Pleural abnormalities: thoracic ultrasound to the rescue!

Diaphragmatic hernias that are diagnosed in adulthood may be traumatic or congenital in nature. Therefore, respiratory specialists need to be aware of the presentation of patients with these conditions. In this report, we describe a case series of patients with congenital and traumatic diaphragmatic hernias and highlight a varied range of their presentations. Abnormalities were noted in the thorax on the chest radiographs, but it was unclear as to the nature of the anomaly. The findings on thoracic ultrasound conducted by a pulmonologist helped to direct appropriate investigations avoiding unn

Respiratory complications of poliomyelitis and multiple sclerosis.

Years after suffering from poliomyelitis and completely recuperating from any of its acute complications, patients can develop postpolio syndrome (PPS), thought to be caused by progressive neuronal loss. The symptoms and manifestations of PPS vary in intensity and distribution, but many patients have respiratory involvement and will require a pulmonary evaluation for new or increased dyspnea, fatigue, or symptoms suggestive of sleep-related disordered breathing. The diagnosis, evaluation, and management of this complex problem are discussed in the first part of this article. In the second part

Self-hypnosis for management of chronic dyspnea in pediatric patients.

INTRODUCTION: Hypnotherapy can be useful in the management of anxiety, discomfort, and psychosomatic symptoms, all of which may contribute to a complaint of dyspnea. Therefore, instruction in self-hypnosis was offered to 17 children and adolescents with chronic dyspnea, which had not resolved despite medical therapy, and who were documented to have normal lung function at rest. This report documents the result of this intervention. METHODS: A retrospective chart review identified all patients followed by a single pediatric pulmonologist (R.D.A.), with a chief complaint of chronic dyspnea from

Pulmonary health is a key aspect of modern medicine, covering all age groups, from neonatology to gerontology. The article addresses the impact of genetic factors, environment, and human lifestyle on the development of respiratory diseases. Particular attention is paid to the role of genetic predisposition, embryonic disorders, and external factors such as air pollutants and tobacco smoking in the development of chronic lung diseases. Data on the structural and functional development of the respiratory system, its age-related changes, and the risks associated with the involution of the lung ti

Intubated or rigid bronchoscopy under high-flow oxygenation and deep sedation without ventilator or anesthesiologist: A report on a new technique.

As the demand for advanced bronchoscopic procedures increases, prolonged sedation with adequate oxygenation has become essential. Traditionally, these procedures require an anesthesiologist to provide (positive-pressure or jet) ventilator support. However, recent innovations have enabled advanced bronchoscopy under high-flow endotracheal oxygenation and deep sedation without these requirements. Following oropharyngeal lidocaine anaesthesia, deep sedation was induced using fentanyl and remimazolam. Thereafter lidocaine was instilled into the larynx, trachea, and main bronchi using a flexible br

Pleural fluid biochemical analysis: the past, present and future.

Identifying the cause of pleural effusion is challenging for pulmonologists. Imaging, biopsy, microbiology and biochemical analyses are routinely used for diagnosing pleural effusion. Among these diagnostic tools, biochemical analyses are promising because they have the advantages of low cost, minimal invasiveness, observer independence and short turn-around time. Here, we reviewed the past, present and future of pleural fluid biochemical analysis. We reviewed the history of Light's criteria and its modifications and the current status of biomarkers for heart failure, malignant pleural effusio

Diagnosis of lung adenocarcinoma with left adrenal metastasis via transesophageal endoscopic ultrasound-guided fine-needle aspiration biopsy: A case report.

Lung cancer is generally diagnosed through traditional bronchoscopy by pulmonologists, whereas data on the application of transesophageal endoscopic ultrasound-guided fine-needle aspiration (EUS-FNA) for lung cancer diagnosis in China are very rarely reported. We herein describe a case of a 62-year-old woman who presented with non-productive cough and was found to have a pulmonary mass in the upper lobe of the right lung. The results of positron emission tomography revealed an elevated level of 18F-fluorodeoxyglucose accumulation (maximum standardized uptake value of 13.6) in the left adrenal

A novel online education module to teach clinicians how to correctly identify ultrasonographic features of mediastinal lymph nodes during endobronchial ultrasound.

BACKGROUND: Ultrasonographic features can be used to predict mediastinal lymph node malignancy during endobronchial ultrasonography. Despite the validity of using these features for this purpose, the features are not being widely used in clinical practice. This may be attributable to the absence of educational programs that teach clinicians how to identify the features. To address this knowledge gap, we developed an online educational module to teach clinicians how to correctly interpret ultrasonographic features. METHODS: The module was designed using corrective feedback and test-enhanced le

A Rare Case of Sudden Fatal Airway Obstruction Caused by an Undiagnosed Laryngeal Lesion.

Mechanical asphyxia is a severe condition caused by a physical blockage that impedes breathing in the upper airways, trachea, and lungs. We present a case of a 39-year-old man who died suddenly at home while getting ready for work. He had previously experienced shortness of breath and a sore throat. Despite consulting a pulmonologist, who ruled out lung diseases, no other specialist evaluations were conducted. We performed a comprehensive forensic investigation, including collecting medical history and criminological data, a forensic autopsy, and subsequent toxicological and histological analy

[Managment of tuberculosis in an University of Campania].

Tuberculosis (TBC) is an infectious disease with the highest mortality and morbidity by single pathogen, affecting about one third of worldwide population. Although Mantoux test is the most used, IGRA (Interferon-gamma Release Assays) tests seem to give good results for presumptive diagnosis of active or latent tuberculosis. From June 2011 to June 2012 we made about 1,000 visits for TBC prevention among the exposed to biological risks of our University. The management of suspected latent or active tuberculosis infection was carried out in collaboration with the pulmonologist, assessing the ris

Diagnostic assessment of patients with interstitial lung disease.

The diagnosis of interstitial lung disease (ILD) is frequently delayed because clinical clues are neglected and respiratory symptoms are ascribed to more common pulmonary diagnoses such as chronic obstructive pulmonary disease (COPD) in the primary care setting. While ILD cases ultimately require referral to a pulmonologist, general practitioners can play a crucial role in recognising the need for, and initiating, a diagnostic evaluation. An initial assessment hinges upon a structured history and physical examination with careful attention paid to occupational, environmental and drug exposures

Agreement on radiological diagnosis of acute lower respiratory tract infection in children.

Chest radiographs are often used to support the decision to use antibiotics in children aged <5 years with moderate to severe lower acute respiratory infection (ARI). This study aimed to evaluate inter-observer agreement in the interpretation of chest radiographs of children with suspected lower ARI. Three experienced paediatric sub-specialists: a radiologist, an intensivist and a pulmonologist were provided with basic clinical information on each of 48 cases, but the individual standardized evaluations were blinded for clinical diagnoses and for the assessment by the other two specialists. Flexible bronchoscopic argon plasma coagulation for management of massive hemoptysis in bronchial Dieulafoy's disease.

Dieulafoy's disease is an uncommon condition, the usual site of occurrence being the gastrointestinal tract. The condition refers to the presence of a dysplastic submucosal artery with mucosal vascular branches that has propensity to cause recurrent bleeding. Dieulafoy's disease of the bronchus is rare. Herein, we describe the case of a 26-year-old male who presented with recurrent bouts of hemoptysis and bronchial Dieulafoy's disease was diagnosed. Flexible bronchoscopy was performed, and argon plasma coagulation (APC) of the bleeding lesion was done. The procedure was successful and was foll

Chest CT Signs in Pulmonary Disease: A Pictorial Review.

CT scanning of the chest is one of the most important imaging modalities available to a pulmonologist. The advent of high-resolution CT scanning of the chest has led to its increasing use. Although chest radiographs are still useful as an initial test, their utility is limited in the diagnosis of lung diseases that depend on higher resolution images such as interstitial lung diseases and pulmonary vascular diseases. Several metaphoric chest CT scan signs have been described linking abnormal imaging patterns to lung diseases. Some of these are specific to a disease, whereas others help narrow t

The utility of a smartphone electronic referral application in facilitating telemedicine in pulmonology.

BACKGROUND: A streamlined referral system for respiratory diseases is essential, and telemedicine offers a viable solution, particularly within the public healthcare sector. There is a paucity of data on the utility of the Vula medical referral application in pulmonology, a field that lends itself to telemedicine. OBJECTIVES: To perform a 1-year retrospective observational study to assess the Vula medical referral application utility in a tertiary referral centre in Cape Town, South Africa (SA). METHODS: This retrospective study was conducted at a large tertiary-level hospital located in Cap

An Investigation of the Pediatric Rigid Bronchoscopy Complication with Three Different Anesthesia Regimes.

BACKGROUND: Foreign body aspiration is common in children and poses a significant risk of morbidity and mortality. Rigid bronchoscopy is the most common method for removing aspirated foreign bodies. OBJECTIVES: Anesthesiologists play a critical role in managing these procedures, aiming to find the best strategies with the fewest complications. This study aims to compare anesthesia-related complications during rigid bronchoscopy in children using muscle relaxants versus no muscle relaxants. METHODS: In this clinical trial, 60 eligible children were randomly divided into three equal groups: SP

Influenza vaccination in children with pulmonary disease during the COVID-19 pandemic.

INTRODUCTION: Influenza is associated with considerable respiratory morbidity and mortality. Healthcare authorities recommend immunization of all children as vaccinations protect vulnerable populations, minimize influenza triggered asthma attacks, and reduced the burden of respiratory illnesses during the SARS-CoV-2 pandemic. Medical professionals should counsel parents of children with chronic lung disease to receive annual influenza vaccinations as part of supportive care. We aimed to describe adherence to influenza vaccination in respiratory patients and identify potential reasons for non-v

Contribution of four common pulmonary function tests to diagnosis of patients with respiratory symptoms: a prospective cohort study.

BACKGROUND: Few studies of the diagnostic value of pulmonary function testing are available. We assessed the diagnostic contribution of four basic pulmonary function tests: spirometry, lung volume, airway resistance, and diffusing capacity. METHODS: In this prospective cohort study, we enrolled patients presenting to a pulmonologist with respiratory symptoms but no clear diagnosis from 33 hospitals in Belgium. Each patient had spirometry, lung volume, airway resistance, and diffusing capacity testing and all other tests necessary for a definitive diagnosis. Clinical history and pulmonary func
