Conditional survival and long-term efficacy with nivolumab plus ipilimumab versus sunitinib in patients with advanced renal cell carcinoma.

BACKGROUND: Conditional survival estimates provide critical prognostic information for patients with advanced renal cell carcinoma (aRCC). Efficacy, safety, and conditional survival outcomes were assessed in CheckMate 214 (ClinicalTrials.gov identifier NCT02231749) with a minimum follow-up of 5 years. METHODS: Patients with untreated aRCC were randomized to receive nivolumab (NIVO) (3 mg/kg) plus ipilimumab (IPI) (1 mg/kg) every 3 weeks for 4 cycles, then either NIVO monotherapy or sunitinib (SUN) (50 mg) daily (four 6-week cycles). Efficacy was assessed in intent-to-treat, International Meta

Hand-Foot Syndrome and Progression-Free Survival in Patients Treated with Sunitinib for Metastatic Clear Cell Renal Cell Carcinoma.

Patients with metastatic clear cell renal cell carcinoma (mRCC) typically receive systemic treatment with tyrosine kinase inhibitors (TKI). Side effects include the hand-foot syndrome (HFS), tiredness, nausea, decreased appetite, diarrhea, myelosuppression, and hypertension. This study seeks to define the relationship between the incidence of HFS after the first cycle of treatment with sunitinib as the first-line treatment for mRCC (50 mg/day, 6-week schedule: 4 weeks on and 2 weeks off) and progression-free survival. We found that patients, treated with sunitinib for mRCC, who did not experie

Phase II Study of Two Weeks on, One Week off Sunitinib Scheduling in Patients With Metastatic Renal Cell Carcinoma.

Purpose Standard frontline treatment of patients with metastatic renal cell carcinoma currently includes sunitinib. A barrier to long-term treatment with sunitinib includes the development of significant adverse effects, including diarrhea, hand-foot syndrome (HFS), and fatigue. This trial assessed the effect of an alternate 2 weeks on, 1 week off (2/1) schedule of sunitinib on toxicity and efficacy in previously untreated patients with metastatic renal cell carcinoma. Methods Patients started with oral administration of 50 mg sunitinib on a 2/1 schedule and underwent schedule and dose alterat

Endothelial SMAD1-MCAM axis facilitates sunitinib resistance and progression of clear cell renal cell carcinoma via LAMB1-ITGB1 signaling.

Clear cell renal cell carcinoma (ccRCC) is characterized by high metastatic potential and frequent resistance to conventional therapies, highlighting the need for a deeper understanding of its tumor microenvironment. Here, we integrated single-cell RNA sequencing and spatial transcriptomics from large ccRCC cohorts to map the cellular landscape of ccRCC. We identified a distinct stromal cell subpopulation, Melanoma Cell Adhesion Molecule, MCAM+ endothelial cells (MCAM+ ECs), which spatially associates with EMT-like tumor cells and promotes disease progression. Mechanistically, our findings sug

High-Dose Sunitinib Accelerates Triple-Negative Breast Cancer Metastasis by Modulating an Immunosuppressive Microenvironment: A Preclinical 4T1 Model Study.

BACKGROUND: Sunitinib (SUN), a multi-targeted anti-angiogenic tyrosine kinase inhibitor, could inhibit tumor growth and affect tumor microenvironment (TME). However, survival benefit for triple-negative breast cancer (TNBC) patients was limited and the drug resistance probably related to its pro-metastatic effect. The association between metastasis progression and SUN dosage, also the underlying mechanism still remains unclear. Herein, we illustrated the biphasic effect of SUN on metastasis attributed to divergent TME conditions modulated by different therapeutic SUN doses. METHODS: 4T1 murin

Early-Onset Posterior Reversible Encephalopathy Syndrome at Reduced-Dose Sunitinib in Metastatic Papillary Renal Cell Carcinoma: A Case Report.

Posterior reversible encephalopathy syndrome (PRES) is a rare but potentially life-threatening neurological complication associated with vascular endothelial growth factor (VEGF)-targeted tyrosine kinase inhibitors. Although it is typically reported after prolonged exposure and at standard or high doses, early onset at reduced dosing may occur and requires prompt recognition. We report a 64-year-old woman with metastatic papillary renal cell carcinoma who developed PRES after two months of treatment with reduced-dose sunitinib (37.5 mg daily). She presented with severe headache, nausea, vomiti

An intercostal artery aneurysm rupture in a patient with neurofibromatosis type 1-associated gastrointestinal stromal tumor during sunitinib therapy.

Neurofibromatosis type 1 (NF1) is associated with an increased risk of gastrointestinal stromal tumors (GISTs). Vasculopathy, such as aneurysms, is a recognized complication of NF1; however, it is little known among gastroenterologists. We report a 65-year-old woman with NF1 who developed hemorrhagic shock due to rupture of an intercostal artery aneurysm during sunitinib therapy. The patient underwent distal gastrectomy for duodenal GIST at age 49, but later developed hepatic and peritoneal recurrence. Because of failure of imatinib therapy and multiple transarterial embolization procedures, s

MiRNA-21 silencing mediated by tumor-targeted nanoparticles combined with sunitinib: A new multimodal gene therapy approach for glioblastoma.

Malignant brain tumors, including glioblastoma (GBM), are among the most lethal human cancers, due to their tremendous invasive capacity and limited therapeutic options. Despite remarkable advances in cancer theranostics, which resulted in significant improvement of clinical outcomes, GBM relapse is very frequent and patient survival remains under one year. The elucidation of the role of abnormally-expressed miRNAs in different steps of GBM pathogenesis and in tumor resistance to therapy paved the way for the development of new miRNA-based therapeutic approaches targeting this disease, aiming

Selective ETA vs. dual ETA/B receptor blockade for the prevention of sunitinib-induced hypertension and albuminuria in WKY rats.

AIMS: Although effective in preventing tumour growth, angiogenesis inhibitors cause off-target effects including cardiovascular toxicity and renal injury, most likely via endothelin (ET)-1 up-regulation. ET-1 via stimulation of the ETA receptor has pro-hypertensive actions whereas stimulation of the ETB receptor can elicit both pro- or anti-hypertensive effects. In this study, our aim was to determine the efficacy of selective ETA vs. dual ETA/B receptor blockade for the prevention of angiogenesis inhibitor-induced hypertension and albuminuria. METHODS AND RESULTS: Male Wistar Kyoto (WKY) rat

[A Long-Term Survivor Who Are Responding to Sunitinib Treatment for Recurrent Imatinib-Resistant Gastrointestinal Stromal Tumor(GIST)of the Stomach].

A 63-year-old man underwent a laparoscopic partial gastrectomy for GIST on September, 2001. Three years and 2 months after the gastrectomy, an abdominal CT showed multiple recurrences in S5 and S6 in the liver and peritoneum. After 4 months from the start of imatinib treatment(400mg/day), peritoneal tumors disappeared and the patient maintained stable disease by imatinib treatment for 8 years. However, on December, 2012, an abdominal CT scan revealed the regrowth of the tumor in S6 in the liver, and PET-CT showed focal progression of only the S6 lesion. Although the lesion was resectable, the

A routine feasible HPLC analysis for the anti-angiogenic tyrosine kinase inhibitor, sunitinib, and its main metabolite, SU12662, in plasma.

Sunitinib is an oral inhibitor of multiple tyrosine kinase receptors with antitumor activity in metastatic renal cell carcinoma. So far, published methods for analysis of sunitinib and its active metabolite (SU12662) in plasma are exclusively based on mass spectrometry. In the context of a large-scale feasibility pharmacokinetic analysis, we developed an original, simple, high-performance liquid chromatography (HPLC) assay with UV detection. A stability study of sunitinib and SU12662 in different light exposure conditions is presented. Due to photo-instability of the compounds, blood sampling

Novel APLNR antagonist candesartan induces tumor vascular normalization through ROS/cGAS/STING axis and augmented sunitinib response in breast cancer.

BACKGROUND: The tumor vasculature exhibits immature development characterized by inadequate pericyte coverage, resulting in excessive permeability, poor perfusion, and hypoxia. The aberrant vascular network not only facilitates metastasis but also confers therapeutic resistance. Accumulating evidence indicates that inducing tumor vascular normalization represents a promising antitumor strategy. Given the pivotal role of the APLN/APLNR axis in vascular maturation positions it as a promising therapeutic target for anti-tumor vascular therapy. This study aims to discover novel APLNR antagonists, i

Restoring the epigenetically silenced PCK2 suppresses renal cell carcinoma progression and increases sensitivity to sunitinib by promoting endoplasmic reticulum stress.

Rationale: Tumors have significant abnormalities in various biological properties. In renal cell carcinoma (RCC), metabolic abnormalities are characteristic biological dysfunction that cannot be ignored. Despite this, many aspects of this dysfunction have not been fully explained. The purpose of this study was to reveal a new mechanism of metabolic and energy-related biological abnormalities in RCC. Methods: Molecular screening and bioinformatics analysis were performed in RCC based on data from The Cancer Genome Atlas (TCGA) database. Regulated pathways were investigated by qRT-PCR, immunoblo
