Nd:YAG laser capsulotomy under general anesthesia in the sitting position.

Neodymium-yttrium-aluminum-garnet (Nd:YAG) laser capsulotomy is commonly used to treat posterior capsule opacification after cataract surgery in adults. Children and adults with developmental delay, however, are not always cooperative, and the procedure must be performed under general anesthesia. We describe a technique for Nd:YAG capsulotomy under general anesthesia in the sitting position using the standard widely available Nd:YAG laser. Anesthesia was induced in the supine position and the patient was then transferred to the sitting position with chin in the slit-lamp laser delivery system.

[Pars plana lensectomy II - Short- and long-term complications].

Pars plana lensectomies or membranulectomies were performed on 67 eyes with posterior capsule removal in every case, with follow-up of 60 eyes for more than 3 months (mean: 18 months). No peroperative or early postoperative complication was specifically related to the procedure employed. Irvine's syndromes and retinal detachments were the two most severe late complications. Four of the 5 Irvine's syndromes occurred after a membranulectomy and this encouraged systematic removal of the posterior capsule, knowing the risk of postoperative opacification; 3 of these 5 eyes, in fact, had a final vis

Enhanced PCO prevention of drug eluting IOLs via endocytosis and autophagy effects of a PAMAM dendrimer.

Drug-loaded intraocular lenses (IOLs) have received considerable attention in treating complications that arise after cataract surgery, especially posterior capsular opacification (PCO). However, for a better therapeutic effect, the drug concentration in IOLs usually needs to be increased. Herein, we developed multilayer (doxorubicin (DOX)@polyaminoamide (PAMAM) (D@P)/heparin sodium (HEP))5 modified IOLs, which efficiently enhance the inhibitory effect on PCO using the enhanced autophagy effect of a cationic PAMAM. The chemotherapeutic drug DOX was encapsulated in PAMAM to formulate cationic D

[A case of A3-A3 bypass and proximal clipping for a ruptured left A1 dissecting aneurysm].

A 58-year-old woman was admitted to our hospital with sudden onset of headache. CT and angiogram revealed subarachnoid hemorrhage due to a dissecting aneurysm at the left A1 segment. ACoA was not identified. We performed proximal clipping with A3-A3 bypass for preventing the recurrence of the hemorrhage and for reducing ischemia in the territory of the left ACA. After the operation, CT revealed the infarctions in the corpus callosum, the left medial frontal lobe and the genu of the internal capsule. But the postoperative angiogram demonstrated no aneurysm and the opacification of the recurrent

Micheliolide suppresses epithelial-mesenchymal transition of lens epithelial cells via downregulating matrix metalloproteinase 8 to ameliorate posterior capsular opacification.

OBJECTIVE: Posterior capsular opacification (PCO) is a significant complication that causes decreased vision in cataract patients after surgery. Epithelial-mesenchymal transition (EMT) and inflammation are key factors in PCO development. This study examined the protective effect of Micheliolide (MCL) in preventing PCO and its related mechanisms. METHODS: In vivo studies were conducted using an extracapsular lens extraction (ECLE) model combined with intracameral injection, while in vitro experiments utilized TGF-β2-induced EMT in lens epithelial cells (LECs). In vivo biological effects were as

MyoD is required for the differentiation of Myo/Nog cell progenitors of myofibroblasts in explants of human lens tissue.

INTRODUCTION: Posterior capsule opacification (PCO) is a complication of cataract surgery that impairs vision. Clouding and distortion of the posterior capsule occur as a result of cell migration, deposition of extracellular matrix proteins and contractions of myofibroblasts. The focus of this study is a subpopulation of cells within the lens, called Myo/Nog cells, that differentiate into myofibroblasts in response to wounding. Myo/Nog cells express the skeletal muscle specific transcription factor MyoD, bone morphogenetic protein inhibitor Noggin and brain-specific angiogenesis inhibitor (BAI

Investigating the effect of capsular thickness and material properties on biomechanical and optical parameters in eye models.

The mechanism of accommodation, which defines the shape change of the eye lens in order to provide clear focus over a range of distances, is still not fully understood. The uncertainties in understanding pose an obstacle to comprehending the causal factors in age-related loss of accommodation and in how to design effective accommodating implants to replace lenses with opacification caused by cataract. Finite element modelling has been widely used to study accommodation and can investigate parts of the lens, its capsule and anatomical entities involved in accommodation that are obscured by the

Epithelial-Mesenchymal Transdifferentiation in Pediatric Lens Epithelial Cells.

PURPOSE: Posterior capsule opacification (PCO) is a complication after cataract surgery, particularly in children. Epithelial-mesenchymal transition (EMT) of lens epithelial cells, mediated by transforming growth factor beta (TGFβ), contributes to PCO. However, its pathogenesis in children is poorly understood. We correlated cell growth in culture with patient characteristics, studied gene expression of pediatric lens epithelial cells (pLEC), and examined the effects of TGFβ-2 on these cells in vitro. METHODS: Clinical characteristics of children with cataracts correlated with growth behavior
