Concomitant therapy in people with epilepsy: potential drug-drug interactions and patient awareness.

People with epilepsy (PWE) may use prescription and over-the-counter (OTC) drugs for the treatment of concomitant diseases. Combinations of these drugs, as well as dietary supplements, with antiepileptic drugs (AEDs) may lead to reduced control of seizures and of coexisting medical conditions and increased risk of adverse drug reactions (ADRs). The aims of this study were to obtain comprehensive lists of medications, dietary supplements, botanicals, and specific food components used by adult PWE and to evaluate the potential for interactions involving AEDs and patients' awareness of such poten

Prevalence and predictors of potential drug-drug interactions in Regione Emilia-Romagna, Italy.

BACKGROUND AND OBJECTIVE: Drug-drug interactions (DDIs) are preventable medication errors associated with potentially serious adverse events and death. Several studies have examined the prevalence of potential DDIs among ambulatory patients in various countries. Limited recent data on the prevalence of potential DDIs in Italy are available in the medical literature. The objective of this study was to estimate the prevalence of clinically important potential DDIs among the approximately 4 million residents of Regione Emilia-Romagna (RER), Italy, and to examine possible predictors of potential D

Drug-drug interactions associated with second-generation antipsychotics: considerations for clinicians and patients.

OBJECTIVES: While not always clinically significant, patients with schizophrenia may be at risk for drug-drug interactions (DDIs) with second-generation antipsychotics. Second-generation antipsychotics are increasingly being used in a broader population of patients and, therefore, for those with comorbid illnesses, adjunctive treatments, or other diagnoses, the clinical significance of DDIs is increasing. This paper reviews currently available data concerning DDIs that occur between second generation antipsychotics, and other medications or substances, when metabolized by the cytochrome P-450

Identification of severe potential drug-drug interactions using an Italian general-practitioner database.

OBJECTIVE: To analyze prescriptions in a general-practitioner database over 1 year to determine the frequency, the characteristics, and the monitoring of the severe potential drug-drug interactions (DDIs). METHODS: We retrospectively analyzed the clinical records from 16 general practitioners in the Veneto region, an area in northern Italy. The study covered the period from January 1 to December 31, 2004. We selected all severe and well-documented interactions according to the book Drug Interaction Facts by David S. Tatro (Facts and Comparisons, St. Louis, MO, 2006). We grouped severe potenti

CYP3A4 substrate selection and substitution in the prediction of potential drug-drug interactions.

The complexity of in vitro kinetic phenomena observed for CYP3A4 substrates (homo- or heterotropic cooperativity) confounds the prediction of drug-drug interactions, and an evaluation of alternative and/or pragmatic approaches and substrates is needed. The current study focused on the utility of the three most commonly used CYP3A4 in vitro probes for the prediction of 26 reported in vivo interactions with azole inhibitors (increase in area under the curve ranged from 1.2 to 24, 50% in the range of potent inhibition). In addition to midazolam, testosterone, and nifedipine, quinidine was explore

Potential drug--drug interactions in 5,125 mostly elderly out-patients in Gothenburg, Sweden.

Of 5,125 mostly elderly out-patients, average age 78.2 years, 1,594 (31%) had at least one interacting drug combination according to the Swedish National Formulary. On average, each patient with drug-drug interactions (DDIs) had 1.6 DDIs. There was no difference in the sex or age between those having DDIs and those without. However, patients with one or more DDIs used a significantly larger number of drugs than those without DDIs, on average 8.1 versus 5.2. This significant difference was independent of age and sex. When classified for clinical significance according to the handbook "Drug Inte

Factors associated with knowledge, attitudes, and practices regarding food-drug interactions and drug-nutrient interactions among clinical nutrition specialists in Saudi Arabia.

BACKGROUND: Food-drug interactions (FDIs) and drug-nutrient interactions (DNIs) can significantly affect therapeutic outcomes and patient safety. Clinical nutrition specialists play a key role in identifying and managing these interactions; however, their knowledge, attitudes, and practices (KAP) regarding FDIs and DNIs remain insufficiently explored in Saudi Arabia. METHODS: A cross-sectional study was conducted among 353 licensed clinical nutrition specialists across Saudi Arabia. Data were collected using a validated self-administered questionnaire. Three separate multivariable linear regr

[Interactions of statins with antithrombotic drugs].

Due to a wealth of scientific clinical data, statins are being extensively used for the primary and secondary prevention of ischemic events. Therefore, statins play a major part in the pharmacological treatment of cardiovascular disease. Even before it became apparent that statins are effective despite normal baseline lipid levels, it was known that statins exert additional effects on the vascular wall that are independent of lipid lowering and protective toward the arteriosclerotic plaque. Some of these pleiotropic effects are related to hemostasis: through profibrinolytic, antiadhesive and a

Tolterodine-warfarin drug interaction.

OBJECTIVE: To report two cases of warfarin therapy in which the addition of tolterodine resulted in prolonged international normalized ratios (INRs). CASE SUMMARY: Two patients, each receiving warfarin for stroke prophylaxis in association with chronic atrial fibrillation, developed adverse effects after the initiation of tolterodine for urinary disorders. Other medications for concurrent medical diagnoses had remained unchanged. One patient had an episode of prostatitis, which was treated with levofloxacin immediately prior to tolterodine initiation. The warfarin dosage had remained constant

Pesticide-drug interaction in rats.

The possible interactions between a fungicide, thiram, and some drugs acting on the central nervous system were investigated in rats by means of behavioral methods. Potentiation was found after combined treatment with thiram and promethazine, and after thiram and meprobamate. In the case of trihexyphenidyl, an additive interaction was observed. It is presumed that these observations will be of practical toxicological significance.

Ototoxicity and noise-drug interaction.

Diverse therapeutic agents, such as certain antibiotics, diuretics, and analgesics can cause functional and morphological damage to the inner ear. Noise present in recreational or occupational environment may also be potentially damaging to the auditory system when combined with some ototoxic therapeutic agents. This article documents recent research and controversies about the site and mechanism of action of major ototoxic drugs, noise-drug interaction, and possible prophylaxis against such ototoxicity.

Pharmacokinetic interactions of antiepileptic drugs.

The problem of antiepileptic drug interactions is significant in that many epileptic patients are treated with multiple drug therapy. Moreover, patients may also be receiving additional medication for other concurrent disorders. Most drug interactions are pharmacokinetic, involving changes in absorption, protein binding, metabolism, or excretion. As a result, plasma levels of the antiepileptic drug may decrease leading to exacerbation of seizures. Alternatively, plasma levels may rise resulting in toxic side effects. Similar changes may also occur with drugs given for other disorders. In this

Interactions of analgesics with other drugs.

Antipyretic analgesics, such as salicylates, acetaminophen, and pyrazolones, are often given concomitantly with a variety of other drugs. Drug interactions that occur at the receptors are known as pharmacodynamic interactions; alterations in absorption (bioavailability), distribution (plasma protein-binding), and elimination (renal excretion, hepatic metabolism) are termed pharmacokinetic interactions. For example, antacids and food both delay the absorption of analgesics. Highly protein-bound drugs (such as phenylbutazone, phenytoin, or warfarin) can compete with the common binding sites of s
