[Two cases of Swyer-James syndrome].

Case 1: 55-year-old male was admitted to our hospital for further examination of increased transparency of X-ray in the left lower lung. He had history of pneumonia in his childhood. Left bronchography revealed mild cylindrical bronchi-ectasia in the proximal bronchi but poor filling by contrast in the peripheral bronchi. Case 2: 61-year-old male was referred to our hospital with palpitation and dyspnea. Chest X-ray film revealed hyperlucency of the left lower lung. The ventilation scan showed a marked decrease in ventilation to the left lung and air trapping was present in the left lung. The

Lungworm: (Dictyocaulus arnfieldi) infection in donkeys.

The clinical, parasitological and pathological findings in a group of six donkeys naturally infected with D arnfieldi larvae are described. One animal had to be sacrificed at an early date because it developed pneumonia. The remaining five were unthrifty, showed mild clinical respiratory signs and had heavy strongyle infections. They had varying numbers of adult worms in the airways of the lungs and eggs were found coiled up in the smaller bronchi where they had apparently lead to an obstruction to airflow in that segment. The most striking gross pathological changes were circular discrete are

Medial approach for subcarinal lymphadenectomy during thoracoscopic esophagectomy in the prone position.

PURPOSE: In esophageal squamous cell carcinoma (ESCC), lymph nodes (LNs) around the subcarina and main bronchi are thought to be highly involved. Therefore, complete dissection of these LNs with preservation of the pulmonary branches of the vagus nerves, which control important pulmonary functions, is recommended. The aim of this retrospective study was to investigate the feasibility of reliable method for lymphadenectomy around the subcarina and main bronchi, named the medial approach, during thoracoscopic esophagectomy in the prone position (TEP). METHODS: This was a case-matched control st

Severe tracheal and bronchial collapse in adults with type II mucopolysaccharidosis.

BACKGROUND: Mucopolysaccharidosis type II (MPSII) patients frequently suffer from dyspnoea caused by restrictive airway disease due to skeletal abnormalities as well as glycosaminoglycans (GAG) accumulation at different levels of the airway, including the trachea. In this study we describe the extent of the tracheal and bronchial narrowing, the changes in airway diameter during respiration and the effects of these obstructions on respiratory function in adult MPSII patients. METHODS: Five adult MPSII patients (mean age 40 years) were included. Pulmonary function tests and in- and expiratory c

[Clinical analysis of bronchial foreign bodies in 246 children].

OBJECTIVE: To analyze the characters of bronchial foreign bodies in children and the utilization of bronchoscope in the treatment of bronchial foreign bodies. METHODS: A total of 246 children were diagnosed with bronchial foreign bodies at our hospital during January 2000 until August 2009. Under local mucosal anesthesia, a bronchoscope was inserted through nasal cavity into bronchi. After identifying the site of foreign body, grasping forceps was guided through bronchoscope to remove the foreign body from airway. RESULTS: Among 246 cases, hard nut and skin of melon seed were found (n = 230,

Sphingosine-1-phosphate/sphingosine kinase pathway is involved in mouse airway hyperresponsiveness.

Sphingosine-1-phosphate (S1P) has been shown to regulate numerous and diverse cell functions, including smooth muscle contraction. Here we assessed the role of S1P/Sphingosine kinase (SPK) pathway in the regulation of bronchial tone. Our objective was to determine, using an integrated pharmacologic and molecular approach, (1) the role of S1P as endogenous modulator of the bronchial tone, and (2) the linkage between S1P pathway and bronchial hyperresponsiveness. We evaluated S1P effects on isolated bronchi and whole lungs, harvested from Balb/c mice sensitized to ovalbumin (OVA) versus vehicle-

Sauropus androgynus-constrictive obliterative bronchitis/bronchiolitis--histopathological study of pneumonectomy and biopsy specimens with emphasis on the inflammatory process and disease progression.

AIMS: The histopathology of the Sauropus androgynus (SA)-constrictive bronchiolitis obliterans (BO) is still controversial. A recent report using pneumonectomy specimens showed that the major histopathology was obliterative arteriopathy with segmental necrosis of small bronchi instead of constrictive BO as previously described. METHODS AND RESULTS: We analysed semiquantitatively and immunohistochemically the histopathology of one pneumonectomy and four biopsies specimens of SA-associated lung disease. We found a significant number of constrictive and obliterative bronchioles 1 mm or less in d

Gender differences of airway dimensions in anatomically matched sites on CT in smokers.

RATIONALE AND OBJECTIVES: There are limited data on, and controversies regarding gender differences in the airway dimensions of smokers. Multi-detector CT (MDCT) images were analyzed to examine whether gender could explain differences in airway dimensions of anatomically matched airways in smokers. MATERIALS AND METHODS: We used VIDA imaging software to analyze MDCT scans from 2047 smokers (M:F, 1021:1026) from the COPDGene® cohort. The airway dimensions were analyzed from segmental to subsubsegmental bronchi. We compared the differences of luminal area, inner diameter, wall thickness, wall a

Mechanical properties of the airway tree: heterogeneous and anisotropic pseudoelastic and viscoelastic tissue responses.

Airway obstruction and pulmonary mechanics remain understudied despite lung disease being the third cause of death in the United States. Lack of relevant data has led computational pulmonary models to infer mechanical properties from available material data for the trachea. Additionally, the time-dependent, viscoelastic behaviors of airways have been largely overlooked, despite their potential physiological relevance and utility as metrics of tissue remodeling and disease progression. Here, we address the clear need for airway-specific material characterization to inform biophysical studies of

Pulmonary Aspiration of Brain Matter in a Motor Vehicle Fatality: A Case Study.

We present the case of an adult man who was involved in a motor vehicle accident as a driver. Histological examination of the lung tissue showed the presence of brain tissue within the lumens of the bronchi and bronchioles. Aspiration of brain matter is a rare phenomenon, with only 1 case described in the literature. The finding of brain tissue within the bronchi and terminal bronchioles supports the scenario of a period of agonal respiration with consequent cerebral aspiration. This article adds to the limited literature, distinguishing antemortem or agonal aspiration from postmortem passive

Serial section-reconstruction of anomalous tracheobronchial branching patterns from CT scan images: bridging bronchus associated with sling left pulmonary artery.

The use of the technique of wax-plate serial section-reconstruction, based on contiguous axial plane CT images of the upper thorax, to prepare a replica of the central air-way (trachea and major bronchi) of an infant with sling left pulmonary artery type 2B, with bridging bronchus, abortive right main bronchus, and tracheal stenosis due to absence of the tracheal pars membranacea with "ring" tracheal cartilages is described. The technique is applicable to demonstration of the anatomic features of other abnormalities of branching pattern or caliber of the trachea and major bronchi.

Tetralogy of Fallot with absent pulmonary valve: evaluation with magnetic resonance imaging.

Two infants with tetralogy of Fallot with absent pulmonary valve were studied with electrocardiographically gated magnetic resonance imaging using a 1.5 T imaging system. In each case, imaging was performed successfully. The magnetic resonance imaging clearly demonstrated a rudimentary pulmonary valve, an aneurysmal pulmonary artery, and compression of the mainstem bronchi. Cine MRI revealed phasic compression of the bronchi by an aneurysmally dilated pulmonary artery. Magnetic resonance imaging produced high quality images and provided all of the anatomic information required for preoperative

Modeling the bifurcating flow in a CT-scanned human lung airway.

The inspiratory flow characteristics in a CT-scanned human lung model were numerically investigated using low Reynolds number (LRN) kappa-omega turbulent model. The five-generation airway is extracted from the trachea to segmental bronchi of a 60-year-old Chinese male patient. Computations were carried out in the Reynolds number range of 900-2100, corresponding to mouth-air breathing rates of 190-440 ml/s. Flow patterns on the Re=2100 and flow rate distribution were presented. In this model, the flow pattern is very complex. To count the effect of laryngeal jet on trachea inlet, the trachea wa

Primary tracheal and bronchial lymphoma displayed on 18F-FDG PET/CT imaging.

A 46-year-old woman presented with cough for over 20 days, unrelieved by anti-inflammation treatment. Diffuse wall thickening and mildly luminal stenosis of the lower trachea and both main bronchi were observed on chest CT examination. The pathology after bronchoscopy confirmed non-Hodgkin lymphoma (mucosa-associated lymphatic tissue, MALT). FDG PET/CT was performed to evaluate the extent of the disease, which demonstrated the abnormal FDG accumulation in the diffusely thicken wall of trachea and bronchi, without lymph nodes and other extranodal tissue involvement.

[Chronic obstructive pulmonary disease. A chronic inflammatory disease].

Chronic Obstructive Pulmonary Disease (COPD) is a disease caused by a chronic inflammatory response induced by the inhalation of cigarette smoke or toxic particles/gases in the airways. However, we actually know that COPD is a disease that does not only induce inflammation in lung parenchyma and bronchi, but also provokes systemic inflammation which plays a role in multiple comorbidities. Thereby, treatment of COPD should not only focus on the bronchi to relieve symptoms, improve respiratory function and reduce the rate of exacerbations, but must also be extended to the systemic effects of the

The Lymphatic Spread of Lung Cancer: An Investigation of the Anatomy of the Lymphatic Drainage of the Lungs and Preoperative Mediastinal Staging.

The knowledge of lymphatic spread of lung cancer permitted the study of anatomy of lymphatic drainage of the lungs. The history of anatomy of lymphatic drainage of the lungs began in the 15th century. In the human, pulmonary lymph flows to the lymph nodes around the lobar bronchi and thence to extrapulmonary lymph nodes located around the main bronchi and trachea and its bifurcation (tracheobronchial lymph nodes). These send their efferents to a right and left mediastinal lymph trunks, which may join the thoracic duct, but usually drain opening directly into the brachiocephalic vein of their o

Bronchoperitoneal fistula from a lung abscess.

Fistula formation between the bronchi and peritoneal cavity is extremely rare. In previous reports, fistulas have occurred secondary to thoraco-abdominal trauma, subphrenic abscess, suppurative biliary tract obstruction, malignancy and iatrogenically through procedures such as biliary surgery or percutaneous biliary drainage. The direction of fistula formation has always been thought to be from the peritoneal cavity to the bronchi: there are no reports of a fistula with a bronchial origin. This case report presents a patient who presented with sepsis and a bronchoperitoneal fistula and pneumop

[Tracheobronchopathia osteochondroplastica].

Tracheobronchopathia osteochondroplastica (TO) is a rare disease of unknown etiology affecting mainly the trachea and large bronchi. It is characterized by the presence of multiple submucosal osseus and/or cartilaginous nodules. The authors report a case of 74-year-old woman in whom fiberoptic bronchoscopy, performed because of hemoptysis, revealed typical feature of TO. Besides the typical nodules protruding into the lumen of trachea and main bronchi, a small soft nodule in the larynx was found. On histological examination it was showed to be polyp with regions of inflammation and necrosis. T

Extended sleeve lobectomy for centrally located non-small-cell lung cancer: a 20-year single-centre experience.

OBJECTIVES: Extended sleeve lobectomy (ESL), an atypical bronchoplasty with resection of more than 1 lobe, might be technically demanding but has certain theoretical advantages, including the avoidance of pneumonectomy. However, clinical outcomes after ESL are not well known. METHODS: Between March 1995 and December 2015, 540 patients with centrally located non-small-cell lung cancer underwent sleeve resection. Among them, 63 patients underwent an ESL procedure. We retrospectively analysed those patients in terms of hospital mortality, postoperative complications and local recurrence and comp
