We have experienced three cases of extramammary Paget's disease (EMPD) of the vulva that received radiation therapy (RT). Here, we analyze the efficacy of RT and include a literature survey.Three patients with EMPD of the vulva were treated with curative RT between 1993 and 1998. One of the patients had associated underlying adenocarcinoma of the vulva. The total doses of radiation administered were 54~78 Gy/6~8 weeks. Radiation fields encompassed 2 to 3 cm outer margins free from all visible disease including or not including the inguinal area using a 9 MeV electron or a 6 MV photon beam. Fol

Six patients with vulvar malignant melanoma are reported. They accounted for 5.2% of all females with vulvar malignancies diagnosed in the south of Israel between 1961 and 1997. Age ranged from 25 to 66 years. Presenting symptoms were pruritus, bleeding and ulcer. Lesion originated in the labia minora in four patients and the labia majora in two, and lesion size ranged from I to 8 cm. Five patients had nodular melanoma, and one had superficial spreading melanoma. Breslow depth ranged from 2.5 to 8 mm, Clark level was IV in four patients and III in two, and Chung level was IV in all patients. T

We studied 19 cases of vulvar melanoma to determine significant clinical and histologic prognostic predictors. The average follow-up time was 32 months. Fourteen patients died of melanoma, four patients are alive with no evidence of melanoma, and one patient is alive with residual melanoma. All disease-free survivors had clinical stage I disease and a maximum tumor thickness of 1.3 mm. The average mitotic count in this group was 5.5 per 10 high power fields (HPF) and 50% of the tumors were superficial spreading melanomas. All survivors were treated by radical vulvectomy with bilateral inguinal

The authors reviewed the records of 50 patients with carcinoma in situ of the vulva seen at the State University of New York at Buffalo affiliated hospitals. Five patients (10%) were found to be immunosuppressed and 12 (24%) had other preinvasive and invasive genital or extragenital neoplasia. Of 43 patients whose symptomatology was available, almost half (46.5%) were asymptomatic. The most common symptom was vulvar itching in 18 (42%), and 32 (65%) patients had white vulvar lesions. Thirteen patients underwent vulvectomy, 23 underwent wide local excision, 8 underwent skinning and skin graft,

INTRODUCTION: Vulvar Paget's disease (VPD) is a rare vulvar malignant tumor commonly observed in postmenopausal females. VPD cases are mainly treated by surgery, and the postoperative recurrence rate is high. Owing to the absence of any specific clinical manifestations, VPD is often misdiagnosed as eczematous skin lesions, which leads to diagnostic delay.
CASE PRESENTATION: A 63-year-old woman experienced vulvar itching for over 4 months after 15 years of menopause. Computed tomography scans revealed a slightly friable vulva. Gynecological examination detected that the bilateral labia majora,

Vulvar cancer (VC) represents about 4% of gynecologic malignancies, its incidence increases with age and peak incidence is found between 70-79 years. In cases of locally advanced disease surgery is often required and radical vulvectomy, with or without mono-bilateral inguino-femoral lymphadenectomy, is standard management. Various devices have been implemented in gynecological surgery in an attempt to minimize or avoid frequent intra/postoperative complications linked to energy use, unfortunately the majority of these devices require monopolar or bipolar energy. Ultracision® represents a uniqu

BACKGROUND: Dermatofibrosarcoma Protuberans (DFSP) of the vulva is rare and oncologic surgery with free margins may lead to severe functional damage, requiring multidisciplinary approach regarding resection, margin assessment and reconstruction.Case Report: Two cases of DFSP in vulva were treated in a single institution. A 28-year-old patient with an incisional biopsy in the vulvar region revealing DFSP underwent a partial vulvectomy with clitoris preservation. Pathological studies revealed free margins and reconstructive surgery was performed. This patient is disease free in a 40 months follo

INTRODUCTION: The current management of vulvar cancer depends on the extension of disease, and includes primary tumor resection with safety margin as well as inguinofemoral lymph node staging. We report the case of the first leg videoendoscopic inguinal lymphadenectomy performed in a woman with a squamous cell vulvar carcinoma.
PRESENTATION OF CASE: A 74 years old female referred to our institution complaining of vulvar mass associated with bleeding and swelling from external genitals, vaginal burning sensation and dysuria for 5 months. A vulvar-vaginal examination under narcosis reported a r

INTRODUCTION: The female genital sensory pathways that initiate sexual arousal reflexes begin with cutaneous corpuscular receptors in the glabrous genital skin, including those of the glans clitoris.
AIM: The aim of this study is to characterize the corpuscular receptors of the glans clitoris. In addition, we compared basic features with the receptors of the glans penis.
MAIN OUTCOME MEASURE: Number of stained receptors.
METHODS: Five cadaveric vulvectomy specimens and four cadaveric penile specimens were used. They were serially sectioned and stained with hematoxylin and eosin. Selected bl

AIMS: Vulvar cancer is a rare disease with increasing incidence over the last decades. Treatment includes surgical, radio- and chemotherapeutical options; however, due to the low incidence of the disease and the lack of randomised trials many questions regarding indication of different treatment approaches remain unanswered. This article discusses the current literature to elaborate recommendations for the management of primary vulvar cancer in clinical routine.
METHODS: We reviewed the available literature on treatment of invasive vulvar cancer with emphasis on therapeutic strategies such as

Subependymal giant cell astrocytoma (SEGA) is a benign, slowly growing tumor typically occurring in the setting of tuberous sclerosis complex (TSC). However there are several reported cases in which patients with a solitary SEGA had no other stigmata of TSC. We describe a case of SEGA in a 75-year-old woman representing the oldest patient reported to-date. The patient had a history of radical vulvectomy for malignant melanoma (MM), and died of autopsy-confirmed widespread systemic metastasis. Postmortem examination of the brain revealed a single 2.1 x 1.0 x 0.8 cm intraventricular nodule in th

BACKGROUND: Vulvar melanoma represents a rare group of malignancies and is the second most common vulvar malignancy. Treatment options range from local excision of the tumor and sentinel lymph node dissection to radical resection involving en bloc vulvectomy and inguinofemoral lymphanedectomy. Vulvar melanomas have an overall poor prognosis, and there is lack of consensus in the published literature regarding treatment options.
OBJECTIVE: To discuss the management of vulvar melanomas through review of the actual literature.
METHODS: Identification of studies through computerized searches (Ja

OBJECTIVE: To evaluate the therapeutic regimens and symptomatic response rates in patients with vulvar lichen sclerosus associated with variable degrees of squamous cell hyperplasia (mixed disease).
MATERIAL AND METHODS: Eighty-three women with biopsy-proven vulvar mixed disease were evaluated for this retrospective study. All patients were initially treated with topical fluorinated corticosteroids, and then 2% testosterone propionate in petrolatum or 0.05% clobetasol 17-propionate (44 (53%) versus 39 (47%)).
RESULTS: The remission rates were 82 and 93% in the testosterone and clobetasol sub

BACKGROUND: Urogenital melanoma is a rare neoplasm with poor prognosis. Its management in the past involved radical vulvectomy and complete bilateral inguinofemoral lymphadenectomy. Sentinel lymph node biopsy is an accurate low-morbidity procedure when used in the context of cutaneous melanoma. However, prophylactic lymphadenectomy has not been shown to improve survival of melanoma patients. We wanted to determine the feasibility of sentinel lymph node biopsy in patients with female urogenital melanoma as a staging procedure.
METHODS: Six patients with vulvar or vaginal melanomas underwent pr

BACKGROUND: The aim of this multicenter study was to investigate the feasibility and negative predictive value of sentinel lymph node detection with blue dye in vulvar carcinoma patients.
METHODS: In patients with squamous cell carcinoma of the vulva without suspicious groin lymph nodes, patent blue V was injected intradermally shortly before surgery. Routine groin lymph node dissection and radical vulvectomy were performed. During the surgery, blue lymph vessels and lymph nodes were identified, and the blue lymph nodes were sent separately for histologic examination. The negative predictive

There is a 100-fold increase in the incidence of carcinomas of the vulva and anus in renal transplant recipients compared with the general population. Anogenital (anus, perianal skin, and external genitalia of both sexes) carcinomas occurred in 65 of 2150 renal transplant recipients who presented with 2298 different types of malignancy. Two-thirds of the patients were female and one-third male. They were much younger than persons with similar tumors in the general population. The average age of the females at the time of diagnosis was 37 years (range, 20-64) and of the males, 45 years (range,

BACKGROUND Vulvar cancer (VC) in pregnancy is a rare case, and its incidence is unknown until now. This case report highlights the rapid progression of disease 3 months after delivery, and our limitation in low resource setting. CASE REPORT A pregnant woman in her early 40's presented with a left vulvar lesion. The lesion was 7×5 cm, with clitoris involvement. No infiltration to adjacent organs was present. Her biopsy showed well-differentiated squamous cell vulva carcinoma. After C-section delivery at 34 weeks of gestation, the diagnosis remained FIGO stage IB (T1b N0 M0). MRI and radical vul
