Differential diagnosis of HTLV-I and HTLV-II infections by restriction enzyme analysis of 'nested' PCR products.

A 'nested' polymerase chain reaction (PCR) assay is described which is capable of detecting single copies of human T-cell lymphotropic virus (HTLV) in genomic DNA extracted from peripheral blood mononuclear cells (PBMCs). A single set of 'nested' oligonucleotide primers, based on the highly conserved tax/rex region of the viral genome, was able to detect both HTLV-I and HTLV-II proviral sequences in clinical samples of diverse geographical origins, from the United States, Great Britain, Japan, the Caribbean, Italy, Greece, Iraq and West Africa. Rapid discrimination between HTLV-I and HTLV-II i

Diagnostic Challenges and Surgical Outcomes of Miyazaki Syndrome: A Report of Two Cases and a Systematic Review.

Miyazaki syndrome is a rare and underrecognized complication of long-standing cerebrospinal fluid (CSF) diversion. It usually occurs in patients with chronic CSF overdrainage and is caused by enlargement of the cervical epidural venous plexus, which may compress the spinal cord and cause progressive myelopathy or radiculopathy. Because symptoms develop slowly and may resemble degenerative cervical myelopathy or other neurological conditions, diagnosis is often challenging. We report two illustrative cases from our institution. The first patient was a 57-year-old man with a ventriculoperitoneal

[X-linked adrenoleukodystrophy].

X-linked adrenoleukodystrophy (ALD) is a severe neurodegenerative disorder. ALD is characterized by progressive demyelination within the central and peripheral nervous system, adrenal insufficiency (Addison's disease) and accumulation of very-long-chain fatty acids (VLCFA) in plasma, fibroblasts and tissues. The overall incidence of ALD is 1:17,000 including hemizygotes and heterozygotes who are frequently symptomatic. There are two main ALD phenotypes: 1) a cerebral demyelinating form which affects boys between 5-12 years, but also 35% of adult males; 2) a form that mainly involves the spinal

Natural history of congenital kyphosis and kyphoscoliosis. A study of one hundred and twelve patients.

BACKGROUND: Congenital kyphosis and kyphoscoliosis are much less common than congenital scoliosis. However, they are potentially more serious because compression of the spinal cord and paraplegia sometimes develop. The goals of the present study were to document the natural history of congenital kyphosis and kyphoscoliosis and to determine the stage at which the natural progression should be interrupted by treatment. METHODS: We reviewed the medical records and radiographs of the spine of 112 consecutive patients. Sixty-eight patients had a type-I kyphosis due to an anterior failure of verteb

Endovascular stent-graft repair of thoracic aortic aneurysms.

UNLABELLED: Conventional repair of aneurysms of the descending thoracic aorta entails thoracotomy and graft interposition. For elderly patients and those with previous operations, obesity, respiratory insufficiency, or other comorbidities, such a procedure entails significant mortality and morbidity. Transluminal stent-graft placement offers an alternative approach with potentially less morbidity and quicker recovery; however, the effectiveness and durability of stent-grafts remain uncertain. METHODS: Since July 1992, thoracic aortic stent-grafts have been placed in 44 patients with a variety

Th1/Th2 balance and HTLV-I proviral load in HAM/TSP patients treated with interferon-alpha.

We studied the immunological and virological effects of interferon-alpha (IFN-alpha) therapy in nine patients with HTLV-I-associated myelopathy (HAM/TSP). After therapy, the percentages of CCR5+ cells in CD4+ cells significantly decreased in the cerebrospinal fluid as well as blood. The therapy also significantly lowered the intracellular IFN-gamma+/interleukin-4+ T-cell ratio in blood. Those helper T-cell type 1 (Th1)-related responses tended to be higher and reduce more evidently following therapy in three patients who clinically improved. Also, all the three patients had one or more HTLV-I

Endovascular and Hybrid Repair in Patients with Heritable Thoracic Aortic Disease.

BACKGROUND: In individuals with heritable thoracic aortic disease (HTAD), endovascular repair for treatment of aortic aneurysm and dissection may be lifesaving, but is associated with increased risk of failure of endovascular repair and adverse outcomes. This study reports our experience with early and late outcomes of endovascular aortic and branch vessel repair in patients with HTAD. METHODS: A retrospective case series was performed by chart review of individuals with HTAD followed at Washington University School of Medicine/Barnes-Jewish Hospital who underwent endovascular aortic and/or b

[A case of primary progressive multiple sclerosis with onset of memory impairment].

We report a 52-year-old woman with primary progressive multiple sclerosis (PPMS) presenting with chronic progressive memory impairment. From a couple of years prior to admission, she had developed impairment of her short-term memory. For example, she forgot her nephew's name, and spoke the same phrases again and again. She also sometimes forgot to turn off her gas stove and forgot things she bought in shops. Moreover, her mental activity gradually decreased and she became apathetic. However, she did not note her memory impairment, and had no hallucinations. She was admitted to our hospital on

Safety and efficacy of delayed-release diameter reduction in constructing physician-modified endografts for complex aortic aneurysms.

BACKGROUND: Physician-modified endografts (PMEGs) remain an option to treat complex aortic aneurysms with fenestrated endovascular aortic repair (FEVAR) if they are unsuitable for commercial endografts or open surgery. FEVAR traditionally involves uniform diameter reduction for graft alignment and target vessel cannulation, after which diameter reduction is released before the delivery of bridging stent grafts. We describe the results of an advanced PMEG design using nonuniform constraints to mold the graft to the inner shape of the visceral aorta with delayed-release diameter reduction. This

Long-term outcomes for hybrid aortic arch repair.

OBJECTIVE: Since its inception in the early 2000s, hybrid arch repair (HAR) has evolved from novel approach to well-established treatment modality for aortic arch pathology in appropriately selected patients. Despite this nearly 20-year history of use, long-term results of HAR remain to be determined. As such, objectives of this study are to detail the long-term outcomes for HAR within an expanded classification scheme. METHODS: From August 2005 to August 2022, 163 consecutive patients underwent HAR at a single referral institution. Operative approach was selected according to an institutiona

Protective use of N-methyl-D-aspartate receptor antagonists as a spinoplegia against excitatory amino acid neurotoxicity.

OBJECTIVE: Paraplegia remains a serious complication of thoracic and thoracoabdominal aortic operations. To avoid this dreadful complication, N-methyl-D-aspartate (NMDA) receptor antagonists have been examined in the ischemic or excitotoxic neuronal injury model. In the present study, we evaluated the protective efficacy of NMDA receptor antagonists that were infused segmentally after aortic clamping, as a spinoplegia, to reduce aspartate neurotoxicity in the spinal cord. METHODS: Infrarenal aortic isolation was performed in New Zealand white rabbits. Group A animals (n = 7) were pretreated w

[Determination of anti-HTLV-I/II antibodies: Experience in 28,897 blood donations in Buenos Aires].

PURPOSE: The first human retrovirus, HTLV-I, was isolated in 1980; HTLV-II was described later. The former is endemic in southwestern Japan, the Caribbean and equatorial Africa; whereas the latter prevails in intravenous drug addicts, being also endemic in American indian populations. Both viruses are either sexually transmitted, from mother to child mainly by breast-feeding, by blood transfusion or by sharing contaminated needles. With regard to transmission, since they are intracellular viruses, it occurs only when whole blood or cellular components are transfused; this is not the case when

Personalized upper limb training combined with anodal-tDCS for sensorimotor recovery in spastic hemiparesis: study protocol for a randomized controlled trial.

BACKGROUND: Recovery of voluntary movement is a main rehabilitation goal. Efforts to identify effective upper limb (UL) interventions after stroke have been unsatisfactory. This study includes personalized impairment-based UL reaching training in virtual reality (VR) combined with non-invasive brain stimulation to enhance motor learning. The approach is guided by limiting reaching training to the angular zone in which active control is preserved ("active control zone") after identification of a "spasticity zone". Anodal transcranial direct current stimulation (a-tDCS) is used to facilitate act

Human T-lymphotropic virus type 1 prevalence in northeastern Iran, Sabzevar: an epidemiologic-based study and phylogenetic analysis.

Human T-lymphotropic virus type 1 (HTLV-I) is an important global health problem in the world mainly in the endemic areas of HTLV-I infection. It was previously reported that Mashhad, in northeastern Iran, is a new endemic region of HTLV-I. The aim of this study was to examine the prevalence and phylogenetic analysis of HTLV-I in Sabzevar, located in the southeast of Mashhad. In this cross-sectional study 1445 individuals were selected by multistage cluster sampling. Serum samples were screened for anti-HTLV-I antibody using enzyme-linked immunosorbent assay (ELISA); all of the ELISA-positive

Intrafamilial clustering and 4-year follow-up of asymptomatic human T-cell leukaemia virus type I (HTLV-I) infection in Benin (West Africa).

BACKGROUND: Few data exist concerning familial human T-cell leukaemia virus type I (HTLV-I) carrier states and transmission in African countries. Two previous surveys performed in Benin in 1989 and 1990 using a three-level cluster sampling method allowed us to identify HTLV-I positive subjects. The evolution of HTLV-I within the families of these subjects is described over a 4-year period, 1991-1995. METHODS: Since 1991, 37 HTLV-I seropositive subjects, six subjects with indeterminate Western-Blot pattern, and their relatives have been followed up once a year clinically and biologically. RES
