 3 months are at greater risk for a serious disease course.- During follow-up it is important to provide parents with proper advice on the expected disease course and who they should contact, and when, in cases of deterioration or concern.- Paracetamol or

[Summary of the NHG guideline 'Abdominal pain in children', the 100th NHG guideline].

The Dutch College of General Practitioners (NHG) guideline 'Abdominal pain in children' was published in September 2012. The guideline distinguishes between children with acute abdominal pain (≤ 1 week) and chronic abdominal pain (> 1 week). Abdominal pain can be caused by an underlying somatic condition or can have a functional nature. The chance of finding a somatic cause is far higher in children with acute abdominal pain than in children with chronic abdominal pain. In children with acute pain the emphasis is on diagnosis, whereas in children with chronic pain it is on treatment. Additiona

Financial factors shaping treatment decisions in oncology: A systematic review.

PURPOSE: The rising cost of cancer care has made financial considerations increasingly prominent in oncology practice. Although patient financial toxicity is well studied, how cost-related factors influence physician treatment decision-making is less clearly understood. This systematic review evaluates how various financial factors shape prescribing and treatment decisions in oncology. METHODS: A comprehensive literature search was conducted in MEDLINE, Embase, and Web of Science databases. Eligible studies evaluated associations between financial exposures (e.g., drug pricing, reimbursement,

Testosterone therapy and lawsuits against prescribers: a legal case review from 2000 to 2024.

BACKGROUND: Testosterone therapy (TT) is widely prescribed for hypogonadism. It leads to symptomatic improvement but as with any medication carries risks, as outlined in US Food and Drug Administration (FDA) labels. AIM: The current study analyzes TT lawsuits against providers to identify litigation patterns, adverse outcomes, and their implications, informing clinical practices for improved patient safety and reduced legal risk. METHODS: We conducted a comprehensive search of the Westlaw and LexisNexis legal databases, covering cases from 2000 to 2024, using the keywords "testosterone" comb

Changes in Vasectomy Practice Patterns After Dobbs: A Multi-Institutional Study.

INTRODUCTION: Following the Dobbs decision overruling Roe v Wade, public interest in vasectomies increased. This ruling directly affected urologic practice patterns and warrants further investigation. We aimed to conduct a multi-institutional study quantifying the change in vasectomy practice volume between the pre-Dobbs (PD) and after-Dobbs (AD) eras. METHODS: Multiple geographically distinct US academic medical centers participated in the study. Patients with an initial vasectomy consult between January 1, 2021, and December 31, 2022, were included and categorized as pre-Dobbs (January 1, 2
