Cryotherapy ablation of parahisian accessory pathways in children.

BACKGROUND: Because of its safety profile, cryoablation has become an alternative therapy for septal arrhythmias, including parahisian accessory pathways (APs). Data regarding its efficacy, safety, and late outcome for parahisian APs in children are limited. OBJECTIVE: The purpose of this study was to evaluate the safety and efficacy of cryoablation of parahisian APs in children. METHODS: Fifty-nine cryoablations of parahisian APs were performed in 50 patients [76% males; median age 16.5 years (range 5.3-20 years)]. Thirty-three had manifest APs and 17 had concealed APs. The time to effect w

50 is the new 70: Short ventriculoatrial times are common in children with atrioventricular reciprocating tachycardia.

BACKGROUND: One of the basic electrophysiological principles of atrioventricular reciprocating tachycardia (AVRT) is that ventriculoatrial (VA) times during tachycardia are >70 ms. We hypothesized, however, that children may commonly have VA times <70 ms in AVRT. OBJECTIVE: This study sought to determine the incidence and characteristics associated with short-VA AVRT in children. METHODS: A retrospective single-center review of children with AVRT from 2000 to 2014 was performed. All patients ≤18 years of age with AVRT at electrophysiology study were included. Patients with persistent junctio

Quantitative effects of functional bundle branch block in patients with atrioventricular reentrant tachycardia.

Changes in the retrograde conduction time (ventriculoatrial [VA]) interval during functional bundle branch block (BBB) have been used to separate septal from free wall accessory pathways (APs), but different values of the VA interval prolongation (deltaVA) have been described in different reports. A total of 95 patients with single nondecremental APs who developed BBB during atrioventricular reentrant tachycardia were studied. Free wall APs were found in 60 patients, and 35 had septal APs. For patients with free wall APs, complete and incomplete BBB ipsilateral to the atrial insertion site of

[A concise history of the Wolff-Parkinson-White syndrome].

In a paper entitled Bundle-branch block with short P-R interval in healthy young people prone to paroxysmal tachycardia (Am Heart J 1930; 5: 685-704), Dr. Louis Wolff (1898-1972), Sir John Parkinson (1885-1976) and Paul Dudley White (1886-1973) described an intricate syndrome. Prior case reports had already pointed out the essentials of this entity, which has borne the eponym (WPW syndrome) since the publication by Levine and Beeson (Am Heart J 1941; 22: 401-409). It was long thought that the first patient with a short PR interval, delta-wave-induced widening of the QRS complex and paroxysmal

[Wolff-Parkinson-White syndrome due to cocaine use: a case report].

BACKGROUND: Wolff-Parkinson-White syndrome (WPWS) causes an accessory pathway between the atria and ventricles, in parallel with the atrioventricular node and the bundle of His; it causes a "short circuit" that deregulates the physiological pacing and causes tachycardia. A case of WPWS is described in a patient with cocaine, alcohol, and tobacco consumption. CLINICAL CASE: A 34-year-old man with a regular history of cocaine, alcohol, and tobacco use for 14 years presented with paroxysmal palpitations, chest pain, and shortness of breath. ECG 1 revealed supraventricular tachycardia with delta

Reciprocated tachycardias in cardiac laminopathy: a clinical case report.

BACKGROUND: Cardiac laminopathies, associated with mutations in the LMNA gene, are a rare inherited disorder characterized by a broad range of clinical manifestations. There are currently no data on the association between supraventricular re-entrant tachycardias and LMNA-related cardiomyopathy. CASE SUMMARY: A 26-year-old male presented with either wide-QRS tachycardia with a left bundle branch block (LBBB) pattern or narrow QRS tachycardia, as well as a history of palpitations since age 15. Echocardiography showed no overt structural heart disease. Electrophysiological studies confirmed the

Open window mapping with extended early meets late algorithm vs. conventional mapping for accessory pathway ablation.

BACKGROUND: Catheter ablation of accessory pathway is the treatment of choice for patients with symptomatic Wolff-Parkinson-White (WPW) syndrome. Accessory pathway (AP) identification relies on point-by-point mapping, raising the need for more precise and efficient methods. High-density open window mapping (OWM) combined with the extended early meets late (EEML) algorithm, utilizing 3D electroanatomic mapping systems, is a promising alternative. However, its role in clinical practice lacks comprehensive investigation, necessitating a comparison with conventional mapping. METHODS: A prospectiv

Characteristic features of patients with multiple accessory pathways.

Objective Only limited clinical and electrophysiological data concerning patients (pts) with multiple accessory pathways (MAP) in comparison to large control groups are available. The aim of our study was to analyse these data from the largest cohort of patients with multiple accessory pathways and a large control group. Method and results We analysed data from pts with MAP (group 1) and pts with a single accessory pathway (AP) (group 2) referred for radiofrequency catheter ablation (RFCA) at our tertiary centre. Group 1 consisted of 124 pts (M 62.10%, mean age 33.00 ± 5.26) with MAP and RFCA.

Ablating the anteroseptal accessory pathway-ablation via the right internal jugular vein may improve safety and efficacy.

PURPOSE: Ablation of anteroseptal accessory pathways have historically been associated with lower success rates and a higher risk of AV nodal injury due to close proximity to the compact AV node. We describe the technique of ablation of anteroseptal APs via the right internal jugular vein (RIJV). METHODS: A retrospective analysis of all patients undergoing EP study and ablation at the Children's Hospital at Montefiore from 2008 to 2011 for SVT and/or WPW was performed. All patients less than 21 years of age who underwent ablation from the RIJV for either WPW or a concealed accessory pathway l

Electrocardiographic criteria for diagnosis of irregular broad complex tachycardia with a high sensitivity for preexcited atrial fibrillation.

AF may appear as an irregular broad complex tachycardia (BCT) if atrioventricular conduction occurs via an accessory pathway (preexcited AF) or if bundle branch block (BBB), preexistent or rate related, exists in the His-Purkinje system (BBB-AF). While BBB-AF is relatively benign, preexcited AF may herald sudden cardiac death. Hence it is important that the two conditions can be reliably distinguished. Yet, there is no preexistent algorithms for this purpose. Griffith et al. previously proposed a simple algorithm for a similar problem, that of distinguishing the two differential diagnoses for

Pre-excitation syndrome and hypertrophic cardiomyopathy.

Among one hundred and five consecutive patients with pre-excitation (PE) syndrome studied during a 10-year period, eight had an associated hypertrophic cardiomyopathy (HC) (7.62 per cent), eight had a coronary heart disease (7.62 per cent) and nine had a hypertensive heart disease (8.57 per cent). Of the eight patients with HC, four had an asymmetrical form (three of them with an obstructive component), and four a symmetrical form. Seven of these patients had a Wolff-Parkinson-White (WPW) type of PE and the remainder a Lown-Ganong-Levine type of PE. The incidence of paroxysmal tachycardias in

Catheter ablation of the atrioventricular junction: a report of the percutaneous mapping and ablation registry.

An international registry was formed to collate data for patients undergoing attempted catheter ablation of the atrioventricular (AV) junction and insertion of a permanent pacemaker. Over the past 2 years, data was submitted for 127 patients who were followed for a mean of 9.9 +/- 8.2 months. The most common arrhythmia treated was chronic or paroxysmal atrial fibrillation or flutter (78 patients, 61%); the remainder had supraventricular tachycardia due to AV node reentry, ectopic atrial tachycardia, or incorporated an accessory pathway. A single shock of 150 to 400 J was effective in producing

The atrioventricular interval during pre-excited tachycardia: a simple way to distinguish between decrementally or rapidly conducting accessory pathways.

BACKGROUND: Recognition of the presence and role of decremental fibers during wide QRS tachycardia requires carefully executed intracardiac studies. OBJECTIVE: This study sought to determine the value of the atrioventricular (AV) conduction time during pre-excited tachycardia to differentiate a fast from a decrementally conducting accessory pathway (AP). METHODS: Fifty-one patients with 56 pre-excited tachycardias were included in the study: Group I: 27 patients with 31 antidromic tachycardia (ADT) using an atriofascicular pathway, Group II: 2 patients with pre-excited tachycardia due to bys
