Several factors exacerbate the risk of HIV infection among Latino men, including low HIV/AIDS knowledge, low perception of HIV risk, and higher related stigma, as well as limited access to health care prevention services. HoMBRES de Familia was a randomized controlled trial to adapt and test the efficacy of an intervention that was implemented with Latino men in semi-rural and urban areas of Miami-Dade County. The HoMBRES de Familia intervention, conducted during the COVID-19 pandemic, aimed to reduce the risk of substance abuse, intimate partner violence, and HIV/AIDS (i.e., SAVA syndemic) am

The coronavirus disease 2019 (COVID-19) pandemic rendered congregate shelter settings high risk, creating vulnerability for people experiencing homelessness (PEH). This study employed participant observation and interviews over 16 months in two Veteran encampments, one located on the grounds of the West Los Angeles Veteran Affairs Medical Center (WLAVA) serving as an emergency COVID-19 mitigation measure, and the other outside the WLAVA gates protesting the lack of onsite VA housing. Study participants included Veterans and VA personnel. Data were analyzed using grounded theory, accompanied by

INTRODUCTION: In Mexico, there is a syndemic in pregnant women, where the epidemic of obesity and chronic diseases coexists with that of coronavirus disease 2019 (COVID-19), which has been associated with a higher risk of maternal mortality.
OBJECTIVE: To evaluate the association of comorbidities during pregnancy with maternal mortality from COVID-19 in Mexico.
MATERIAL AND METHODS: SISVER COVID-19 databases and epidemiological surveillance reports on maternal mortality were used. Multivariate logistic regression models were used to evaluate the association of comorbidities with maternal dea

Although substance use continues to be a significant component of HIV risk among women worldwide, to date, relatively little attention has been paid in research, services, or policy to substance-involved women (SIW). HIV acquisition for SIW stems from transmission risks directly related to substance use and risks associated with sexual activity in which power to negotiate risk and safety are influenced by dynamics of male partnerships, sex work, and criminalization (of both drug use and sex work), among other factors. As such, HIV risk for SIW resides as much in the environment—physical, socia

Tuberculosis (TB) continues to be the leading cause of morbidity and mortality among people living with HIV (PLHIV), making improved prevention and treatment of HIV-associated TB critical to ensuring long-term survival of PLHIV. TB screening among PLHIV is central to implementation of the World Health Organization's 3 I's interventions for reducing the impact of the TB and HIV syndemics. Effective TB screening will result in the identification of PLHIV with presumptive TB disease (ie, those with a positive symptom screen who require appropriate evaluation, including the use of diagnostic tools

Objectives. To use syndemic theory and system dynamics to describe how systemic racism shapes interrelationships among interpersonal violence, alcohol and substance misuse, and reproductive health among American Indian and Alaska Native communities. Methods. We conducted a community-based system dynamics study in a Great Plains metropolitan area. Using purposive and key informant sampling, we interviewed 110 participants (2018-2024) to develop causal loop diagrams. We aggregated 19 diagrams into a synthesized model, refined it through 5 additional group sessions (n = 28), and applied rigorousl

Transgender women and travesti sex workers in Brazil underutilise HIV prevention services. Understanding preferences and decision-making regarding HIV prevention can help develop new programmes to meet their needs. We conducted semi-structured interviews with 26 transgender women and travesti sex workers in São Paulo, Brazil. Interviews examined experiences with and perceptions of condoms, non-occupational post-exposure prophylaxis (nPEP), daily oral pre-exposure prophylaxis (PrEP), and injectable PrEP. Transcripts were thematically analysed. Participants' median age was 34.5 years and most (8

Given the significant syndemic of HIV and substance use in the U.S., this study sought to examine substance use patterns (consistent user, former user, recent adopter, and abstainers) and identify sociodemographic disparities among people with HIV (PWH). Using 2015-2019 National Survey on Drug Use and Health data (N = 212,411), we estimated prevalence rates and conducted multinomial logistic regression to assess associations between HIV status and (1) substance use and (2) sociodemographic factors. Results show PWH have higher rates of consistent and former use of a range of substances, includ

PURPOSE OF REVIEW: This article reviews current efforts to control bacterial sexually transmitted infections (STIs) among HIV pre-exposure prophylaxis (PrEP) users and outlines the opportunities and challenges to controlling STIs within HIV PrEP programs.
RECENT FINDINGS: The incidence of STIs continues to rise globally especially among HIV PrEP users, with an estimated 1 in 4 PrEP users having a curable bacterial STI. STIs and HIV comprise a syndemic needing dual interventions. The majority of STIs are asymptomatic, and when testing is available, many STIs occur in extragenital sites that ar

One of the biggest challenges of the COVID-19 pandemic is the heterogeneity in disease severity exhibited amongst patients. Among multiple factors, latest studies suggest vitamin D deficiency and pre-existing health conditions to be major contributors to death from COVID-19. It is known that certain urban form attributes can impact sun exposure and vitamin D synthesis. Also, long-term exposure to air pollution can play an independent role in vitamin D deficiency. We conducted a correlational analysis of urban form and air quality in relation to the demographics and COVID-19 incidence and morta

This brief report emphasizes the need to focus on women with HIV who are pregnant who use alcohol or other drugs. A recently completed implementation science study tested a gender-focused behavioral intervention, the Women's Health CoOp (WHC), to improve antiretroviral therapy (ART) adherence and reduce alcohol use among women with HIV. The study identified 33 participants who had a positive pregnancy test result at the baseline assessment, of whom five participants remained pregnant during the 6-month duration of the study. Of the 33 pregnant participants at the baseline assessment, 55% repor

PURPOSE OF REVIEW: Alcohol and noninjection drug use has been shown to be associated with increased risk of HIV infection in select populations. In this review, we discuss recent data on the prevalence of alcohol and noninjection drug use and the relationship to HIV acquisition and transmission risk.
RECENT FINDINGS: A strong association between alcohol use and HIV-infection risk has been demonstrated in multiple studies conducted in sub-Saharan Africa. Among men who have sex with men in the USA as well as other countries, substance use is highly prevalent and has been associated with high-ri

Giardia lamblia infections are nearly universal among children in low-income countries and are syndemic with the triumvirate of malnutrition, diarrhea, and developmental growth delays. Amidst the morass of early childhood enteropathogen exposures in these populations, G. lamblia–specific associations with persistent diarrhea, cognitive deficits, stunting, and nutrient deficiencies have demonstrated conflicting results, placing endemic pediatric giardiasis in a state of equipoise. Many infections in endemic settings appear to be asymptomatic/ subclinical, further contributing to uncertainty reg

OBJECTIVE: Type 2 diabetes (T2D) increases the risk of tuberculosis (TB) and TB severity; however, its effects on B-cells in the TB-T2D syndemic are not fully understood.
METHODS: We evaluated by flow cytometry, the distribution of circulating B-cell subsets in patients with TB disease, TB-T2D, subjects with TB infection (TBI), TBI-T2D, healthy controls (HCs), T2D, patients with pulmonary respiratory diseases other than TB (ORDs), and ORD-T2D. Plasma levels of unspecific or PPD-specific IgG and B-cell-related soluble factors were measured by multiplex assays or ELISA.
RESULTS: Total B-cell f

OBJECTIVES: Marginalized urban populations experience a high burden of HIV and HCV. We assessed the prevalence and risk factors for both infections and characterized the care cascades in a cohort of vulnerable individuals in Madrid, Spain.
METHODS: We conducted a cross-sectional study (2019-2023) of 4582 individuals via mobile units in high-risk hotspots, offering integrated rapid HIV/HCV testing with point-of-care HCV-RNA confirmation. Multivariable logistic regression identified independent HIV risk factors.
RESULTS: HIV prevalence was 6.3% (95% CI: 5.6-7.0). Among people with HIV (PWH), 1

OBJECTIVE: The overdose crisis potentially increases risk factors for suicide while decreasing resources available to help those in need. This study sought to determine whether acute increases in drug or alcohol overdoses increase suicide rates.
METHODS: This serial cross-sectional study used comprehensive emergency department, hospital discharge, and mortality data for ZIP-code-months in California, March 2010-December 2018 (n = 163,240). Overdose spikes were defined as ZIP-code-months with rates >2 standard deviations above expected. We used confounder-adjusted Bayesian Poisson space-time m

Young Black Sexual Minority Men with HIV (YBSMM+) in the US South encounter multiple socio-structural challenges that contribute to disproportionately poor HIV-related outcomes across the care continuum. Depression, anxiety, intimate partner violence (IPV), and alcohol use are prominent factors that negatively impact engagement with HIV care. Syndemic theory posits that these multiple factors interact synergistically to promote poor outcomes; however, depression itself is highly heterogeneous in presentation, which may pose issues when examining associations to HIV care engagement. This study

OBJECTIVE: To characterize and address the opioid crisis disproportionately impacting rural U.S. regions.
METHODS: The Rural Opioid Initiative (ROI) is a two-phase project to collect and harmonize quantitative and qualitative data and develop tailored interventions to address rural opioid use. The baseline quantitative survey data from people who use drugs (PWUD) characterizes the current opioid epidemic (2018-2020) in eight geographically diverse regions.
RESULTS: Among 3,084 PWUD, 92% reported ever injecting drugs, 86% reported using opioids (most often heroin) and 74% reported using metha

BACKGROUND: The current SARS-CoV-2 pandemic has especially affected individuals living in conglomerate settings having poverty as common characteristic. However, evidence of the association between COVID-19 severity and social determinants is still scarce, particularly, for Latin American countries. The objective was to assess the effect of socioeconomic deprivation in the clinical severity of COVID-19 infection among different localities of Bogotá, Colombia.
METHODS: Secondary analyses using data of SARS-CoV-2 infected cases in Bogotá from 6 March 2020 to 19 April 2020 were carried out. Dire

The rapid increase in the consumption of ultra-processed foods (UPFs) has become a significant global public health concern. UPFs are typically high in unhealthy fats, refined sugars, sodium, and other additives while being low in proteins, fibers, and other essential nutrients. Their high glycemic index and glycemic load lead to blood sugar spikes, contributing to metabolic dysregulation. Emerging evidence links UPF intake to the development of the cardiovascular-kidney-metabolic (CKM) syndrome and a higher risk of adverse health outcomes, such as all-cause and cardiovascular mortality.
