The authors report a series of 13 horseshoe kidneys observed over a 22-year period. All patients are men with a mean age of 39 years (20-65 years). The clinical features were dominated by abdomino-lumbar pain (nine cases). The horseshoe kidney was associated with renal stones in nine cases, uretero-pelvic junction syndrome in five cases and pyonephrosis in one case. The specific anatomic and surgical features of this rare malformation are emphasized and the therapeutic features of each uropathy associated with horseshoe kidney are discussed.

On the basis of 6 cases of pyonephrosis due to renal stones successfully treated without surgery over a period of one year, the authors define their current therapeutic approach which consists of 2 phases: --1st phase: salvage of the kidney by ultrasound guided percutaneous nephrostomy under local anaesthesia. No hasty endoscopic procedures in an attempt to insert a ureteric catheter into the kidney; --2nd phase: elimination of the obstructing stone (on average, 8 days after the 1st phase), by percutaneous nephrolithotomy, descending ureteroscopy or ascending ureteroscopy.

A young man presented with a history of uncorrected ureteropelvic junction obstruction 18 months in duration and clinical acute pyelonephritis. Retrograde pyelography showed unilateral pyonephrosis and cultures of purulent drainage proximal to the ureteropelvic junction revealed predominantly anaerobic bacteria and no aerobic gram-negative bacilli. Subsequent nephrectomy established a diagnosis of xanthogranulomatous pyelonephritis. Nephrectomy in combination with intensive antimicrobial therapy resulted in recovery. The contribution of anaerobic bacteria in the pathogenesis of xanthogranuloma

OBJECTIVES: To review the feasibility and outcome of retroperitoneoscopic nephrectomy (RPN) for pyonephrotic nonfunctioning kidneys.
METHODS: RPN for pyonephrotic nonfunctioning kidneys was used in 52 patients from July 2001 to May 2007. Percutaneous nephrostomy drainage was instituted in 46 patients because of sepsis before being referred to our institute. However, the remaining 6 patients underwent RPN without previous diversion. The mean patient age was 46.4 years (range 22-72). The etiology was stone disease in 29 patients, ureteropelvic junction obstruction in 18, and genitourinary tuber

Peritoneal fistulization of pyonephrosis is an uncommon condition, leading to a generalized peritonitis, considered as extreme life-threatening emergency. Secondary to an obstructive uropathy, increasing intra-renal pressure. In most cases, ureteral stones represent the underlying causes followed by stenosis, tumor, or connective tissue disease. Through this case report, we present a 60-year-old patient with a history of urolithiasis, admitted for an atypical instance of spontaneous renal pelvis rupture in an ectopic right kidney managed surgically. This case emphasizes the importance of consi

Nephrobronchial fistula is a rare complication seen in association with renal infections, trauma or stone disease. Xanthogranulomatous pyelonephritis (XGP) is an infectious disease with a potential for fistulization to lung, skin, colon and other organs. We present a case of nephrolithiasis complicated by obstruction leading to pyonephrosis and nephrobronchial fistula, treated successfully by nephrectomy and excision of fistulous tracts. Nephrobronchial fistula, although a rare complication of longstanding renal stone, should be considered when a patient presents with perirenal suppurative pro

Damages to the psoas muscle are most often of infectious origin; other causes are observed, notably neoplasia and metastasis, hematomas, which sometimes pose a problem of care, especially in tumor-related conditions. Here, we report a rare case of psoas abscess associated with left pyonephrosis, revealing a squamous cell carcinoma of the left psoas without other tumor sites, along with the diagnostic and therapeutic approach taken in this case. This case report will make clinicians think about psoas muscle lesions, especially tumor lesions, which require a well-defined therapeutic approach.

A 73-year-old woman was referred to our hospital for treatment of a 12 mm stone in the right ureteropelvic junction and pyonephrosis. A double-J ureteral stent was indwelled for right hydronephrosis and emphysematous pyelonephritis. After antibiotic treatment, flexible transurethral lithotripsy (f-TUL) was performed and all the stones were disintegrated and extracted without any complications. On the day after f-TUL, severe back pain occurred, and computed tomography revealed subcapsular hematoma and renal pseudoaneurysm. Angiography and selective embolization of the bleeding artery were perfo

Fistula formation between the kidney and the colon is a rare occurrence. Colorenal fistulas have been reported after renal cryoablation, calculous pyonephrosis, and renal cell carcinoma. Fistula formation is reported in as many as 35% of patients with Crohn's disease. Crohn's-related urinary fistulas may include enterovesical, enteroureteral, rectourethral, urethrocutaneous, and entero-urachal fistulas. Here, we report a rare case of a patient who was found to have a left colorenal fistula in the setting of a left staghorn calculus and recurrent urinary tract infections with a question about t

A 56-year-old woman underwent laparoscopic right nephrectomy due to pyonephrosis associated with right ureteral stones. Moreover, the patient developed a brain stem hemorrhage and became bedridden. At the time of nephrectomy, a renal tumor, with a size of 24 × 24 × 20 mm, was observed in the left renal hilum; the tumor did not show contrast enhancement on computed tomography. After 3 years, the tumor gradually grew to a size of 45 × 35 × 34 mm, and therefore, laparoscopic non-clamping tumor enucleation was performed. Pathological examination confirmed a diagnosis of renal schwannoma.

Nephrectomy after pyonephrosis, repeated acute pyelonephritis or chronic pyelonephritis is a challenge for any surgeon, owing to adhesions around the kidney. We encountered an unusual case of post-nephrectomy urinary fistula, as a complication of subcapsular nephrectomy. This occurred as a result of residual renal tissue after nephrectomy, which was subsequently excised using methylene blue as an aid to ensure complete excision. Such a complication has never been reported in existing literature. We reviewed the literature for any such related complications to gather an insight to its occurrenc

Squamous cell carcinoma of the renal pelvis is a rare tumour, mainly occurring in elderly patients with neglected and secondarily infected pyelocaliceal stones. It has a poor prognosis. The authors report the case of a 74-year old man with a destroyed and painful kidney containing a staghorn calculus for 30 years. The preoperative diagnosis was pyonephrosis, but nephrectomy, justified by the symptoms, showed that the kidney was invaded by a squamous cell carcinoma of the renal pelvis invading the psoas. The patient died 1 month after the operation. Treatment is essentially preventive by remova

A 61-year-old woman visited our hospital complaining of right flank pain and fever. The radiograph demonstrated multiple renal calculi. Radio renography showed no uptake in the right kidney. Therefore, we diagnosed her with pyonephrosis, and recommended open nephrectomy. However, she selected the conservative treatment with extra corporeal shockwave lithotripsy (ESWL). In spite of disappearance of multiple calculi, pyuria continued for 3 months after ESWL. Retrograde pyelography showed a fistula from the right pelvis into the duodenum. The patient was successfully treated by nephrectomy and du

The current status of the pathogenesis of the disease and imaging methods in acute renal infections is reviewed. The spectrum of type of infection includes 1) acute pyelonephritis, 2) acute bacterial nephritis, 3) acute renal abscess, 4) acute pyonephrosis, and 5) infected renal simple cyst. The two most common imaging methods used are ultrasound and computed tomography (CT). Recent experience from the literature suggests that CT offers some advantages over ultrasound particularly in distinguishing degree and type of involvement of the renal parenchyma and in more accurate diagnosis of acute a

The following has been established in examinations under optimal conditions: 1. Ultrasound and CT scan appear to be of equal value in respect of validity and significance of information on the nature of findings, especially in connection with spaceoccupying processes, hydronephrosis and pyonephrosis. However, CT supplies a better anatomical and topographical overall view of the situation. 2. CT scanning, is definitely superior in respect of significance of information on the extension and behaviour in respect of neighbouring organs. This is particularly evident in the visualisation of relapses

Refractory infectious renal calculi with severe pyonephrosis pose a clinical dilemma due to ineffective drainage and contraindicated percutaneous nephrolithotomy (PCNL). We report a 56-year-old male with diabetes and paraplegia who developed left renal pelvic calculi (2.5 cm) with refractory infection after failed stenting/nephrostomy. Retroperitoneal laparoscopic pyelolithotomy was performed, achieving complete stone removal and pus drainage. The patient recovered uneventfully with no infection recurrence at 3-month follow-up. This procedure is a safe, effective minimally invasive salvage the
