Digital vertigo therapy: study protocol for a confirmatory randomized controlled trial in patients with vestibular vertigo.

BACKGROUND: Vestibular vertigo is one of the leading causes of disability. The clinical standard of care for vestibular vertigo includes physical activity producing central vestibular compensation (CVC). Home exercises are considered an integral part of physical therapy. However, a reliable solution is still needed to support the regular and correct execution of home exercises. For this purpose, VH-90-D DiGA was developed, which is a digital therapeutic (DTx) for multimodular in-home therapeutic training. OBJECTIVE: The purpose of this study is to assess the clinical efficacy and safety of a

Similarity of vertigo attacks due to Meniere's disease and benign recurrent vertigo, both with and without migraine.

CONCLUSION: Vertigo attacks in patients with benign recurrent vertigo (BRV) cannot be distinguished from those in patients with Meniere's disease on the basis of duration, triggers or associated symptoms (other than auditory). A subset of BRV is associated with migraine. OBJECTIVE: To investigate whether clinical features of vertigo attacks can distinguish patients with BRV from those with Meniere's disease and whether subtypes of BRV can be identified. METHODS: A structured interview was used to analyze features in patients with BRV, i.e. those who have normal audiograms and caloric test re

Visual analog scale to assess vertigo and dizziness after repositioning maneuvers for benign paroxysmal positional vertigo.

INTRODUCTION: The aim of this study was to evaluate the value of Visual Analog Scale (VAS) in evaluating vertigo and dizziness in patients with benign paroxysmal positional vertigo (BPPV). MATERIALS AND METHODS: 226 adult patients suffering from a BPPV and undergoing repositioning maneuvers were included in this prospective study. Vertigo (V) and dizziness (D) were separately evaluated from day 0 to 5 by VAS. V and D scores decreased both exponentially but with different constants. D scores were not correlated to V suggesting the independency of the ratings. D was related to the dizziness int

Earbuds Induced Benign Paroxysmal Positional Vertigo?

BACKGROUND: Benign paroxysmal positional vertigo (BPPV) is the most common cause of peripheral vertigo. BPPV is usually idiopathic but can also result from trauma, viral labyrinthitis, or Meniere's disease. We present a case of BPPV caused by earbuds.Case Descripton:This is the case of a 43-year-old male, a health care management and emergency medicine specialist, with no history of inner- or middle ear disease, vertigo and dizziness, or other medical conditions. The subject began using earbuds while driving and during routine exercise. He reported suffering from nausea and temporary vertigo a

Soccer and Benign Paroxysmal Positional Vertigo.

Introduction. Benign paroxysmal positional vertigo (BPPV) is the most common cause of vertigo among adults. The etiology of BPPV is unknown in approximately 50 percent of cases. This condition is also termed primary BPPV, if the etiology is unknown, and secondary BPPV if patients have identified predisposing factors. A few studies suggest that there is a correlation between the development of BPPV and specific sports. Case Report. A 19-year-old male presented with recurrent episodes of vertigo during soccer play. Eight months prior to referral, the patient was involved in a car accident with a

Vascular vertigo and dizziness: Diagnostic criteria.

This paper presents diagnostic criteria for vascular vertigo and dizziness as formulated by the Committee for the Classification of Vestibular Disorders of the Bárány Society. The classification includes vertigo/dizziness due to stroke or transient ischemic attack as well as isolated labyrinthine infarction/hemorrhage, and vertebral artery compression syndrome. Vertigo and dizziness are among the most common symptoms of posterior circulation strokes. Vascular vertigo/dizziness may be acute and prolonged (≥24 hours) or transient (minutes to  < 24 hours). Vascular vertigo/dizziness should be con

Differential diagnosis of acute vascular vertigo.

PURPOSE OF REVIEW: The current review covers recent advances in vascular vertigo in terms of diagnostic strategies, clinical/laboratory features, pathophysiology, and differential diagnosis. RECENT FINDINGS: Acute strokes presenting with isolated dizziness/vertigo without other obvious symptoms or signs of central nervous system involvements may be easily mistaken as peripheral vestibulopathy. For correct diagnosis of vascular vertigo, the importance of clinical history (timing and triggers) and targeted bedside examination cannot be overemphasized. In addition to Head Impulse-Nystagmus-Test

Recent surgical options for vestibular vertigo.

Vertigo is not a well-defined disease but a symptom that can occur in heterogeneous entities diagnosed and treated mainly by otolaryngologists, neurologists, internal medicine, and primary care physicians. Most vertigo syndromes have a good prognosis and management is predominantly conservative, whereas the need for surgical therapy is rare, but for a subset of patients often the only remaining option. In this paper, we describe and discuss different surgical therapy options for hydropic inner ear diseases, Menière's disease, dehiscence syndromes, perilymph fistulas, and benign paroxysmal posi

Cervical Vertigo: Historical Reviews and Advances.

Vertigo is one of the most common presentations in adult patients. Among the various causes of vertigo, so-called cervical vertigo is still a controversial entity. Cervical vertigo was first thought to be due to abnormal input from cervical sympathetic nerves based on the work of Barré and Liéou in 1928. Later studies found that cerebral blood flow is not influenced by sympathetic stimulation. Ryan and Cope in 1955 proposed that abnormal sensory information from the damaged joint receptors of upper cervical regions may be related to pathologies of vertigo of cervical origin. Further studies fo

[Peripheral, central and functional vertigo syndromes].

Depending on the temporal course, three forms of vertigo syndrome can be differentiated: 1) vertigo attacks, e.g. benign paroxysmal positional vertigo (BPPV), Menière's disease and vestibular migraine, 2) acute spontaneous vertigo lasting for days, e.g. acute unilateral vestibulopathy, brainstem or cerebellar infarction and 3) symptoms lasting for months or years, e.g. bilateral vestibulopathy and functional vertigo. The specific therapy of the various syndromes is based on three principles: 1) physical treatment with liberatory maneuvers for BPPV and balance training for vestibular deficits,

Bilateral subclavian steal syndrome with vertigo.

Subclavian steal syndrome (SSS) is usually caused by unilateral subclavian artery (SA) occlusion, and bilateral SSS is very rare. Takayasu's arteritis (TA) is a chronic granulomatous form of vasculitis that affects the SA, most commonly in women aged 15-40 years. We report a rare case of bilateral SSS due to TA in a 52-year-old woman, who exhibited severe vertigo. Although her blood pressure was within the normal range and did not differ between her arms, Doppler ultrasonography revealed low antegrade blood flow in the right SA and retrograde flow in the left SA. Computed tomography angiograph

Rotational vertigo associated with putaminal infarction.

A 57-year-old man was admitted because of left hemiparesis. A magnetic resonance imaging scan of the brain revealed a recent infarct from the dorsal side of the right putamen to the corona radiata. Three hours after onset, he complained of rotational vertigo. Truncal ataxia was also found on standing a few days later, which persisted until the chronic phase. Repeat magnetic resonance imaging scans on days 2 and 11 revealed no additional lesions. The present case indicates that rotational vertigo might result from a small, supratentorial, subcortical lesion.

Sound-induced vertigo after cochlear implantation.

AIM: Postoperative vertigo is a well-known complication after cochlear implantation. The aim of the study was to investigate whether the electrical stimulation of the auditory structures via cochlear implant electrodes can affect the vestibular system and induce vertigo. MATERIALS AND METHODS: In the first group, 114 patients were surveyed retrospectively via questionnaires to evaluate the occurrence and frequency of sound-induced vertigo after cochlear implantation. In the second group of 26 patients, the effects of electrical stimulation on the vestibular system were studied prospectively.
